• Use a collective impact approach to bring together capacities of nonprofit, government, corporate, faith, education, and community residents to achieve the vision.

• Leverage our core operating capacity which currently includes 9 human-services partners and over 20 collaborative programs co-located in Herndon.

• Establish a robust data collection system that feeds into a constant process of evaluation.

– Individuals and families access a coordinated range of service providersObjective:

Assist individuals and families in accessing preventive services and supports before crisis occurs

• Goal #2: Better Health Outcomes with Social Service Supports• (PLANNING STARTED)

– Adults access a primary medical home and receive comprehensive careObjective:

Provide assessment and coordinated care to adults with chronic conditions like diabetes and hypertension

Target: Maintain or Improve the HBA1c level for 50 pre-diabetic patients who receive full array of relevant social service supports. Compare informally to improvement of 50 patients not accessing full array of social service supports.

2. Assist adults in reducing barriers to employment and acquiring 21st century workplace skills required by local employers

3. Assist adults in obtaining job readiness, career assessment, search and placement support, as well as entrepreneurial support.

How do we evaluate impact?• Cornerstones is embarking on Year 2 of a grant with Urban

Institute/World Bank on measurement and evaluation

• Urban Institute Technical Assistance has helped C4HP partners look at processes and outcome data more closely ultimately resulting in a better picture of the impact of C4HP on the clients and the community we serve.

• Cornerstones received the pro bono assistance from Canvas Solutions Inc. to develop electronic intake and assessment forms which can be deployed on mobile devices. This platform has improved our data collection requirements.

• Cornerstones received pro bono assistance from Upper Quadrant to develop cloud-based solutions that enable better data reporting and analysis. This platform has increased our capacity to measure program impact on clients we serve.

HealthWorks for Northern Virginia Comprehensive Health Care

• HealthWorks Herndon provides on site behavioral health services – case management, therapy, psychiatry

• HealthWorks provides access to comprehensive oral health care in Leesburg center

• HealthWorks Herndon and Leesburg have dedicated counselors to provide education and enrollment assistance to patients eligible for health insurance Marketplace

• Purposeful navigation and linkage ensure successful utilization, coordination of care, and evaluation – Services Navigation and Community Case Mgrs. at C4HP Office have access to electronic medical records where doctors can make referrals for social service needs, social service notes go in medical record, and health and social determinants are addressed in an integrated partnership.
