SCIENTIFIC ADVANCEMENT MATERNITY HOSPITALS2021
PROF. BRIAN CLEARY – CHIEF PHARMACIST & JENNIFER DONNELLY, CONSULTANT OBSTETRICIAN AND THEIR MULTIDICIPLANARY TEAM IN THE ROTUNDA HOSPITAL FOR THROMBOCALC.
Brian, Jennifer and their MDT, developed an electronic risk assessment tool called Thrombocalc which calculates a simple risk score and makes recommendations on thromboprophylaxis based on each woman's VTE risk factors.
Through collaborative research and quality improvement, Thrombocalcwas embedded in routine care in the Rotunda Hospital, leading to increased compliance with risk assessment, appropriate prevention and a trend to reducing the number of blood clots.

Sile Gill ADON, Dr. Samah Hassan & the Maternity Team in Mayo University Maternity Hospital
Sile & Samah led a QI program to monitor the hospitals practice regarding VTE prophylaxis in mothers and to implement measures to increase compliance.Introduction of a specially designed preforma, visual aids for staff & mothers 'Do you Know your VTE Score' and teaching sessions for doctors and nurses. The service was audited in January and then July 2021and showed a huge increase in compliance with VTE Risk Assessment.

VTE INCIDENCE DATA DASHBOARD
➢ Developed in 2018 the VTE dashboard showing VTE incidents and burden of HAT. This gives real-time VTE incidence paterns, which is crucial to VTE improvement, service provisions and advancement of clinical research. It is the first of its kind in Ireland and inpatient episodes are extracted, processed and uploaded for analysis.
➢ Management & teams at group and hospital level, now have line of sight of VTE activity, raising VTE awareness and local initiatives.

VTE PROPHYOLAXIS DASHBOARD
➢ Using Meg audit tools, an app was developed with the help of VTE Ireland, Prof. Fionnuala Ni Ainle, Dr. Barry Kevane & Dr. Tomas Breslin to audit VTE Prophylaxis efficiently.
➢ This App has enabled SVPH to develop a quality of service and improve patient safety. It identifies areas that are underperforming and areas that continue to improve.
➢ Auditing of VTE Prophylaxis is reported in real time and can be acted on immediately.
➢ The long term vision is that it would be used in all Irish hospitals and that all data would be collected into a National System.

➢ VTE patient satisfaction of 98%
➢ As an admittance avoidance initiative a DVT nurse led service was set up in 2019 with referrals from ED or directly from GP's. Out of 2100 seen to date only 2% have been admitted and 78% are discharged from clinic within 2-4 hours.
➢ In 2020 a virtual nurse led clinic was set up to review and discharge patient with superficial Thrombophlebitis reducing attendance to the anticoagulation clinic.
➢ Ongoing education of staff, patients and public in prevention and treatment of VTE.
➢ Plans to set up a stand alone Thrombosis unit with a Warfarin Clinic, DVT and low risk PE management. This will include an initiative surrounding the psychological impact of VTE on Patients health and well being.

They are the first group nationally to determine VTE incidents and the burden of HAT events within their hospital group.

Jacinta has developed Prophylaxis Protocols for the anti natal and post natal periods.
She tirelessly, educates Patients & staff about the importance of risk assessment in pregnancy and post natal period.
She runs a nurse led Thromboprophylaxis clinic and ensures Patients receive appropriate prophylaxis.
Jacinta is a legend in the National Maternity Hospital and she goes above and beyond to offer support to her patients with an at risk pregnancy.

Catriona, constantly strives to ensure Patients are anticoagulated in a safe and efficacious way. She runs an anticoagulation review clinic and she also runs a Thrombosis Clinic.
She is a leader in educating Patients and fellow clinicians.

Nominated by her Patients, Sarah has a passion for VTE and is determined to see a VTE service evolve in Beaumont. She follows her patients from diagnosis to discharge and ensures that they go home with their unknowns answered and their safety concerns talked through.
Her leadership and passion for VTE shines through in her care for her patients.
She constantly provides VTE Education to hospital colleagues in innovative ways.

CLINICAL LEAD, NATIONAL MEDICATION SAFETY PROGRAM HSE
Ciara led the recent National Collaborative Project which demonstrated that significant improvements can be achieved in the area of Hospital Acquired VTE when a systematic evidence based approach to VTE Risk Assessment and prescribing is adopted. This work led to the publication of the first National Guideline for VTE Prevention in Irish Hospitals.
Ciara worked with Thrombosis Ireland to create and distribute our HSE approved Blood Clot Alert Card to all Acute Hospitals and more recently to introduce our VTE safety message in our Maternity Hospiital's through the NWIP.
