REQUEST FOR PROVISIONAL FACILITY APPROVAL

The ____________________________________________ Fire Department/FireProtection District hereby requests that the Office of the State Fire Marshal, Division ofPersonnel Standards and Education, recognize their fire station as a Provisional TrainingFacility in the State Training and Certification Program.

We hereby agree to provide the following criteria:

1. A fire station that serves as a classroom;

2. A pumper apparatus equipped in accordance with NFPA No. 1901;

3. A current set of IFSTA Manuals (Red Books);

4. Improvise to provide all learning experiences required in the“Certified Firefighter” course; and

5. Keep complete approved records based on the Training Record Jacket and Course Approval outline.

Provisional schools may not receive fees for offering training for certification to a personwho is not a member of their fire department; they may, however, offer approved trainingto members of other departments for certification.

This Division will approve a facility as unlimited for a regional training school if it is inpossession of or has immediate access to all of the resources required for unlimited facilitycertification as enumerated on the “Application for Certification as an Unlimited FireService Training Center”.
