Dr. Gabriel Ariola Wins the Bernard P. Tillis Award for Exceptional WritingFor the third time in five years, a Second District member has been honored as the winner of the Bernard P. Tillis Award for excellence in dental writing. Dr. Gabriel Ariola was selected to receive the award, presented by the NYSDA Council on Membership and Communications, for his essay “Purple Heart,” which appeared in the October 2011 Bulletin of the Second District Dental Society.

His winning essay is a positive comment on the long lost art of communicating with our patients – an art that reconnected one of his patients with a long lost army medal.

Dr. Ariola received his inscribed plaque at the January 2013 SDDS Board of Trustees meeting. The Tillis Award was established in 1996 to honor the memory of the longtime New York State Dental Journal and SDDS Editor. It recognizes members of the New York State Dental Association who, through their writing in The New York State Dental Journal or in a component publication, pro-mote a positive image of organized dentistry. Dr. Stuart Segelnick won the award in 2010 and Dr. Craig Ratner in 2008.

New ADA-AAOS Guideline:Evidence Does Not Support Antibiotics for Dental Patients with Joint Replacements

Following a collaborative systematic re-view of scientific evidence, the ADA and the American Academy of Orthopaedic Surgeons recently released a co-developed guideline that does not support the routine prescription of antibiotic prophylaxis for joint replacement patients undergoing dental procedures.

The joint AAOS-ADA work group conducted a systematic review of existing clinical research published in peer-reviewed jour-nals to develop the published guidelines in “Prevention of Or-thopaedic Implant Infection in Patients Undergoing Dental Procedures.” They found no clinically relevant correlation between dental procedures and prosthetic joint infection (PJI). “This guideline was based primarily on clinical research which examined a large group of patients, all hav-ing a prosthetic hip or knee and half with an infected prosthetic joint. The research showed that invasive dental procedures, with or without antibiotics, did not increase the odds of developing a prosthetic joint infection,” said Dr. Elliot Abt, a member of the ADA Council on Scientific Affairs who served on the AAOS-ADA work group.

“It has been long debated that patients with orthopaedic implants, primarily hip and knee replacements, are prone to implant infections from routine dental procedures,” added Dr. Jevsevar, an orthopaedic surgeon. “What we found in this analysis is that there is no conclusive evidence that demonstrates a need to routinely administer antibiotics to patients with an orthopaedic implant who

undergo dental proce-dures.”

According to the publi-cation, the recommen-dation is founded in evidence that dental pro-cedures are unrelated to PJI and that subsequent

antibiotic prophylaxis does not reduce the risk for PJI. High strength evidence sug-gests that antibiotic prophylaxis reduces the incidence of post-dental procedure related bacteremia but there is no evidence that bacteremia increases the risk of PJI. Prophy-laxis can be useful in reducing subsequent bacteremia, but bacteremia is a surrogate measure since no direct evidence exists linking bacteremia to PJI. As always, phy-sicians, dentists and patients should work collaboratively to customize a treatment plan that is based on the evidence, clinical judgment and patient preferences.

A new 2.3 percent federal excise tax which applies to the medical devices dentists and physicians use went into effect Jan. 1, 2013. According to the ADA Division of Legal Affairs, the term “device” can apply to devices, instruments, products, materials and substances.

Dentists are not responsible for assessing or collecting the tax and they are not respon-sible for reporting it to the government. That is the responsibility of the manufacturers, suppliers or vendors who sell covered items to dentists. However, dentists can expect a modest increase in the cost of covered items because of the tax.

The Internal Revenue Service estimates that there are around 180,000 medical devices and 130 of those are specific to dentistry. These 130 devices appear on the Food and Drug Administration’s list of dental devices, which the ADA has made available at www. ada.org/8054.aspx. The ADA also posted more detailed information on the new excise tax at www.ada.org/8053.aspx.

Even if a device appears on the FDA list, it is not subject to the excise tax if it is available at retail. Examples include power or manual toothbrushes, dental floss and teething rings. This retail exemption is the major exception to application of the device tax and applies to all medical devices. Dentists have asked the ADA whether they will be responsible

for assessing the tax on bridges or custom-ized crowns they make in their dental of-fices. Since neither bridges nor customized crowns appear on the FDA list, the answer is no, according to the Association’s legal division. If any components that the dentist uses to fabricate the customized devices are on the FDA list, however, the manufacturer or supplier will add the tax and the dentist will pay it at the time of purchase. As an-other example, a restoration is not subject to the tax but the material used for the restora-tion—gold or amalgam, for example—will be subject to the tax and collected by the manufacturer or supplier from the dentist, since both materials appear on the FDA list.

This also means that if a dentist purchases, for example, a finished bridge from a do-mestic dental lab, the excise tax should not be applied to the item. Instead, any price increase in the product should only reflect the amount of excise tax the manufacturer paid when it purchased the materials it used to make the bridge or finished crown.

The examples given here are not meant to represent the whole scope of the law, but to bring to dentists’ attention some of the pos-sible real-life applications of the tax.

The ADA continues to urge Congress to re-peal this tax. The Association will continue to track developments and relay information to members as it receives it.

CDHM Activities Set for 2013Volunteers Needed!

Plans for this year’s Children’s Dental Health Month Activities are underway. Second District’s activities in Brooklyn and Staten Island are perhaps the largest component sponsored program of CDHM activities in the state! Hundreds of children are seen in schools and centers across the two boroughs during the month of February where they are given presentations on the importance of maintaining a lifetime of proper oral health, and in some cases, screenings and referrals to area dentists. Of course, none of this can be done without you, the SDDS volunteers, to make yourselves available. So this is an all-important call to all of our Second District colleagues to make that call to SDDS

Headquarters to let us know if you can assist with programs in Brooklyn and/or Staten Island. Give Veronica Molina of the SDDS staff a call, TODAY, at 718-522-3939, or e-mail her at [email protected].

Among the many programs planned so far are the 2013 Give Kids a Smile Day programs. On Friday, February 21, 2013 volunteers from the Oral Health Committee and any of you who are interested in assisting are needed for the following:

• At P.S. 11 on Waverly Avenue in Fort Greene, a screening and education program which will include Colgate’s Bright Smiles/ Bright Futures van. Children from Pre-K to 2nd grade will have a presentation and dental screenings. It is expected to reach over 400 children.

• At the Dental Hygiene Department of NYC College of Technology on Jay Street in downtown Brooklyn, where hundreds of school-aged children will receive oral health education and screenings by SDDS volunteers and the dental hygiene students of the school.

• New York Methodist Hospital simultaneously will be conducting screenings and preventive services including oral prophylaxis, fluoride varnish, and oral health presentation for area children.

We are also finalizing plans to have a screening program with the Colgate Bright Smiles / Bright Futures Van outside the new Barclay Center on Sunday, February 24 from 11am to 1:45pm. More details will follow.

As usual, the annual poster contest will be held where all area schools are invited to submit posters to SDDS. The contest is perhaps the best received in the state. Members of the Oral Health Committee will judge the posters for originality and creativity. The Contest Rules appear below.

Title: “Provisionalization of the Dental Implant Patient”

Clinician: Michael Klein, D.D.S. Adjunct Assistant Professor, Department of Prosthodontics and Biomaterials, UMDNJ

Synopsis:Provisional restorations are used prior to implant surgery to manage patient esthetics and function while eliminating pathology. The provisional may be used to try out the diagnostic wax-up. During implant surgery, the provisional restoration aids in determining appropriate three-dimensional implant placement and is designed to protect implant/bone healing. It may also be used at this stage to guide soft tissue healing. After implant healing, the provi-sional restoration is used to test out tooth position, shape, phonetics, esthetics and function, as well as overall comfort. The provisional may be used to guide soft tissue or determine if prosthetic soft tissue will add or detract from the esthetic appearance of the patient. Other uses of a provisional restoration can be to extend treatment time for financial consideration or use as an interim long-term solution for the patient.

Aspects of the provisional restoration that will be discussed include diagnostic uses, treat-ment planning, provisional modalities (indications and design), transitional removeable partial denture, essix appliance, fixed provisional, screw retained versus cementable and Maryland Bridge. This presentation will also cover immediate load, long-term provisional, technical implementation, management of esthetics, function and soft tissue, testing out function, assessing vertical dimension, immediate provisionalization and developing tooth soft tissue esthetics. Upon successful completion of this course, attendees will learn that the provisional restoration may be the key to treatment success in implant dentistry.

Title: “Oral Surgery 1.0”

Synopsis:Basic dentoalveolar oral surgical procedures continue to be part of most general prac-tices and whether dentists enjoy these procedures or do them grudgingly depends on their level of preparedness. Topics to be covered during this presentation include:

• The biology of wound healing• Case selection (when to treat and when to refer)• Pre-operative patient evaluation (medical history, medications)• Radiographic utilization, evaluation and interpretation• Selection of local anesthesia and efficacious techniques• Useful surgical armamentarium• Techniques for “routine” surgical extractions and root recovery• Osseous procedures; alveoloplasty, tuberosity reduction, excision of tori• Soft tissue procedures; frenectomy, biopsy• Evaluation and management of odontogenic infections• Antibiotics (including prophylaxis) and analgesics for the oral surgical patient• Diagnosis and management of common post surgical complications

Upon successful completion of this course, attendees should be able to confidently evaluate their patients preoperatively, provide proficient basic oral surgical treatment, make rational selections of medications and effectively manage common post-operative complications.

Title: “CPR & Medical Emergencies in the Dental Office”

Instructor: Michael Kaliroff Certified Instructor, American Heart Association

Synopsis:This course provides dentists and their staffs with the knowledge and skills needed to prop-erly respond to cardiopulmonary and other medical emergencies which can occur in dental offices. The specific advantages and disadvantages of performing CPR within the dental op-eratory will be discussed. This program utilizes hands-on practice sessions for all partici-pants. Topics to be covered will include heart attacks and strokes (signs and symptoms), sud-den cardiac arrest/defibrillation, airway obstruction (choking), respiratory emergencies, anaphylactic reactions, emergency drug kits, and more.

Upon successful completion of this course, participants will gain the knowledge and hands-on experience needed to administer CPR safely and effectively. Participants will receive a 2-year certification card from the American Heart Association as well as continuing educa-tion credits. Completion of this course will also satisfy the New York State Education De-partment’s requirement for licensed dentists.

Title: “Orthodontic Diagnosis and Treatment Planning”

Clinician: Peter A. Costalos, D.M.D. Former Clinical Instructor, Columbia University College of Dental Medicine

Synopsis:This course will serve as an introduction to orthodontic diagnosis and treatment plan-ning to the dentist in general practice. Developing a diagnosis is the result of a sci-entific inquiry and its goal is truth. Orthodontic diagnosis begins with the collection of relevant patient information including the patient questionnaire, the patient’s chief complaint, medical and dental history, physical growth evaluation, social and behavioral evaluation and the clinical evaluation. The clinical evaluation includes evaluation of oral health, occlusion, facial proportions and the analysis of diagnostic records.

From this database of patient information, a prioritized comprehensive problem list, the diagnosis, is then established. The problem list helps establish treatment objectives and eventually a treatment plan, the strategy by which the existing problems are trans-formed into the desired result and maximal benefit for the patient. Issues faced in de-veloping the treatment plan include the timing of treatment, complexity of the required treatment, predictability of treatment success, and patients’ goals. This process culmi-nates with the determination of a specific therapeutic approach and the appliance sys-tem to be used. Upon successful completion of this course, attendees will be acquainted with the use of a problem-oriented approach in establishing an orthodontic diagnosis and ultimately developing an orthodontic treatment plan.

UNFINISHED BUSINESS:Since the November general meeting was cancelled, a special membership meet-ing was held on December 3 for the specific purpose of holding annual elections. As there were no further nominations for elected positions as published in the October SDDS Bulletin and presented at the October Membership meeting, the Secretary cast one ballot to elect the slate of officers and other elective positions for 2013 as previously pre-sented to the membership.

NEW BUSINESS:Motion passed (Medrano): To allocate $600 toward the videotaping of SDDS Instal-lation of Officers in memory of Dr. Robert Halikias. Motion passed (McIntyre as amended by Medrano):

That SDDS contribute up to $200,000 to the SDDS Benevolent Fund for the specific purpose of relief from Hurricane Sandy.

GOOD AND WELFARE:• Dr. Oshrain requested to see a copy of the attorney bill regarding GNYDM. • Dr. Medrano noted that NYSDA lost members from the previous year. Why? And is anything being done to stop the trend?• Dr. Medrano questioned why Dr. Lerner wasn’t appointed to NYS Den-tal Foundation after being told he was. Answer was resolved by Dr. Ratner. Dr. Ratner said he had misspo-ken the information. Dr. Lerner name was submitted for consideration, not that he had gotten the position.• Dr. Medrano questioned why the SDDS office hours were posted as 9am – 2pm and not 5pm. (Answer
