1Introduction to ServicesIsabella County is an area that has a very clear need for affordable health care services as well as wellness education.

This group plans to develop a clinic to provide services to the less fortunate. This population includes the uninsured and under-insured citizens of the community as well as the large population of college students in the area.

The services will be provided for free or low cost, with a goal of improving the overall health of the citizens of Isabella County.Facts:

In 2009, Isabella county had an unemployment rate of 17%, with the national average at 9.4% (City-data, 2010)

Isabella county has a higher than state average of binge drinking and motor vehicle crash death rate (Snapshot, 2010)

Isabella County is home to Central Michigan University, adding to the population of uninsured citizens.

6Objectives for Patient CareAssess and Treat

The clinic will provide treatment to walk-in patients.

We will screen for health problems.

We will gain access to low or no-cost medications for needy patients.

We will evaluate all incoming clients BMI value and offer information on healthy living.

The clinic will provide routine exams such as pelvic exams and pregnancy testing.Offer Classes

We will provide nutrition classes on a bi-weekly basis.

The clinic will hold bi-weekly diabetic education sessions.

We will provide smoking cessation support.

We will talk to high school students of the risks of alcohol consumption.

We will work with the area college to support pregnancy and STI prevention.

Obtain free medications and coupons for discounts for patients that cannot afford their medications.

Responsible for maintaining the training files and files associated with licensing and accreditation. Duty 1: Provides evidence based training programs in general and specific nursing care, dietary training, mental health, and client relations and client care. Delivers nursing focused educational programs to clients. Provides health care instruction and education to individuals relating to the promotion of health and prevention or management of illness, disease, injury, or disability. (60%)Duty 2: Evaluates staff and volunteer competencies on an annual or as needed basis. Maintains training and accreditation files ensuring that the clinic is in compliance with all standards and legal requirements. General Summary of the Function/Purpose of this Position: Instruct patients, families, and health care staff in individualized diet therapy, general nutrition principles, and food selection and preparation. Conduct nutritional screenings and assist in the development, review and modification of individuals therapeutic diets.Duty 1: Serves as the teams expert and acts as nutrition resource person on the health care treatment team. Develops, reviews, and adapts therapeutic and modified diets to meet the needs of individual residents and/or clients in cooperation with health care staff. Inspects food preparation and food service for conformance with prescribed diets and standards of sanitation, palatability, appearance, and nutritional quality. Plans menus for clients at the agency and adjusts menu cycles for nutritional adequacy, balance and variety, food availability, and budgetary control. (60%)Duty 2: Calculates nutritional values of food served. Determines serving size, frequency, and method of preparation of foods. Inspects food supplies for quantity, quality, and compliance with state and federal standards. Recommends type and quantities of food to be purchased. 13ConclusionStayin Alive will provide a community in need with much needed health services.

We have a plan to develop this service and to obtain and utilize resources in a responsible manner, in order to maximize the benefits to the community.

We will develop a clinic that will not only treat those in need, but provide ongoing educational services to improve the overall health of the community.

Grade: 93 out of 100 GROUP 3...PRESENTATION EVALUATION

Group 3:Excellent work on the group project. Please review the following comments and take it as constructive feedback. The one suggestion I would like to offer is when designing powerpoint presentations, be mindful of the amount of content included in each slide. Sometimes, we tend to want to added a large amount of information that we miss addressing the key issues of the project.

COMMENTS:

The mission statement and philosophy on the ppt slides were very weak and did not clearly address the points that would convey the direction of the clinic, however those areas were covered thoroughly in the paper.

The budget procurement details were outlined extensively but the layout of the chart did not stay contained in the format in which it was developed and thus was a bit scrambled. The goals of the clinic are admirable and the extensive list of health education topics seemed rather industrious and was unremitting in print.

The group only completed a few of the job descriptions. I did not see a marketing plan or strategy at all.

Powerpoint presentation lacked some key information, but it was found in the written paper. Overall nice idea, and well laid out plan.

What a professional plan and presentation. Excellent Job!Great PowerPoint and I really loved the narration. It is very challenging to narrate and get the timing right. Very thorough business plan and paper. CMU has a Physician Assistant program that might be included as a source of volunteer providers.
