Ground-breaking procedure allows stroke sufferer to walk out of hospital three days laterA stroke sufferer who became the first Bay of Plenty man to undergo a ground-breaking procedure is welcoming in 2018 with a sense of gratitude and relief.

“I am very lucky,” says Omokoroa resident George Stirling, who underwent Endovascular Clot Retrieval last September. “There’s no doubt about that.”

Prior to the stroke George’s GP had observed irregularities on an ECG test and sent him to Tauranga Hospital for further investigations. It was whilst he was a hospital inpatient that the stroke occurred.

“I was chatting with the hospital chaplain and then all of a sudden I’m just talking gobbledygook,” says the 82-year-old, who remembers very little of what happened next. Consultant Stroke Physician and Geriatrician Dr Mohana Maddula takes up the story.

“George had acute ischaemic stroke due to blockage (clot) in one of the major blood vessels in the brain,” said Dr Maddula. “The clot was stopping blood flow to the brain and there was a risk of permanent brain injury which could lead to significant long-term disability and death. After urgent brain scans and discussion with specialists in Auckland he was transferred by helicopter to Auckland City Hospital for Endovascular Clot Retrieval.

“This is a ground-breaking new treatment where a device is inserted through the femoral artery in the groin up into the brain, to extract the clot and restore blood flow to the affected region of the brain.”

The procedure is undertaken by Interventional Neuroradiologists and is currently only provided in Auckland, Wellington and Christchurch. It is not suitable for all stroke sufferers. The procedure needs to be performed as soon as possible after the onset of stroke symptoms, meaning the patient needs to present to hospital quickly, a helicopter be available, and the weather conditions be suitable for transfer.

“This procedure is very time-critical,” said Dr Maddula. “George arrived at Auckland City Hospital

in good time and had this emergency procedure soon after arrival. The clot was extracted and blood flow was restored.

“George returned to Tauranga Hospital the next day and was discharged home a couple of days later. He walked home, almost completely recovered from his stroke. Without this procedure it was very likely that he would’ve suffered long-term disability and may not have been able to return to his ‘normal life’.”

George is now relishing that ‘normal life’ which includes regular Thursday motorbike rides with a group of friends. He also hopes to get back to playing golf in the future as well.

“I’m just very thankful to all the doctors and nurses involved in my care. I’m very aware that it could’ve turned out so differently for me.”

George was the first Bay of Plenty patient to undergo Endovascular Clot Retrieval. The Bay of Plenty District Health Board is working with colleagues in Auckland to develop and streamline this treatment pathway for patients like him.

“A lot more work needs to be done to make this treatment available for the whole BOP region,” said Dr Maddula. “We were lucky with George because when he had the stroke, the helicopter and flight team were available and the weather was good. To consistently provide this treatment 24/7 we need to develop transfer systems that are readily available and can operate in all weather conditions.”

Estate PlanningThe importance of having a current Will, particularly as you age, has been well documented. A Will is an important tool in making sure your wishes are carried out and your loved ones are provided for after you are gone.

It is equally important to have legal arrangements in place should you become incapacitated and unable to manage your own affairs.

Enduring Powers of Attorney are documents which appoint someone to manage your property affairs (including finances) and your personal care and welfare in the event you become incapacitated. You can appoint more than one attorney in the case of your property affairs.

Well considered Enduring Powers of Attorney will ensure you have the right person or people making decisions for you. This aims to avoid confusion

among family members and the possible exploitation of your financial affairs.

If you do not have Enduring Powers of Attorney in place and you become incapacitated, your family members will likely need to apply to the Family Court for the appointment of a Property Manager and/or Welfare Guardian on your behalf. This can be a costly, time consuming and overall distressing process.

While Enduring Powers of Attorney may not be an easy conversation to have with family members, it is far easier than the hassle you will put your family through should they need to go through the Family Court process.

To protect your property, finances and, most importantly, your wellbeing as you age, start the conversation today.

• 1946 - New Zealand summer time (12 hours in advance of GMT) was adopted as New Zealand standard time. Daylight saving time was effectively discontinued at this point.

• 1974-1975 - Daylight saving was trialled again in 1974, and introduced in 1975. Daylight saving time is 1 hour ahead of New Zealand standard time.

• 1985 - Public attitudes were surveyed and over the next few years the period of daylight saving time was extended twice.

• 2006-2007 - Following public debate and a petition presented to Parliament the period of daylight saving was extended to its current dates. New Zealand observes daylight saving from the last Sunday in September to the first Sunday in April.

• 2008 - Daylight saving public attitude survey.

Remember that daylight saving ends on 1 April at 2.00am – so remember to turn your clocks back one hour before you go

to bed. Also time to check your smoke alarms and electric

blankets.

The Home Maintenance and Safety programme aims to help older people stay in their own homes longer by keeping on top of house maintenance and repairs. Often this work is put off because it feels like it is too hard or could cost too much.

There is an online checklist for inside and outside the house, which gives information on priority and who might do the repairs. You can download this tool from Age Concern’s website: www.ageconcerntauranga.org.nz A printed copy is available from our office at a small cost.

If you would prefer to have your home assessed by a professional company, Age Concern will refer you to Total Home Inspection Services, who have agreed to do the inspection for us at an agreed price. They will provide a report with photographs and give an indication of priority work. Age Concern will provide follow up assistance.

Raisin Bread(Preparation: 10 minutes Cook: 1 hour)

Ingredients• 3 cups plain flour• ½ cup white sugar• 3 teaspoons baking powder• ½ teaspoons baking soda• ¾ teaspoon ground cinnamon• 1 cup raisins• 1 egg• ¼ cup melted butter• 1 teaspoon salt• 1 cup milk

Instructions1. Preheat oven to 350 degrees F (175 degrees C). Grease 20x13x6cm loaf pan.

2. In large bowl measure flour, sugar, baking powder, soda, salt, cinnamon, and raisins. Stir thoroughly. Make a well in centre.

3. In a small bowl beat egg until blended. Mix in melted butter and milk. Pour into well. Stir just enough to moisten. Scrap into 20x13x6cm loaf pan.

4. Bake in oven for 1 hour. Serve plain or toasted with butter.

preparing for the cooler months aheadChecklist• Gutters, downpipes and drains - Check that they are free of leaves and debris, and that nothing needs repairing or cleaning.

• Roof and flashings – Check to ensure there are no leaks or worn tiles, so that water can’t permeate into your property.

• Heat pump - Make sure you get the filter changed and ensure the exterior unit is free from excess leaves, debris, etc

• Musty smells and/or mould – check particularly in places such as wardrobes, cupboards and backs of curtains.

• Extractor fans – Check that they are in good working order.

• Ventilation system – make sure the filter has been changed in the past 12 months.

• Fireplaces - Most insurance policies include a condition that fireplaces must be cleaned annually. If you do not comply, your policy may be deemed to be invalid in the event of a fire. If the fireplace doesn’t get used, consider blocking it off as this will reduce warm air escaping the house.

• Chimneys – Clean chimneys and flues before you light the first fire this year.

• Smoke alarms – Test your

smoke alarm(s), does the battery need changing?

• Electric blankets – Is your electric blanket 5 years and older? If so, the fire service recommends that you replace it with the newer heat protected model.

• Clothes Dryer – Ensure the lint filter is cleared. This should be cleared after every use.

• Portable LPG Gas Heater – should be serviced every 12 months

Suggestions/Ideas• Consider installing leaf guards on the gutters and extensions on the downspouts to direct water away from the property.

• Consider installing extractor fans into kitchens and bathrooms.

• Consider pruning back trees that block sunlight from the property.

• It’s a good idea, especially if you live in an older home, to get a warrant of fitness for the wiring of your home on a regular basis to ensure everything is still functioning correctly, and there’s no chance of it being a fire hazard.

• Is your hot water cylinder more than 10 years old? cylinders should last 10-20 years but if they fail they can do a lot of expensive damage. Get your cylinder checked out by a plumber if it’s past the 10 year mark.

• Power bills - If you haven’t already done so, consider contacting your energy company to discuss managing your energy costs by spreading payments

out over the year.

• Rates rebate – You may be entitled a rates rebate if you are on a low income. Contact your local Council for further information.

• Gardening: - Sow a new lawn - Harvest seeds from summer

crops - Add compost etc to replace lost nutrients to veggies gardens and flower beds - Plant up pots and containers with autumn potted colour - Sow seeds for winter crops such as carrots, radish, swede - Plant lettuce, broccoli, spinach, cauliflower, cabbage
