How do you log onto Vision?
What is Vision?
Vision is a web-based reporting tool that positions members of Trilogy, the Clinically Integrated Network (CIN), to further integrate patient care, target improvement opportunities in preventative care of their patient population, and deliver increasing value to the marketplace.

Navigating Vision
Landing Page
If you are a practice manager, but wishto view what your provider sees, youcan switch views. This is accomplishedby clicking the down arrow next to yourname in the top right corner.
The Measures module provides a detailed breakdown of each eligible measure including: count of eligible population, prior results, and percentcompliance by quartile.
Selecting the patient detail hyperlink shows all eligible patients, and whether they met the measure at both the measure and submeasure levels. The patient’s name is a hyperlink to the patient’s file.
If you want to review the patient’s file, click on the patient’s name. This will send you directly to the patient’s profile without having to manually search for the patient.
HINTAccording to the role you are assigned, the landing page provides a snapshot of activities and performance of the practice, or the provider’s population.
HINT When viewing a measure, if you wantto view the complete measure definition, click on the hyperlink entitled,“View Quality Measure Documentation”. This will open a new window with the definition, the inclusions, and attribution criteria.

The Patient module yields a wealth of information including:
• Recent encounters (office, ED, and inpatient)

Correspondence by letter or email can also be generated, if a patient has overdue care elements.
