1. Sharing Experiences With SCI/Stroke/Brain Injury People And Family

Basically, I would like to share my experiences of being disabled since Aug 2004, with SCI people / their family and those with Stroke / brain injury. You are welcomed to visit my home, to see how I cope at home. My caregiver [FDW] is also willing to share her experiences with your caregiver. I can be contacted @mobile phone 94245354 [Richard].

I go to Rehab Centre twice/thrice weekly. I would chat with SCI / stroke patients / family n family of brain injury person after my session.

This would make it easier, so that someone from Jurong/Woodlands/Yishun etc, don’t need to come all the way to the East of Singapore, where I Am.

TetraplegiaWorkGroup [TWG] hopes to help them along.

Tetraplegia Workgroup started in 1999, a non-profit voluntary group functioning under the umbrella of Friends of Tan Tock Seng Hospital (Singapore). Aim to cater to the needs of te traplegics in the workgroup and most importantly to instill and foster a sense of "Family Spirit" into the workgroup.

Objective is to befriend tetraplegic persons and assist in meeting their medical, social, vocational and physical needs by linking with existing services available. Most of these persons are home-bound; being constantly surrounded by 4 walls is only one of their enclosures. This is achieved through the process of organising more events, outings and recreational activities for the tetraplegics hoping to form a network of support to one another. 6. Vendors of Equipment/Medical Supplies / Pressure Relief Mattress At Annex B, is a list of vendors where you can buy homecare equipment, like bed, wheelchair, hoist, pressure relief mattress and other medical supplies, like catheters, pampers etc.

This will provide a better choice / cost when buying equipment / homecare needs. Prior to discharge, the therapist will inform you of the equipment that you need.

7. Therapy After Discharge From Hospital –

If the hospital doesn’t provides therapy after your discharge, for whatever reasons,

You can contact SPD or HWA as they provide therapy services, please see para 8 and 9. Community Hospitals also provide therapy, pse see list of Community Hospitals at para 10.

SPD , HWA and Community Hospitals require a referral letter from your Rehab doctor. SPD , HWA and Community Hospital also require a copy of the patient’s ‘discharge summary’, which can be obtained from the hospital, prior to being discharged.

SPD Charges : -Subsidies are given depending on household income. SPD charges $54.50 for therapy [before subsidy]. SPD provides transport at $14.20 if you can’t travel to SPD by public transport.[before subsidy] -SPD has ‘stair-crawler’ service for those living on a ‘non-lift landing’ floor and using a standard wheelchair. HWA Membership - People with physical disabilities are welcome to join HWA as a member to participate in its activities and benefit from its welfare programs. There are three categories of membership:

Ordinary Membership Life Membership Associate Membership

Membership Ordinary membership shall be opened to persons with physical disabilities and who are Singapore Citizens within the age group of 16 to 60 years old at the time of application.

Conversion to Life Membership shall be opened to Ordinary Members with at least 5 continuous years of membership. Conversion is based on application and is subjected to approval by the Executive Committee and upon payment of $100.00 (one-time payment only) as membership fee.

Associate Membership shall be open to all other persons with physical disabilities within the age group of 16 to 60 years old at the time of application and who are not Singapore Citizens.

1. Will present members who turn 61yo be allowed to continue as HWA's members?

HWA’s REPLY : Yes, as long as their membership is approved before the age of 60 years. Existing members could enjoy their membership benefits till the last day of their lives.

2. In view of people living to an older age, could HWA raises its membership age to 70yo? This way 61yo to 70yo PWDs can join HWA. I do meet quite a few PWDs in their early 60s at TTSH Rehab Centre.

HWA’s REPLY : Any change of membership entry criteria would require constitutional amendments and a longer process. HWA is considering all such suggestions received regularly from members and the public. However, for the time being, if you meet any person interested on HWA programme and services, please inform him that all HWA services are available for both members and public. Non-members could access these services as a public client.

3. Does HWA makes exceptions for younger PWD to join HWA? Presently has a 13yo SCI boy at TTSH Rehab who wants to join HWA.

HWA’s REPLY : HWA has implemented a new beneficiary member category for youths between 12 years to 16 years. Please contact our membership department for details.

10. Therapy at Community Hospitals Community Hospital provides intermediate care facility to facilitate smooth transition from hospital to community-based services or at home. The core services of a community hospital are rehabilitative, sub-acute, and convalescent and respite care for the sick, that requires longer inpatient stay but do not require acute hospital care. Care Cab Club The Care Cab Club promotes better understanding among taxi drivers and people with disabilities. The taxi-driver is paid the fare according to the meter. BIZLINK Centre Singapore empathises with the views expressed by Mrs Koh Soek Ying ("Review age limit for special needs schools"; June 20), who asked for more support to enable people with special needs to continue learning and growing after leaving special needs schools.

Using the Life Cycle Approach, disability services should be as seamless as possible for users and their caregivers.

Ideally, a special needs child should be able to transit from early intervention programmes to special education schools, then advance to vocational training and be eventually placed in open employment, sheltered employment or day activity centres.

Bizlink is a registered charity whose mission is to help fulfil the employment needs of persons with disabilities (PWDs).

As one of the leading employment centres for PWDs, Bizlink has placed more than 6,000 PWDs into open employment and created sheltered employment in our structured community environment for a generation of PWDs. The cohort of PWDs in our community today ranges from ages 16 to 82.

PWDs are first assessed by a team of allied health professionals from our vocational assessment division to better understand their social backgrounds, employment needs and residual abilities to right site open or sheltered employment services for them.

Bizlink is well-placed to be the employer of choice for PWDs who are not quite ready for open employment. We are one of the largest employers of PWDs in Singapore, hiring 140 PWDs and 50 other disadvantaged persons through our sheltered production workshop and seven social enterprises, such as our food and beverage, cleaning services, and floral and hampers enterprises.

Our PWDs in sheltered employment form a holding pool of special needs labour resource who, after an appropriate period of job hardening and skills training, can be eventually placed in open employment if they so choose. They can also choose to stay in long-term sheltered employment in the Bizlink community, with some finding their spouses and setting up their own families within this community.

The Foreign Domestic Worker (FDW) Levy Concession for Persons with Disabilities Scheme

enables families to pay a lower monthly levy of $170 [instead of $265] when employing full-time caregivers to help look after their loved ones with disabilities.

Each household is also only eligible for levy concessions for up to two FDWs at any one time, including those hired with levy concessions pertaining to young children or elderly persons.

Who

The person needing care must be a Singapore Citizen or Singapore Permanent Resident aged ≤12

and/or ≥65 and certified by an appointed medical practitioner as requiring assistance in at least one

Activity of Daily Living*.

The person needing care should be the employer of the FDW or related to the employer – as the child,

grandchild, spouse, sibling, sibling-in-law, parent, parent-in-law, grandparent, or grandparent-in-law. If

the person needing care is related to the employer, he/she should be living with the employer at the

same NRIC-registered address.

Once all the above conditions are fulfilled, the employer of the FDW will automatically qualify for the

levy concession under the Ministry of Manpower (MOM). Please apply directly with MOM.

The Foreign Domestic Worker (FDW) Grant is a monthly grant of $120 given by the Ministry of Health to provide more support to families who care for the frail elderly.

Eligibility

To qualify for the FDW Grant, you need to meet all of the following criteria:

Citizenship

As an employer of the Foreign Domestic Worker (FDW) for your care recipient:

• If your care recipient is 65 years old and above, he or she must be a Singapore citizen or a Permanent Resident (PR). If your care recipient is a PR, then you must be a Singapore citizen If your care recipient is younger than 65, he or she must be a Singapore citizen As a care recipient and an FDW employer:

• You must be a Singapore citizen Relationship with care recipient

You must be legally related to your care recipient (i.e. child, spouse, grandchild, sibling, sibling-in-law, parent, parent-in-law, grandparent or grandparent-in-law) You must live at the same address as your care recipient, as reflected in your NRIC

Household income

Through Means-Testing, your per capita monthly household income must be $2,200 or less If your household has no income, the Annual Value of your property must be less than $13,000

You do not need to undergo Means-Testing if you have successfully applied for the Community Health Assist Scheme (CHAS), Senior’s Mobility Fund (SMF) or Interim Disability Assistance Programme for the Elderly (IDAPE) in the past two years. You simply need to send the Agency for Integrated Care (AIC) a photocopy of your CHAS card, SMF delivery order and/or IDAPE approval letter.

Your care recipient must have at least moderate disability as assessed by a Singapore-registered doctor. The doctor will complete a Functional Assessment Report which you have to send to AIC

You can also refer to the list of Appointed Medical Assessors if you are unable to find a suitable doctor. A medical assessment IS NOT required if your care recipient has any of the following documents:

• Approval letter for ElderShield or IDAPE • Doctor certification of mental incapacity, Lasting Power of Attorney and Donee Card • Doctor certification of mental incapacity and Court Order for Court Appointed Deputy • A memo or document from any Singapore-registered doctor certifying that your care recipient is bedridden Please refer to the FDW Medical Assessment Guide for more information on what is considered moderate disability and the medical assessment.

Caregivers’ training

• Your FDW must have attended the relevant AIC-approved caregivers’ training courses. Click here for the list of FDW Grant training courses.

You can apply for the Caregivers Training Grant (CTG) to offset the cost of the training. Other than the FDWG training courses, any other caregivers’ training will be considered on a case-by-case basis.

The Caregivers Training Grant (CTG) provides caregivers with subsidies to attend training that builds their capabilities

in caring for the physical, social and emotional needs of the care recipients. Caregivers can use this annual training

grant of $200* for every dependent under their care, regardless of their income level, to attend training programmes.

They can choose from a mix of home-based or classroom-based courses offered by the Agency for Integrated Care

(AIC)’s pre-approved training providers. As AIC continues to engage more training partners, caregivers can look

forward to even more caregiver training options including e-learning courses.

While the grant can be used to offset the training fees, caregivers still need to co-pay $10 when they register for

courses under the CTG. The $10 is paid to the training provider. It serves to ensure that caregivers attend the

course they have registered for in order to maximise their learning opportunities.

Centre for the Physically Challenged launched [Straits Times Published on Feb 25, 2012]

The Centre for the Physically Challenged (CPC) was officially launched on Saturday morning.

The CPC provides a programme which aims to help those who have acquired disabilities during their productive years. Since its inception in December 2010, the centre has helped 64 people to find job placements successfully.

7The centre, a collaborative effort between the Handicaps Welfare Association (HWA) and Abilities Beyond Limitations and Expectations (ABLE), aims to provide a holistic programme comprising rehabilitation, transportation, skills training and job placement for people with physical challenges. More than 150 clients have since benefitted from the programme.

'The primary focus of the centre is to offer assistance to adults whose physical shortcomings presented themselves during their productive years and have since hampered their capacity to retain employment as well as to generate a steady flow of income,' said Mr Quek Khor Ping, chairman of the CPC.

How to Apply for CHAS?

To qualify for CHAS, you must be:

Singapore citizens aged 40 and above or disabled (i.e. unable to do any of the following six activities of daily living (ADLs) without assistance: washing/bathing, toileting, transferring, feeding, dressing, and/or mobility);

and meet the following income criteria:

� Per capita household monthly income is $1,500 and below for economically active households; or � Annual Value (AV) of residence as reflected on NRICs is $13,000 and below for economically inactive households.

Members who are on Public Assistance (PA) scheme will also be eligible for CHAS subsidies.

How do I apply?

If more than one family member from the same household is applying for CHAS, only one duly completed & signed application form is required to be sent to P.O. Box 680, Bukit Merah Central Post Office, Singapore 911536, with:

� clear copies of NRICs (front and back) of the main applicant and all family members, and/or � clear copies of birth certificate(s) if NRIC is not available for family members below 15 years old

Please also include foreign identification documents for submission if your family members are not Singapore citizens.

How do I go for disability assessment? (Applicable to Disabled Applicants Only)

After we receive your completed CHAS application, a Functional Assessment Report will be mailed to you if you meet the income criteria. With the report, yo0u will then need to undergo a disability assessment conducted by any Singapore Medical Council-registered doctor. You will have to bear the cost of the disability assessment as part of the application process. You are advised to check with the clinic prior to your visit, whether the doctor is willing and able to conduct the assessment. You may also wish to check on the fees charged. Alternatively, you may wish to approach any of our preferred medical assessors.

Functional Assessment Report has to be mailed back to P.O. Box 680 Bukit Merah Central Post Office, Singapore 911536.

A copy of the Functional Assessment Report that will be issued to you and a list of preferred medical assessors can be found under the Enquiries/Downloads page.

How do I know if my application is successful?

If your application is successful, you will be issued a Health Assist Card, which is valid for 2 years.

Your CMB/Health Assist card is only for your own use and cannot be used by family members.

If you lose the card, please inform AIC at 1800-ASK-CHAS (1800-275-2427) and a replacement card will be sent to you.
