Partner OrganizationsThe partners of C4HP, each with their own core competency, are co-located near the corner of Elden St. and Alabama Dr. in Herndon. They are:• Cornerstones (Managing Partner)• Computer CORE • Empowered Women International• HealthWorks for Northern Virginia• Herndon-Reston FISH• Literacy Council of Northern Virginia• Just Neighbors• Vecinos Unidos• Fairfax County
• Use a collective impact approach to bring together capacities of nonprofit, government, corporate, faith, education, and community residents to achieve the vision.
• Integrate health and human services with a focus on children, youth, and adults.
• Leverage our core operating capacity which currently includes 9 human-services partners and over 20 collaborative programs co-located in Herndon.
• Establish a robust data collection system that feeds into a constant process of evaluation.

Occurs when actors from different organizations and sectors commit to a common agenda for solving specific social or environmental problems

3 Priority Goals/Objectives Identified• Goal #1: Services Navigation and Integration (Completed)
– Individuals and families access a coordinated range of service providersObjective:
Assist individuals and families in accessing preventive services and supports before crisis occurs
• Goal #2: Better Health Outcomes with Social Service Supports• (PLANNING STARTED)
– Adults access a primary medical home and receive comprehensive careObjective:
Provide assessment and coordinated care to adults with chronic conditions like diabetes and hypertension
Target: Maintain or Improve the HBA1c level for 50 pre-diabetic patients who receive full array of relevant social service supports. Compare informally to improvement of 50 patients not accessing full array of social service supports.

– Adults secure stable employment (IN PROCESS)Objectives: Adults from C4HP Partner programs…
1. Focus on pathway employment, “filling the skills gap” in three sectors: Healthcare, IT and accounting/finance
2. Assist adults in reducing barriers to employment and acquiring 21st century workplace skills required by local employers
3. Assist adults in obtaining job readiness, career assessment, search and placement support, as well as entrepreneurial support.
4. Work through cross-sector Advisory Work Group

How do we evaluate impact?• Cornerstones is embarking on Year 2 of a grant with Urban
Institute/World Bank on measurement and evaluation
• Urban Institute Technical Assistance has helped C4HP partners look at processes and outcome data more closely ultimately resulting in a better picture of the impact of C4HP on the clients and the community we serve.
• Cornerstones received the pro bono assistance from Canvas Solutions Inc. to develop electronic intake and assessment forms which can be deployed on mobile devices. This platform has improved our data collection requirements.
• Cornerstones received pro bono assistance from Upper Quadrant to develop cloud-based solutions that enable better data reporting and analysis. This platform has increased our capacity to measure program impact on clients we serve.
HealthWorks for Northern Virginia Comprehensive Health Care
• HealthWorks Herndon provides on site behavioral health services – case management, therapy, psychiatry
• HealthWorks provides access to comprehensive oral health care in Leesburg center
• HealthWorks Herndon and Leesburg have dedicated counselors to provide education and enrollment assistance to patients eligible for health insurance Marketplace
• Purposeful navigation and linkage ensure successful utilization, coordination of care, and evaluation – Services Navigation and Community Case Mgrs. at C4HP Office have access to electronic medical records where doctors can make referrals for social service needs, social service notes go in medical record, and health and social determinants are addressed in an integrated partnership.

• Clinical outcomes improve when psychosocial stressors identified and addressedExample: Patient becomes compliant with medical treatment and keeps appointments when enrolled with transportation assistance
• Utilization of community services improves when client is healthyExample: Patient who is feeling better attends job training and ESOL classes
Public-Private Partnerships
• Within HealthWorks, Fairfax County delegated staff ensure seamless assessment and linkage to and from County services
• Public Health Liaison Nurse• Assessment Therapist, Community Services Board• Access to Designated Eligibility Worker
• Multiple public and private wrap around resources available at 2nd floor Connections for Hope Partnership office and across street at HNRC
