This usage guide describes the basic operation procedures of using the e-Questionnaire (e-Q) to provide the required information for the 2011 Population Census (11C). Further information about how to use the e-Q can be found on the 11C website (

http://www.census2011.gov.hk) or obtained by calling the 11C enquiry hotline at 2153 2011 (Service hours: 9:00am – 10:30pm from 23 June 2011 to 2 August 2011 (Monday to Sunday, including public holidays).

First time user

2.1 Activate account

l Enter the e-Q system by clicking the related link from the homepage of the 11C website (

l Click the “First Time User” under “Login” on the left of the e-Q homepage

Note: A unique Screen Code is assigned to each screen of the e-Q (e.g. the Screen Code of the e-Q homepage is “SCR-0102”). When you encounter problems in using the e-Q, you can quote the relevant Screen Code for enquiries.

l Enter the Household Reference No., Household Account No. and Activation Key, which are printed on the short form questionnaire

Step 3:

l Enter the verification code shown on the right of the input box (The verification code comprises only alphabets and is not case sensitive. You can click the icon to listen to the sound clip spelling out the verification code.)

l Click “Login”

l Check whether the address shown is the unit of quarters you are residing in or in charge of. If yes, select the first choice “The address information has been checked. I want to activate the account now.”, and then click “Next”

Step 4:

Note: Address confirmation is part of the login process. You will be required to re-enter the Household

Reference No., Household Account No. and Activation Key as well as the verification code if you click any button other than “Next” on this page.

2.2 Register for using the e-Questionnaire Step 1:l Read the Terms and Conditions

Note: The version of “Terms and Conditions” shown above may be slightly different from the final version

posted onto the e-Q website for registration.

l Fill in the information of contact person

l Fill in the household information l Click “Continue” to proceed to the next page

Notes: (1) The Local Contact Phone No. and Email Address provided here will be used for (i) authentication in case you forget your password and apply for account re-activation, or (ii) follow-up by staff of the Census and Statistics Department on the e-Questionnaire content. These two data fields are mandatory.

(2) For a few questions, there is an icon shown at the end of the question (see the “Household Information” section on the page above). You may click this icon and a pop-up window will appear to show you the explanatory notes for this question.

(3) The e-Q system has built in some validation rules to check the answers provided. If an icon is shown and you cannot proceed to the next page after clicking “Continue”, you will need to

amend the incorrect answers or fill in the missing information. In some cases, a dialogue box will be displayed to ask you to confirm the answers before proceeding to the next page.

l The name of contact person is pre-filled as the household head. If the household head is another household member, please revise the information accordingly.

l Fill in the information of other household member(s)

l Click “Continue” to proceed to the next page

Notes: (1) Click “Add” or “Remove” to add or remove a row for a household member if needed. After

doing so, you may be required to click “Back” to return to the previous page to update “the number of household members” or “the number of other persons who are in the unit of quarters at 3 a.m. on 30 June 2011” so that the total number of persons provided there equals the number of person records on this page.

(2) Pull-down menu is often used in the e-Q system. Because of space constraint, the descriptions of a few choices in the list may be too lengthy that they cannot be fully displayed like the one for “Relationship to the Household Head (HH Head)” on this page. For each of these pull-down menus, a list of choices is given at the bottom to show the full descriptions.

l Input a login password (The password

must comprise both alpha and numeric characters with length of 8 to 12

l Choose whether to remove all questionnaire data from the e-Q in case account re-activation is required (e.g. password forgotten or account frozen after 5 consecutive unsuccessful logins)

l Click “Confirm” to complete the registration and proceed to the next page

Note: The default option is to have all questionnaire data erased in case of account re-activation so as to provide full protection to your information.

l Choose whether to start form filling right away Step 5:

l Click “Continue” to proceed to the next page

Note: If you want to login the system after registration, you will need to click “Existing User” on the e-Q

homepage. For details, please refer to Section 3.1.

2.3 Fill in the e-Questionnaire

l Click “Go” to fill in the information of the household member selected

Step 1:

Notes: (1) For household members with form filling not yet completed, the “Status” column will show

“Pending Completion”.

(2) If you want to leave the e-Q system, please click “Logout” on the top right corner. If you close the browser without clicking “Logout”, your account will still remain in the log-in mode and the system will block all log-in attempts until time-out after inactivity for 20 minutes. In that case, you should take a break and re-enter the e-Q system after 20 minutes.

l Fill in or select the answers according to the question order Step 2:

l Check the answers provided after form filling for a household member

l Click “Back” to amend the answers if needed l Click “Finish” to finish form filling for that household member after confirming the answers

l Continue to fill in the information of other household members

Note: After finishing form filling for a household member, the “Status” column will show “Completed”.

Nevertheless, you still can click “Go” to amend the information of that member if necessary.

2.4 Submit the e-Questionnaire

l Click “Submit” after finishing form filling for all household members Step 1:

l System reminds you that the questionnaire data cannot be retrieved or changed through the e-Q system once the e-Q is submitted

l Click “OK” to confirm e-Q submission

l After e-Q submission, an Acknowledgement Reference No. will be generated for your future enquiry

Step 3:

l Click “Save” to save a copy of this page in your computer, or click “Print” to print this page for record

l Click “Logout” on the top right corner to leave the system

Note: After e-Q submission, you cannot login the e-Q system again. If you want to amend the information provided, you may call us at 2153 2011, quoting the Acknowledgement Reference No. and stating which items you want to amend. Telephone enquiry service hours: 9:00am – 10:30pm, from 23 June 2011 to 2 August 2011 (Monday to Sunday, including public holidays).

Existing user

3.1 Login

l Click the “Existing User” under “Login” on the left of e-Q homepage Step 1:

l Enter the Household Reference No., Household Account No. and password Step 2:

l Enter the verification code shown l Click “Login”

3.2 Fill in the e-Questionnaire

The steps of filling in e-Q by Existing User are similar to the First Time User. Please refer to Section 2.3 for details.

3.3 Submit the e-Questionnaire

The steps of e-Q submission by Existing User are similar to the First Time User. Please refer to Section 2.4 for details.

You may apply for account re-activation in case you have forgotten the login password or when the account is frozen after five consecutive login failures.

4.1 Request account re-activation

l Enter the Household Reference No. and Household Account No.

l Enter the Local Contact Phone No. and Email Address you provided in registration for account authentication

l Click “Re-activate”

l After reading the system message confirming receipt of your application for account

re-activation, click “Home” on the top right corner to leave the system

Note: For data security reasons, the new Activation Key will not be delivered by email. Instead, it will be

sent by post to your unit of quarters using the address kept in the records of the Census and Statistics Department. Please keep the Household Reference No. and Household Account No. for account re-activation. These two pieces of information will not be given in the letter on new activation key.

4.2 Re-activate account After receiving the new Activation Key, you can re-activate the account by clicking “First Time User” on the e-Q homepage and entering the new Activation Key together with other required information. You may refer to Sections 2.1 and 2.2 for details.
