POSITION TITLE: Adult Day Services Licensed Practical Nurse
ADULT DAY POSITION SUMMARY: The primary function of the Licensed Practical Nurse in Adult Day Services is to deliver skilled nursing services, under the supervision of a Registered Nurse, according to the established plan of care, following established department philosophy, policies and procedures.
ACCOUNTABILITY: The Adult Day Services Licensed Practical Nurse reports to the Adult Day Services Coordinator &/or Nursing Department Manager.
WORKING CONDITIONS/PHYSICAL REQUIREMENTS: Ability to follow Adult Day Services staffing patterns by working scheduled hours. Flexibility in scheduling of hours required to accommodate changes in census and needs of clients.
PRINCIPLE DUTIES & RESPONSIBILITIES: Encourages maintenance of appropriate diet to ensure proper nutrition Maintains knowledge of supplies and their usage Utilizes proper body mechanics and promote patient and employee safety through appropriate use of assistive devices. Assists in planning activities and events for clients.
Prepare monthly plan of daily programs. Utilize established guidelines for monthly planning. Utilize available resources for activity planning. Complete monthly activity calendar.
Participates in daily activity plan of clients. Perform delegated job functions under direct supervision of Adult Day Supervisor and/or Registered Nurse Consultant.

POSITION TITLE: Registered Nurse Consultant
POSITION SUMMARY: A registered nurse shall provide consultation and review of the health services at least monthly.
QUALIFICATIONS: Graduate of an accredited Registered Nursing program Holds a current licensure to practice as a Registered Nurse in Minnesota Current Basic Life Support & First Aide Certification Possesses verbal and written communication skills to effectively communicate with center staff, physicians, interdisciplinary team members, and patients/families.
PRINCIPLE DUTIES & RESPONSIBILITIES: Monitoring participants’ health status and reporting changes to the participant’s caregiver and physician and the center director; Educating and counseling participants on good health practices Maintaining a listing of professional health resources available for referrals as needed by participants; Developing policies and monitoring procedures for participant self-administration of medications for training unlicensed personnel who provide medication assistance; and Supervising staff distribution of medication and assistance with participant self-administration of medication and ensuring compliance with part 9555.9680, subpart 2, item C.
