Access To Care
You will see two charges from Student Health on the tuition and fees statement:
1. The Student Health Fee: As part of the tuition and fees package, the fee
covers the cost of most routine office visits. Nominal fees apply for services
such as labs, X-rays, immunizations, physicals, and procedures. Fee also
covers public health initiatives and responses on campus.
2. Student Health Insurance Plan Charge: All UNC System (including NC State)
students are required to have health insurance while enrolled in classes. 1. Students can purchase the University Sponsored Health Plan, Student Blue provided by BCBS.
2. Students can remain on a current health insurance plan or choose to purchase other coverage.

Talking to Your Student About Insurance Benefits
1. Call your insurance carrier to make sure Student Health is in
network with your plan
2. Have your student take a picture of all their insurance cards
3. Educate your student on your insurance plan

• Copays for prescriptions will be the
same as going off campus or less
depending on the medication
• Affordable over-the-counter medications
• Other items include contact lens
solution, sunscreen, heating pads and
thermometers and much more

Immunization Requirement
Students are required by North Carolina Public Health Law to provide proof of
immunizations within 30 days of registration of classes.
3 Steps to Compliance - All Steps are Completed on the HP Portal
1. Upload completed Immunization Record (Accepted forms of documentation are
available on our website).
2. Enter dates immunizations were received on Immunizations tab
3. Complete required TB Questionnaire and the Health History Form
Check HealthyPack Portal secure messages periodically for notes about missing
documentation or call 919.513.4302 with questions.
Student Privacy & Access to Patient Information
•Students over the age of 18 are protected under the Federal Health Insurance
Portability and Accountability Act (HIPAA)
•Regulations require that students age 18 or older sign a release form in order for any
health care information as well as billing information from Student Health to be shared
with parents or guardians. Students can complete this form and either mail, fax or
drop it off at Student Health. (Form found on HealthyPack Portal under “Forms” tab)
• For students under the age of 18, parent or guardian will need to complete a
“Consent to Treat (Under 18) form. This form can be found on the HealthyPack Portal
under “Forms” and then uploaded under the “Upload” option.

For Insurance Compliance
• Enroll or waive the Student Health Insurance Plan by September 10, 2018
• Get a copy of your insurance card before arriving to campus (hard copy or picture)
• Upload a copy of your insurance card to the HealthyPackPortal under the Insurance tab
For Immunization Compliance
• Upload immunization documentation to Student Health Services, via the HealthyPack Portal, within 30 days of
registration of classes
• Use the Portal to enter immunization dates
• Complete TB questionnaire and Health History Form online via the HealthyPackPortal
General Student Health
• Transfer your prescriptions to the Student Health Pharmacy by calling 919.515.5040
• Get your flu shot at one of our clinics on campus (September-January)
• Follow us on social media for important date reminders, health tips and outreach events
