SUPPLEMENTARY METHODS

Instrumentation

Two LCQ Deca XP Plus quadrupole ion trap mass spectrometers (Thermo Finnigan) were used

as mass analyzer units. The instruments (without factory chassis) were installed into noise-

insulating housing made of welded steel housing and stainless steel walls. The walls were lined

with 50 mm thick polyurethane-based insulating material. The exhaust from the air cooling and

vacuum pumps of the instrument was vented to the operating room atmosphere through a 500

mm × 350 mm HEPA filter to avoid the emission of infectious agents. In order to increase the

lifetime of the HEPA filter, another dust filter was used at the cooling air intake of instrument.

Overall noise emission of instrument was decreased to 67 dB, while the operating temperature of

the instrument did not exceed 55 C. All accessories of the mass spectrometer including rotary

vane pumps and helium tank were incorporated into the housing. The atmospheric interface of

instrument was re-built and was optionally equipped with a Venturi air jet pump-based ion

transfer apparatus. The interface and ion transfer device are depicted on Fig. 1A. The Venturi

pump was driven by pressurized air (from the central pressurized air system of the hospital) at

nominal inlet pressure of 4 bars.

Electrosurgical handpieces were coupled to mass spectrometric instrumentation by means

of an aerosol (“smoke”) evacuation tube (Fig. 1) (14, 16). Aerosol formed during electrosurgical

tissue dissection or coagulation was directly and continuously sampled by the handpiece in both

monopolar and bipolar electrosurgery applications. Transfer of the aerosol to the distant mass

spectrometer was carried out in two distinctively different ways depending on whether

monopolar or bipolar electrosurgery was used. In the case of monopolar electrosurgery, the

aerosol is transferred using a Venturi pump (Fig. 1A) and 4 m long polymer tubing. Since linear

gas velocities in the range of 100-200 m/s are achievable using the described setup, desorbed

particles reach the mass spectrometric analyzer within 20-30 ms. When bipolar forceps are used,

the aerosol is directly transferred to the mass analyzer using a polymer tube without the aid of an

air jet pump. A lower rate of ion formation (relative to monopolar electrosurgery) and near

complete absence of interfering combustion products in the aerosol allow for such direct

introduction which is assisted by the suction force induced by the vacuum system of the mass

spectrometer.

The modified electrosurgical handpiece, incorporating ion transfer tubing, was obtained

from Meyer-Haake GmbH. The handpiece was connected to either an Erbotom ICC 300 (Erbe

Elektromedizin GmbH) or a Radiosurg 2200 (Meyer-Haake GmbHy) electrosurgical unit. The

devices were used both in cut and coagulation modes. Laboratory analyses of ex vivo specimens

were carried out using LCQ Deca XP Plus and LTQ-Orbitrap Discovery (Thermo Scientific)

mass spectrometer instruments. Instruments were equipped with modified atmospheric interfaces

featuring heated capillary atmospheric inlet and orthogonal Venturi-pump aerosol transfer

device. During laboratory experiments, a 70 cm long, 1/8” OD, 2 mm ID PTFE tubing was used

for aerosol transfer, connected to a monopolar electrosurgical handpiece, which was functionally

identical to the ones used in the surgical environment. The calculations were performed on a

Lenovo ThinkCentre computer equipped with Intel core 2 Duo CPU at 2.80 GHz and 2 GB of

RAM.

Table S1. Patient cases where preoperative and postoperative histology results did not

match. In 15 cases, pre-operative and post-operative histological diagnoses did not match.

Tissue from these cases were analyzed post-operatively ex vivo with the REIMS-iKnife

approach, looking for both the tissue type and whether or not it was malignant. “Outlier”

represents a sample for which tissue-type could not be recognized because it was not present in

our spectral database (n = 4). WHO column indicates the international classification of each

pathology and is included in the final pathology report in many cases. REIMS-iKnife diagnoses

that were healthy (matching WHO-based assessment) are in italics (n = 9).
