Newsletter Fall 2006 Volume 6, Number 1 University of Ottawa Department of Anesthesiology As Christmas approaches the University of Ottawa Advanced Institute of Medical Education, under the Directorship of Dr Meredith Marks, is getting ready to set up the Simulation Center and is hiring PhD Educators. The Department of Anesthesia has committed to support the funding of the PhD Educators for a defined period of time. With a number of individuals com- mitted to the simulator (including our re- cent recruit Dr George Dumitrascu) and to medical education, the Department of An- esthesia is in a position to contribute greatly to these education initiatives. I would like to encourage the individuals interested in medical education (along with the Department at large) to become actively involved. Educational activities are occurring at many levels: undergradu- ate, postgraduate, simulation, undergradu- ate curriculum reforms, or in the unique University of Ottawa e-Curriculum initia- tive. Any individuals with questions re- garding the Department’s involvement in medical education should feel free to con- tact me. Following is a brief summary of the De- partmental happenings. There continues to be external pressures from the Wait List Initiative Ontario to get more cases done. The recent round of submissions of Ex- pression of Interest on Anesthesia Care Team Demonstration Site has just been completed. I believe all institutions in Ottawa have made their submissions and are awaiting the decisions. In the mean- time, the search process for the Chief of Anesthesia at CHEO is still on-going. Dr Philip Mossdorf is the Interim Chief. A review of the Division of Cardiac Anes- thesia has been completed and the report is in. Dr Robblee has been continuing as the Chief of Cardiac Anesthesia and a process to finalize this is underway. University of Ottawa Department of Anesthesiology

Finally, it was with great pleasure that I attended the “Living Legends” awards ceremony on August 19, when the World Society of Cardiothoracic Sur-geons bestowed the Living Legend award to our Professor Emeritus Earl Wynands for his lifetime achievements. He shares the awards among the likes of DeBakey and Cooley. It was an honour for Earl to receive such an award but also an honour for our entire specialty when our surgical col-leagues recognize one of us. Please join me in con-gratulating Earl.

I hope everyone had a great summer. Mine was excellent. I took 3rd place in my 1st bridge tournament (novice category) and had a wonderful trip to Turkey – a destination I would highly recommend. Sun, sand & history – and more “Greek & Roman” sites than either Greece or Italy. It is also wrapped up in a lot of Biblical history so, all in all, it’s just fascinating. Despite what you read, we had no trouble at all driving our-selves around, though we sensibly avoided driving in Istanbul. If we’d have been REALLY sensible, we would have avoided some of the Istanbul taxi drivers too!! Perhaps the award for “Most Interesting Answer to: What I Did on My Summer Vacation/Elective” must go to our PGY-4 Natalie Dupuis. She came back from a summer session at the International Space University in Strasbourg with great photos, stories and even interesting information for us as clinicians. She even got to meet such famous astronauts as Buzz Aldrin. We are starting a new academic year with high expectations and a record number of residents, at 48. So far, our first two Journal Clubs and Visiting Professor Lectures have had an ex-cellent turnout. Keep it up! The Mentor Dinner was another successful social event, once again at Allium. Our Mentor Pro-gram continues to be a strong asset of the residency program, highly valued by both staff and residents alike. We have sev-eral new mentors this year to cope with the increased number of mentees. The new rotations of Vascular Anesthesia & Anesthesia for Remote Locations are proving valuable and will continue. The residency program is a fluid, ever-changing entity – all sugges-tions are gratefully received! Recently, the program went through a mini-review at the inter-nal level. Although we are not scheduled for a formal external Accreditation Review until 2010, the University of Ottawa holds periodic reviews to check that the programs are on track. It’s a very useful exercise for all concerned, especially Holly and I going through the process for the first time. The residents get the opportunity to talk frankly about the program without any anesthesia staff present, which is helpful for them. I await the report with interest. We welcome Dan Power, who joined the program in September as a member of NDMC undergoing retraining in Anesthesiol-ogy. He follows in the footsteps of Jo Po, Rob Johnston & Leilani Doyle. Although Dan is from the east coast of New-foundland and Dr. Cathy Gallant is from the west coast, they discovered many friends in common!! Ilia Charapov is doing a great job in keeping the website up to date. You will have noticed that much of the information is up there – I encourage everyone to check it out if you haven’t done so.

We welcome new members to the RPC. Joanna Nawrocka has taken over as Resident Site Coordinator at CHEO & Donna Nicholson fulfills the same role at the Heart Institute. We thank Philipp Mossdorf (now Acting Site Chief at CHEO) and Bernie MacDonald (devoting more time to research) for their sterling work. Now that we have 10 residents shared with NOSM, the Chief Resident from the Northern Stream also sits on the RPC. At the moment this is Chris Hudson. Yvette Grabowski is also a new addition to the RPC, representing not only NOSM but also resident research interests. We are now entering CaRMS season, culminating in interviews February 12-16th 2007. It’s an inspiring time of year, meeting the medical students who will be the anesthesiologists of tomor-row. We have been allocated a 5th CaRMS spot this year. As always, I would like to mention the amazing work of Lynne McHardy & Holly Ladouceur. Few people realize just how much work goes on behind the scenes. Have a great fall. Soon the gloomy days of October/November will give way to the brilliantly sunny but bitterly cold days of winter. Enjoy!

valuable assistance from Dr. Ken Fong, the bugs have been ironed out of the “time-away” submission page. The site is being continuously updated with various bits of news, Core presentations, as well as Wednesday morning rounds. The CARMs information section has also been updated in time for the Fall CARMs deadline. The call stipend dispute – a sore issue since July 2006 – is cur-rently in the hands of PAIRO and the Council of Academic Hospitals of Ontario (CAHO). I am pleased to tell you that PAIRO has agreed with our position on the issue and will lobby CAHO on our behalf. We are pushing for retroactive changes and $100 per weekend call. I have been re-assured by PAIRO that this dispute will not be put on a back burner and should have its resolution well before the current agreement expires in 2008. Our third annual camping trip in July 2006 was again a huge success. It was organized by Stephane Moffett, Sanj Gupta, and Ilia Charapov and held at Fitzroy Harbor Provincial Park. We had over 30 residents in attendance, a great number of PGY-1’s, and guest appearances by many staff. Patron Kevin Gagné (B.Sc,MD,FRCPC) presided over the opening night of the camping trip, which will remain forever etched in the memories of those who attended. The weather was perfect, the swim in the Ottawa River was refreshing, and several soccer matches revealed a few of our very own Pele’s and Mara-dona’s. We had a great time. The mentor dinner took place in September at Allium restaurant – a repeat winner from last year. The food was scrumptious and the place was filled with boisterous socializing and mentor-ing. Organized by Dr. Tammy Barrows, this event was well attended by staff (mental workers) and residents (their men-tees). A big “Thank you” to all our mentors who have volun-teered for this program. Tania Di Renna is busy creating her final masterpiece in her trilogy “The Christmas Rounds”. It will be her last movie crea-tion before she hangs up the director’s hat next year in prepara-tion for the “year-end-interview” in 2008. The ‘on-location’ shooting is well underway. If you see somebody taking your picture or aiming a video camera at you – it’s likely too late to be nice, so try to be ‘yourself’. This way it will invariably come out natural and funny. Congratulations are in order - Jenn Backstrom and Ian Ozard were married this fall. Mohammed Hassan and Ali Namazie are expecting new additions to their families in the near future. A great Fall to all!

Chief Resident’s Report

The new academic year is once again upon us. As the new chief resident I would like to welcome all of the new GP-anesthesia residents as well as the new PGY-1’s and 2’s who have joined our program. On behalf of all the residents, I would like to thank outgoing chiefs Anna Wyand, Gavin Gracias, and Rob Johnston for doing a great job over the past year - a hard act to follow. As Abeer Arab (our associate chief and General Cam-pus rep) and Shawn Hicks (CHEO rep) would attest, the initia-tion into the chiefdom went very well. The outgoing chiefs pa-tiently took us through the paces, the rules, and the Swiss bank accounts that hold the chief residents’ embezzlement fund... A big “Thank you” to Anna, Gavin, and Rob. There have been a few organizational changes this year. Resi-dent representation on the Residency Planning Committee has been expanded to include the CHEO and Sudbury chief residents (Chris Hudson and Yvette Grabowski). The Educational Direc-tor position was finally established and Dr. Patti Murphy is guid-ing us toward the final interview (ahem, that would be Royal College Exam). Holly Ladouceur is our new Resident Coordina-tor and assisstant to the Program Director. She took on the chal-lenge with enthusiasm and everybody is very happy to have her on board. The new research rep this year is Yvette Grabowski, who enthusiastically has taken upon herself a tough mission of resuscitating residents’ participation in academic research. She will need a lot of help from all of us. The academic year started off with the Pharmacology Core. Serene musings on the subject of pharmacodynamics, benzene

rings, and context sensitive half-times will no doubt bring down the level of the post-summer excitement amongst residents and prepare us for a few months of serious study and work…. The PGY-1’s are continuing their introductory core which started this summer. Senior residents and staff continue their participation in the creation and presentation of the PGY-1 core - it has been a lot of fun for both the presenters and the PGY-1’s. The PGY-4 oral exam prep session just took off with a lot of help from the Program and Dr. Gavin Gracias. The PGY-5 half days are well underway – and the list of review topics seems daunting… We are successfully continuing with a two-resident call system at the Civic and split weekend call at the General Campus. Renovations are underway at the Civic campus to create a 2nd resident on-call room close to OR. The PGY-1 buddy call has been going well as well – a tribute to both the PGY-1’s and their senior resident mentors. The Program website, www.ottawa-anesthesia.org has been dusted off and spruced up over summer - thanks to the initiative of Drs. Gavin Gracias, Anna Wyand, Paul Bragg, Patti Murphy, and Linda Wynne. This year’s annual camping trip took place on July 15, 2006. It was held at Fitzroy Provincial Park, west of Kanata-Stittsville. There was very good attendance, with a mix of junior and senior residents, along with a few staff members. Some people came by for an afternoon visit, while others pitched up their tents for a couple of nights. In terms of activities, some people played Frisbee and soccer, some went swimming in the lake and some just sat around, relaxing and consuming beverages (for hydration, of course). Whatever the chosen pace, an excellent time was had by all who attended. A great big thank you to Stephane Moffatt, who organized the trip. Those of you who didn’t make it out this year should seriously consider going next summer. You won’t regret it! Lucie Filteau

The 51st Annual OMA CAS Meeting (now called The Ontario Anesthesia Meeting) was held at the Royal Botanical Gardens in Hamilton again this year. Anesthesiology Section executive members have been working hard to improve the meeting and to make it a Canadian meeting of choice. I think we've succeeded. Ask any of the folks who went from TOH: Drs Chris Wher-rett, Mike Curran, Dave Ewing, Don Wilson, Rick Hladkowicz, Sanjiv Gupta and Tania Di Renna, to name a few. There were over 300 registrants again this year. The meeting had over 20 hrs of some of the cheapest CME you can find, along with nearby wineries, all within a reason-able distance from Ottawa. All of the academic centres in Ontario now contribute speakers, with quality talks on diverse topics as well as workshops on Airway Management and Ultrasound (for vascular access/nerve blocks). It's also a great opportunity to meet old friends and past residents from the Ottawa program, find out what's going on in different parts of the Province, check out new job opportunities, and literally take time to "smell the roses" if you want a break from the CME. Sanjiv Gupta has volunteered to become the OMA CAS resident representative as we look to improve our communication with the resident anesthesiologists of the province. It's not easy to balance work and the few hours of free time any of us have, but the reality is it's political in-volvement that determines our work environment. The Section on Anesthesiology is the political lobby voice that represents us amongst our peers and with government. We need your input to stay effective. Next year's meeting is in Niagara Falls, so book early to avoid disappointment! Give it a try - you will be pleasantly surprised.

The annual scientific meeting of the Canadian Pain Society will be held in Ottawa on May 23-26, 2007. The motto for the meet-ing is: Pain relief, a basic human right. A number of department members have agreed to serve on the local arrangements committee: John Penning Cathy Smyth Ellen Thompson (Chair) Linda Robinson Mary Redmond Sue Madden, nursing education Ed Fitzgibbon, palliative care Roman Jovey The Education Day at the start of the meeting is the responsibil-ity of the local committee. Our goal is to put together a day of excellence with key speakers, all leaders in their fields. We hope local medical students, trainees from all relevant special-ties (including ours) and any local physicians (specialists or gen-

Mentorship Dinner

The dinner was a huge success and I would like to thank Dr. Wynne, our Program Director, for her continued support of the mentorship program. I encourage all residents to take full ad-vantage of their mentors over the next year. Your mentors are a wonderful resource of knowledge, have a lot of experience upon which to draw for advice, and are always supportive.

The mentorship program kicked off this year with the annual Mentorship Dinner at Allium Restaurant on September 25th. This was a very challenging year as the number of residents increased to forty-nine and hence the need for new mentors in-creased. As usual, our staff came through for us and we were able to find mentors for all of the residents. Thank you to all of the new mentors who signed up!!

Alan joined the department at the Civic Campus in July 2006. He started his career as a pharmacist, having worked in several places including Toronto, Ottawa, Buffalo, London Fort Erie, Welland, Etobicoke and St. Catharines. He then proceeded to do his medical school and residency training at the Uni-versity of Toronto. During his residency, Alan also obtained a Master’s De-gree in Health Research Methodology from McMaster University. Alan’s research activities have been mainly in the areas of health economics, pharmacologic studies (especially clonidine in recent years), and post partum headaches. His extracurricular activities have included being Chair of the student section of the OMA, the Ontario rep for the CFMS, as well as being on the editorial board of the U of T Medical Journal. Alan is happily married and has three children: Natalie (8yrs) and twins Julie and Eric (4yrs). On his spare time, Alan managed to obtain his certification in open water scuba diving. He is an avid runner and has participated in several marathons.

Susan Goheen joined the group at the Civic Campus in September 2006. She grew up in Port Hope, Ontario. Prior to her anesthesia training, she obtained a Degree in Math and in Physical/Health Education, both at Queen’s University. This was followed by a Bachelor’s Degree in Education at the University of Western Ontario. A few years later, Susan began her medical education at Dalhousie University. She then pursued her Anesthesiology Residency and an Obstetrical Anesthesiology Fellowship at the University of Manitoba. Once Susan completed her training, she worked and taught in Winnipeg for a couple of years, followed by a year in Toronto. She then worked for 4.5 years in Barrie, On-tario and was a ROMP preceptor (Rural Ontario Medical Education Program), holding an academic appointment at McMaster University. Susan was involved in hypothermia studies during her residency, which led to 2 publi-cations as well as television coverage by CTV’s Canada AM, the Discovery Channel and CBC’s Health Show. Her more recent research activities have been in the field of Obstetrical Anesthesiology. Susan is married to LCol (Dr.) Kevin Goheen, who is a Prosthodontist in the Canadian Forces. They have three children: Ben (16yrs), Casey (14 yrs) and Josh (12 yrs). Her hobbies include golfing, curling, skiing, jogging and swimming.

after Cervical Spine Trauma, appeared in the June 2006 issue. It was selected for the Anesthesiology CME Pro-gram, which allowed readers to earn credits by reading the article and taking a test on the mate-rial.

Wedding Announcements

Anna Wyand and Martin Kreuser were married on August 26, 2006 in Petite Riviere, Nova Scotia. According to several attendees, an excellent maritime experience was had by all.

Ottawa alumnus Michael Law and his fiancé Margaret Campbell were wed on October 14th, 2006 in Vancouver, BC. A number of Ottawa grads attended Mike and Maggie’s wedding celebrations (see below). Mike has finished his cardiac fel-lowship in Seattle and is back working at the Royal Columbian Hospital in New Westminster, B.C.

Braedan Thomas Neilipovitz was born on September 7, 2006. Dave states that he peed on a nurse, just for his daddy. Shahd Alsefaou was born on September 23, 2006. Abdualla tells us that her name means Honey in arabic. Naomi Wei Chiu Kemp was born on September 28, 2006. Big sister Amelia has been reading her stories and Toby is warmly embracing a houseful of girls. Abdumalik Hassan was born in October, 2006. Kim Walton and Dave Eibner are happily expecting their first baby in May. Congratulations to all!!

A number of things have been going on at the undergraduate level. The number of students continues to increase, making quality education an ongoing challenge! We have tried to sup-plement intraop teaching with more formal Power Point lectures (with voice over) covering most of the students’ core learning objectives. For the past year, the students have also been start-ing the anesthesia rotation off with an orientation afternoon, consisting of an airway lecture, a hands-on airway workshop and a simulation session. The feedback on these educational activities has been excellent. Many residents have been in-volved in this workshop and their help has been invaluable. We are also seeing more intraop teaching being done by the residents as we are booking students with senior residents more often. This has allowed us to accommodate the higher number of students as well as give the anesthesia residents more oppor-tunities to teach (a CanMeds role that has been difficult to evaluate to date). On the evaluation side of things, the student evaluation card at the Civic Campus has been updated and is in print. The staff evaluation cards, filled out by the students at both Campuses over the past year, have provided us with some very useful in-formation. It has given us the opportunity to recognize excel-lence in teaching at the undergraduate level and a new teaching award was developed this year to reflect this. Congratulations to Dr. Rob MacNeil, who was awarded with the first Under-

graduate Teaching Award this past spring at the Gary Johnson Research Day Dinner. As part of a nationwide initiative to quantify the students’ clini-cal exposure/ experience, the students are now being required to log their various activities on a daily basis. This will eventually tie in to an overall learning portfolio, which they will continue into residency and ultimately staffhood (for CME documenta-tion). An anesthesia undergrad web site is being developed, with the help of the medical I.T. department at the University. This will contain orientation information for all of the sites, all lecture material, evaluation forms and rotation feedback forms. Finally, the entire undergraduate curriculum is undergoing a facelift. The management of acute and chronic pain has been identified as a topic that is currently not adequately covered. A Multidisciplinary Working Group has been formed to examine this issue and make recommendations to the University as to how to improve coverage of this important area. Thankfully, anesthesia has been selected to head this working group and Drs. John Penning and Ellen Thompson rose to the challenge. Hopefully, our increasing involvement at the grassroots level will have a positive impact in the years to come…

Alumni News—Aviva Rappaport (GP Anesthesia)

Hi everyone, Hope all is well with you. We have no recent family photos, however I have included a re-cent photo of Jessica. She was born July 14, 2005 and was 14 months old in the attached pic-ture. I married Hugh Stewart a year after finishing the GP anes-thesia year (hence the name change) and Jessica followed soon after. We are living in Ottawa and I work part time in the OR in Pembroke and part time in the ER at CHEO. I get nostalgic for the friendly Civic Hospital anesthesia group every time I run into an anesthesia resident doing their rotations through the CHEO ER :)

Dr. Lionel Busque

Le 15 septembre, 2006 Chère Claudette et membres de la famille Busque, Monsieur l’abbé Soucy, Chers amis, dear friends and colleagues, Suite à David, qui nous rappelait les activités profession-nelles et la contribution de Lionel à la vie du département d’anesthésie de l’hôpital Général, j’aimerais vous présenter brièvement une facette de la vie de notre ami que mal-heureusement peu d’entre nous ont connu, sauf évidemment les membres immédiats de sa famille. Art of Literature, and had been since a very young age,

possédant une maitrise churchillienne de la langue française. You may remember the poem that he had written for our

common colleague Michel Tousignant and read in similar

circumstances as today. Mais il en est bien d’autres à son répertoire qu’il me fait souvent plaisir de relire tel Le Vieux Moulin et Les Glaces dont je prendrai la liberté de vous lire quelques rimes dans un instant. Dans son poème Les Glaces il se sert typiquement et habille-ment des mots les plus précis de la langue française pour décrire le glissement des glaces sur le fleuve St Laurent à la hauteur de Québec, pour nous les faire entendre et oui, sentir, et les voir s’acheminer et se bousculer enchevêtrées vers un océan lointain. Il termine par un analogie à nos vies trop éphémères, à nos départs souvent subits et imprévisibles et par un témoignage de sa foie et de sa croyance en l’au delà. Permettez moi donc de vous lire les quatre dernières stro-phes du poème Les Glaces, de Lionel Busque:

Mais ou donc allez vous minuscule banquise,

Qui sans cesse fuyez sous l’oeuil morne des cieux?

Pourquoi voguer ainsi dans le souffle des brises

Ou le calme infini des grands flots silencieux?

Vous ètes des humains la véritable image,

O glaçons qui passez pour ne plus revenir!

Et l’homme, de ses jours, tourne l’ultime page,

Lorsqu’il se croit encore au seuil de l’avenir.

Oui, glissez lentement vers l’abime des ondes

On September 11, 2006, we lost a beloved member of our anesthesia community. Dr Lionel Busque, well known to all of us here in Ottawa, passed away suddenly at the age of 74. Dr. Busque was the founder of the Anesthesia Department at The Ottawa Hospital and practised medicine for over 40 years. He is survived by his beloved wife of 43 yrs, Clau-dette Trudel, his three sons (Christian, Pierre-Michel and Carl) and six grandchildren. Included over the next two pages are the eulogy delivered in his honour by his colleague and friend, Dr. Benoit Samson, as well as photographs recently taken at Dr. Busque’s retire-ment party.

If you are reading this editorial then you are probably one of those dedi-cated newsletter fans that likes to read this thing from cover to cover. I thank you for your enthusiasm and am hoping that at least a few of you out there have contemplated being more involved in this publication than just being an avid reader. It is no surprise that a publication such as this takes a tremendous amount of work. I know of nothing else comparable that is being put out by a single person as an after-hours endeavour. There are usually editorial boards, or a single editor that has the opportunity to work on this stuff during daytime hours. Truth be told, it is simply not sustainable long term when it is created in the wee hours in lieu of sleep…That is why you are currently reading the Fall issue just before Christmas! So then, I am looking for volunteers, staff and/or residents, who are inter-ested in joining the new “editorial board” and participating in the creation of this newsletter. Your involvement may be in the form of event cover-age, submission seeking, proofreading, photo editing and/or layout design. You can stick to one task or try different things out, depending on your areas of interest. No experience whatsoever is required. You simply need to bring your enthusiasm and I will “show you the ropes”. Send me an email if you are considering it and we can talk, no obligation!
