Getting started in your new termIf you are entering a GP term in 2016.2, don’t forget there are a number of important activities that need to take place in the first few weeks of the term starting:

Practice orientation

When you start in new practice, it is important you are properly orientated. To help practices orientate registrars to a practice, we have developed some orientation resources which registrars can also access. Head to GPRime and look under ‘Forms, Documents and Links’ – just search ‘orientation’.

Direct observation

To help supervisors assess your competence and level of support required, we strongly recommend supervisors spend some time observing your consultations in the first few weeks of term.

Discuss your learning plan needs and the supervision plan with your supervisor

Within the first three weeks of term your supervisor will spend time reviewing your learning needs to help you build your learning plan.

Supervision plan

As part of this process, your supervisor will complete a supervision plan and consider how to assess your current level of competence, especially with regard to high risk or red flag areas. The aim of the plan is to identify areas requiring closer supervision to optimise training and minimise the risk of adverse outcomes for patients and registrars.

The supervision plan covers a comprehensive range of presentations and procedures that are common to general practice. There are two distinct sections:

1. A section which lists potentially high risk presentations. These require review by a supervisor, initially before the patient leaves the room, and then at the end of the day as the registrars competence and confidence in managing these important presentations increases.

2. A list of presentations and procedures which may be signed off without first being observed, depending on the level of previous experience and confidence of the registrar in dealing with these issues.

Pre-Term Assessment Report (PTAR)

If you have already commenced in a GP term, your supervisor will have reviewed your Pre-Term Assessment Report (PTAR) prior to you starting. The PTAR is a summary of your training. It includes your contact details, a history of your placements, and all the competency assessments completed on you during GP community based terms, such Clinical Teaching Visits (CTVs) and competency assessments completed by your other supervisors. Please note: If you joined GP Synergy from another RTP, information will only appear from 1 January 2016 onwards.

You can access your PTAR at any time in GPRime – go to your ‘Trainee Portfolio’ and click on the ‘Summary tab’.

Ongoing in-practice requirements – at a glanceA summary of all the in-practice training requirements can be found in the ‘In-practice training requirements at a glance’ document available in GPRime (and also attached to the rear of this document). Remember – to access the resource links you will need to be logged into GPRime.

REMINDER: Mandatory ALS / CPR / Emergency skills course requirements

RACGP

CPR

• It is mandatory that registrars complete a CPR course within 12 months of the start of their first GP term.

ALS

• The RACGP requires registrars complete an approved ALS course within 4 years of applying for fellowship.

ACRRM

Registrars enrolling in ACRRM vocational training from 2016 onwards must successfully complete:

• Rural Emergency Skills Training (REST) by the end of first year Primary Rural and Remote Training and

• One further ACRRM accredited Tier 1 course or two accredited Tier 2 courses.

Those with limited or no intrapartum care experience must successfully complete a REOT, ALSO or CRANA course. REOT, ALSO courses would count towards EM course requirements described in 3.4. Fellowship Handbook.

Registrars enrolling in ACRRM vocational training prior to 2016 must successfully complete either:

• Two ACRRM accredited Tier 1 courses; or

• One ACRRM accredited Tier 1 course and two accredited Tier 2 courses.

The emergency medicine courses referred above must have been undertaken within 10 years of Fellowship (maximum training time allowed as defined in Training Time Policy). These courses must be accredited by ACRRM for vocational training.

The registrar is required to ensure that at least one course covering the Advanced Life Support (ALS) skills and knowledge has been completed within three years of Fellowship.

ReCEnT is an educational and research program in GP vocational training, centered on an ongoing cohort project of registrars’ in-practice clinical and educational experience. Registrars complete encounter forms, recording details of 60 consultations, in each of their GP training terms. These take around 1-2 minutes to complete per form (i.e. up to 2 hours in total each term).

What happens to the data collected?

After completion of the forms, the data is collated and registrars receive a feedback report that should be used as a tool to aid reflection on practice. Reflection can be facilitated by comparing the registrars’ data with other registrars, with established GPs, as well as with the registrar’s results in previous terms. Critical evaluation is encouraged when reading through the report – was this a typical week? Are the results unexpected? Am I different from other registrars? If so, why might that be? Am I getting an adequate range of clinical presentations for optimal learning (and exam preparation)?

Additional Uses of the ReCEnT data

ReCEnT data is also used to provide Regional Training Organisations (RTOs) and medical educators with information on registrars’ learning needs, to evaluate the effectiveness of teaching/education sessions, to provide ‘contextual’ data to illustrate individual teaching sessions (e.g. in a session on acne, how often will you see acne as a registrar?), and as a basis for registrar and supervisor research projects.

It is also being used to ‘map’ the content of GP registrars’ in-practice clinical and educational experiences in much the same way as BEACH mapped the content of Australian general practice in the past decade and a half. This provides rich information on which to base education and training approaches.

Activities of interest

CareTrackKids

CareTrackKids is an NHMRC Partnership project run by Jeffrey Braithwaite’s’ AIHI group looking at the appropriateness of care delivered to children with 16 common conditions https://www.caretrackaustralia.org.au/.

The project is being done in NSW, SA and QLD and is looking to engage more GPs in NSW. It has been approved by the RACGP for 40 category one CPD points.

ANU - How and Why Seminar

After the very successful event last year, the ANU medical school and ACT Health are co-hosting another seminar on “The How and Why of Clinical Research”. If you’re a clinician interested in research but not sure how you can make it work, come along to hear how senior clinicians have developed their research career.

CA Competency Assessment A competency assessment is an assessment of specific competencies that a registrar gains relevant to their stage of general practice training. GP Synergy uses a competency grid to provide a framework for giving feedback to registrars about their progress in training. Competency assessments are used by the supervisor and clinical teaching visitor as part of their formative assessment process and requires direct observation of registrar consultations.

TA Training/Teaching Advisory contact A formal contact between a registrar and GP Synergy medical educator which is recorded on GP Synergy’s online training management system, GPRime2.

Training Review Meeting

Re-enrolment All registrars must re-enrol prior to a new term commencing (this includes registrars going on leave, Extension Awaiting Fellowship, hospitals . Re-enrolment generally opens bi-annually in March and Aug/Sept.

Term placement All registrars seeking GP terms must participate in term placement which generally opens prior to a new term commencing in April / Oct. Only enrolled registrars can participate. For more information please see Term Placement Information for Registrars on GPRime2.
