FULL BACKGROUND CHECK

Cryobank America wants the BEST donors possible and by performing a criminal background check we can find red flags in their past that may indicate they have had some trouble. If determined between the Laboratory Director, Medical Director, and Management to be any threat or unstable based on the report, the donor will not be allowed into the program. In addition, full educational transcripts will be required from donors to verify their educational background. We only want the best donor for you and your family.

PSYCHIATRIC EVALUATION (A CRYOBANK AMERICA EXCLUSIVE!)

As part of joining the Cryobank America Sperm Donor Program, each donor will visit a psychiatrist and undergo a full mental health evaluation and counseling.

CHROMOSOME ANALYSIS

Also known as Karyotyping, this analysis is used to detect chromosome abnormalities, which can help uncover and diagnose hidden genetic diseases, birth defects, and certain disorders of the blood and lymphatic system. All Cryobank America donors undergo Chromosome Analysis testing.

Dr. Crider is responsible for the overall operation and administration of Cryobank America, including employment of personnel who are competent to perform test procedures, record and report test results, promptly, accurately and proficiently, and for assuring compliance with applicable regulations. The Tissue Bank Director is also responsible for development and implementation of policies and procedures for the operation of the sperm bank, and for the selection of donors or establishment of criteria governing that selection.

Our laboratory staff is comprised of individuals with at least a Bachelor’s of Science. Most of which hold a Masters in Science. They are responsible for all specimen processing and go through a rigorous 6-month training period and are tested annually for competency and techniques.

Each donor is identified via a biometric identification device and is compared to facial photographs stored on-site to ensure proper proof of identity.

DONOR TYPES

Cryobank America offers two types of donors from which our clients may choose, including Open ID and Closed ID donors. Protecting the privacy both of our donors and future patents is one of the most important things that we do here at Cryobank America. By properly identifying donor type, prospective parents will be better able to choose the type of donor which suits their needs.

OPEN ID DONORS

These donors have agreed to allow for Cryobank America to share their full name and also establish at least a one-time contact with his offspring, once that child has reached the age of 18.

There is no requirement that the donor must commit to a long-lasting relationship with the child, though it is up to the donor and child to arrange any contact after initial exchange.

Most future parents find security in knowing their child will be able to have some sort of contact with the donor who has contributed to their genetic makeup. It is important to note however, that the child, and only the child, who must be the one to initiate the initial contact through Cryobank America. Therefore, we leave it to the child, once he or she is 18 years old, to determine whether or not they want to communicate with their donor.

In order for future parents to get a better overall picture of their prospective donor, we ask that all our donors provide some information and documents which allow the future parents to get to know the donor as much as possible, without actually knowing who the donor is.

CLOSED ID DONORS

These donors have not agreed to allow for Cryobank America to release any identifying information about them.

Nor have they agreed to allow for Cryobank America to facilitate a one-time contact between the donor and the child. All donor and recipient records are maintained internally indefinitely and we will not compromise the anonymity between the donor and any recipients.

WHAT IS CMV AND HOW DOES IT AFFECT ME?CMV stands for cytomegalovirus. It is a virus that can be transmitted to a developing fetus before birth. It is a member of the herpes family of viruses that include mono and chickenpox. The CDC states that nearly 1 in 3 children are already infected with CMV by the age of 5, and by the age of 40 over half of the population is infected.

Primary CMV infection occurs in people who have never been exposed to the CMV virus before. Once a person becomes infected with CMV, the virus remains alive but dormant inside that person’s body for the rest of their lives. Recurrent CMV infection, is when a dormant virus become active again. CMV infection is usually harmless and rarely causes illness. However, for pregnant women, primary CMV infection can cause more serious problems than recurrent CMV infection.

The FDA requires that CMV testing be performed on all men who intend to donate sperm. A positive result however, doesn’t necessarily mean that a man will be ineligible to donate. Cryobank America will obtain semen samples from potential donors and then quarantine those specimens for at least six months. During that time, the man may have CMV antibody levels tested several times. If the antibody tests indicate the

possibility of a CMV infection close to the time of the sperm donation, the man will not be allowed to donate those specimens. If the testing is uncertain, those men will also not be allowed to donate those specimens. However, if a donor tests positive for CMV IgG only, indicating a past infection, he will be allowed to donate. Those samples will be labeled in the Cryobank America

database as CMV positive.

CMV AND USING A DONOR

RECOMMENDATIONS

A woman who is considering pregnancy with donor semen should have CMV antibody testing as part of their IDT. Those who have had a past infection are at very low risk of transmitting CMV infection to a fetus. And are at little to no risk if they decide to use a CMV positive donor. Women who have never been exposed to CMV should consider using a CMV negative donor. Even though the risk from a CMV positive donor is low it is impossible to determine whether there will be risk for infection.

“Washed” vs. “Unwashed” Semen

Cryobank America sells a few different types of sperm vials. This section is dedicated to going through the different types to better inform the consumer and help make an educated decision on the type of vial you would like to purchase.

First, we will go through the difference between washed and unwashed semen. Washed semen is a processing method we use which takes the raw semen and “filters” it from the seminal fluid and allowing for non-motile and dead sperm to be filtered out and the strongest swimmers to be saved. This also removes any white blood cellsand prostaglandins. This is done toreduce discomfort in pregnancy and

increase chances of getting pregnant. By using washed semen placed directly in the uterus you are reducing discomfort side effects such as pain, cramping, nausea and vomiting. Washed sampled only come in IUI vials which contain 25 million/mL oror more of motile semen. We also offer IUI-A.R.T vials which have <25 million/mL.

Secondly, we offer “unwashed” semen. This is minimally processed semen in more of its natural state. It still has undergone the same testing to ensure safety. An “unwashed” or ICI specimen is used when the semen is inserted just behind the vagina in the cervix. This process can be performed at home or at your physician’s office. Since it is “unwashed” it tends to have a lower concentration but some physicians may use this sample to perform the wash at their facility.
