Go to the garage elevator on any level of the Hospital Parking Garageand take elevator to Level L, the lobby level. From the lobby, followthe main hallway back to the Heart Center Elevators. Take elevator to3rd floor. On exiting, turn right and follow the blue wall and signs forRegistration C/ACU. (Heart Center, 3rd floor)

1 Emergency Department (ED) Patient Access/Pre-Admission Testing

For the ED, enter the main vehicle entrance and bear left, followingsigns for EMERGENCY. For Patient Access or Pre-AdmissionTesting, follow directions to the Main Lobby. Enter the first door tothe left of the Information Desk.

2 Arrhythmia Center

Arrhythmia Center parking is limited to patients only. All others should use the Hospital Parking Garage and follow directions to thefront of the Heart Center Main Lobby. Then follow signs to the Emergency Department (ED). The Arrhythmia Center is adjacent to the ED (Heart Center, first floor).

4 Endoscopy Registration

Go to the garage elevators on any level of the hospital parking garage and take the elevator to Level L, the lobby level. From the Lobby level gopast the Information Desk and enter the door on the right marked Pre-Admission Testing for Endoscopy Registrations.

6 Physician Offices

Most physician offices are located in the Robert F. Vizza Pavilion. Enterthe main vehicle entrance of the hospital and bear right. Parking forthese offices is located along the north side of the campus. See the directory at entrance of the Pavilion for office locations.

5 Health Information ManagementMedical Records (lower level)

To receive a copy of your medical record, please contact the MedicalRecords Department at (516) 562-6085 to obtain a HIPAA complaint request form. (Heart Center, lower level)

7 Heart Center Main LobbyInformation Desk

Offers visitor assistance with information, a comfortable waitingarea, public phones and restrooms. Patio Café, Gift Shop, ATMs,and Meditation Chapel are all adjacent to the Main lobby. (Heart Center, first floor)

8 DeMatteis Surgery LobbyInformation Desk (lower level)

Offers visitor assistance with information, a comfortable waitingarea and restrooms. (DeMatteis Pavilion, lower level)

9 Registration AAmbulatory SurgerySurgical Services (lower level)

Enter hospital through the DeMatteis Surgery Entrance (see B). Stop at the Information Desk and you’ll be directed to Registration A.(DeMatteis Pavilion, lower level)

10 Registration B

Go to the garage elevator on any level of the Hospital parking Garageand take the elevator to Level L, the lobby level. From the lobby level gopast the Main Information Desk to the door marked Registration B. Register here for Radiology, Cardiac Imaging, Outpatient Clinical Laboratory and Pulmonary Medicine.

11 Heart Valve Center

The Center is located on the Ground Level of the Vizza Pavilion, inSuite G01. (Vizza Pavilion, Level G)

12 Outpatient LaboratoryLaboratory Patient Service Center

Enter the main vehicle entrance and bear right. Park along thenorth side of the campus. Enter the Vizza Pavilion from the NorthEntrance. The lab is at the end of the hall. (Vizza Pavilion, Level G)
