CHLAMYDIA

Urogenital chlamydia infection is a common STD worldwide. The causative organism is Chlamydia trachomatis. Infection is usually asymptomatic in both men and women. Alaska has the highest rate but LA is in 3rd place. Noninvasive sampling (urine or vaginal) - Nucleic acid amplification test (NAAT)- is as effective as invasive sampling (endocervical or penile urethral swab) and is more acceptable to patients. Rx azithromycin : 1 g orally as a single dose

GONORRHEA

Neisseria gonorrhoeae is a gram-negative diplococcus bacteria

Noninvasive sampling (urine or vaginal) - Nucleic acid amplification test (NAAT)- is as effective as invasive sampling (endocervical or penile urethral swab) and is more acceptable to patients.

Rx ceftriaxone : 250 mg intramuscularly as a single dose

SYPHILIS

A common sexually transmitted infection. Approximately 10 million to 12 million new infections worldwide each year. Good news, the rate in LA is now only the 3rd highest in the nation.

Clinical presentation is often asymptomatic, but can manifest in a number of ways.

Diagnosis is usually straightforward after clinical exam and serologic tests.

Treatment is with penicillin. Untreated syphilis facilitates HIV transmission and causes considerable morbidity, such as cardiovascular and neurologic disease

TRICHOMONAS

Usually presents as vaginitis with vaginal discharge, discomfort on urination and typical findings on examination.

Diagnosis involves a wet mount in which the protozoan is easily identifiable.

Treatment of both partners with metronidazole is generally curative.

Unpleasant, however not particularly dangerous.

HERPES SIMPLEX

Infection with HSV-1 or HSV-2 can cause oral, genital, and ocular ulcers. Most have unrecognized disease based on serologic testing later in life.

Serologic testing will confirm that the individual has antibodies to the virus but does not necessarily diagnose active infection nor does it predict infectivity or transmission.

Acyclovir or equivalent antiviral agents will shorten the duration and extent of the disease when it recurs.

Once to have the condition, it stays with you for life. The virus can complicate childbirth and can cause meningitis in newborns.

GENITAL WARTS (CHONDYLOMATA)

Caused by HPV infection, most commonly HPV types 6 and 11.

Peak incidence of genital HPV infection occurs in 16- to 25-year-olds.

Diagnosis is made based on clinical presentation.

Treatment can be challenging, as there is no cure and recurrences are common

CERVICAL CANCER

A human papilloma virus (HPV)-related malignancy,

Preventable by HPV vaccination PAP test used for screening,

HIV INFECTION Caused by a retrovirus that infects and replicates in human

lymphocytes and macrophages, eroding the integrity of the human immune system over a number of years.

AIDS occurs as a result of HIV infection, usually after approximately 6 to 9 years of infection. It is a constellation of infections, conditions, or malignancies that occur as a result of the increasing immune depletion that HIV infection incurs over time.

Diagnosis is established using an HIV antibody test and confirmed using a more specific test. Patients should be clinically staged according to WHO or CDC criteria.

Diagnosis and management varies between resource-intensive settings and resource-limited settings.
