The ASQ developmental

assessment

• All referrals to the EYP need to be discussed with the child’s health visitor.

• An up-to-date ‘Ages and Stages’ (ASQ) questionnaire must be submitted with the referral

• The Panel needs the ASQ, in full, to make decisions

• Send it with the referral form or make it clear when we can expect it

Additional information

• EYP meets on the first Tuesday of each

month in term time

• The cut off date for receiving information for

the next panel meeting is the Thursday 10

days before.

• Dates and deadlines save time in the long run

• Handwritten forms can pose some problems!

• Specific requests can be helpful but may not

necessarily be acted on

• The EYP is not the lead professional
