1. The language in the Electronic And Information Technologies (IT) Section of the Instructions, Information and General Conditions on page 11 of the RFQ&P shall be replaced in its entirety as follows:
ELECTRONIC AND INFORMATION TECHNOLOGIES (IT)
The Proposer hereby warrants that the goods or services to be provided to the District comply with the accessibility requirements of Section 508 of the Rehabilitation Act of 1973, as amended (29 US.C. S794d), and its implementing regulations set forth at Title 36, Code of Federal Regulations, Part 194. The Proposer agrees to promptly respond to and resolve any complaint regarding accessibility of its products or services. Proposer further agrees to indemnify and hold harmless the District from any claim arising out of its failure to comply with the aforesaid requirements. Failure to comply with these requirements shall constitute a breach and be grounds for termination of this order or agreement. Proposer is responsible for following all Federal and California accessibility laws set forth under Sec 508 of the Rehabilitation Act of 1973, passed in 2000 and updated in 2017 and California Government Code Section 7405. All materials and Information and Communication Technology (ICT) produced or provided by the Proposer, as part of this contract must meet the standards set forth under these laws. These requirements include, but are not limited to, closed captioning of all videos or portions of videos; all presentations; training materials; curriculum; computers; and all other ICT as defined under the law, must be created and delivered in a manner where they meet accessible requirements. All websites developed and maintained must be accessible, built to the most current and highest Web Content Accessibility Guidelines (WCAG), and be delivered with documentation allowing the District to certify it as accessible and in compliance with California Government Code Sections 7405 and 11135. Proposer is responsible for all claims and expenses borne by the District, which arise out of the work under this contract, found to be non-compliant with Federal and California Laws. These costs include but are not limited to legal costs, court costs, and costs for remediation of work produced.

2. PROPOSAL FORM H - IT Accessibility Standards and Compliance Requirements has been added to
the RFQ&P through this Addendum as a required document. Proposal Form H is attached to this Addendum.
3. APPENDIX N – Projector Mount Drawings and Specifications is attached to this Addendum. The original RFQ&P stated that the Appendix N would be issued in an Addendum.
4. APPENDIX O – Submission Checklist has been revised to reflect the addition of Proposal Form H –
IT Accessibility Standards and Compliance Requirements. The revised Appendix Form O is attached to this Addendum.

PROPOSAL FORM H
IT ACCESSIBILITY STANDARDS AND COMPLIANCE REQUIREMENTS
For Web-Based Products/Services being purchased in this RFP, Firm must demonstrate compliance with the following standard: WCAG 2.0 level AA for products/services with web or Internet access. Firm shall provide detailed information to the below questions along with the necessary demonstrations as required about the accessibility of the proposed IT products or services.
1. For services to develop web-related products, include a description of how each of the WCAG 2.0 level AA standard/requirement will be implemented.
2. For each area of noncompliance, describe any planned remediation roadmaps, including timelines and steps that will be taken to achieve full compliance, as well as interim workarounds to enable access by individuals with disabilities.
3. Provide your Firm’s policy or commitment statement regarding electronic accessibility.
4. Who in your Firm is responsible for the electronic accessibility policy and compliance (provide contact information)?
5. Do you have an accessibility function or team responsible for technical development? Describe its role in your organization.
6. How does your company achieve compliance with IT accessibility standards?
7. Describe the testing protocols you use to assess the accessibility of your product/service.
8. Can you provide live or pre-recorded demonstrations of the accessibility of your product?
9. How do you assure that you keep your product current with changing legal requirements and accessibility best practices?
10. If needed and when requested, Frim must provide evaluation products for additional District validation testing.
Demonstration and Tests
RFP Finalists should be required to provide demonstrations during the Site Visit for Product Demonstration or any other venue to support their statements about the accessibility of their products or services.
At a minimum, the Firm should show how blind or low-vision users would access the product using a screen reader. The supplier also may be asked to include members of other disabled communities to demonstrate different types of accessibility compliance.
For the IT Products being purchased in this RFP, Firm must demonstrate compliance with the
following standard: Section 508 of the Rehabilitation Act for telecommunications products;

video and multi-media products; self-contained, closed products; and desktop and portable
computers.
Firm shall provide detailed information to the below questions and must include a response to applicable sections of the Voluntary Product Accessibility Template (VPAT) in order to describe product compliance with Section 508 of the Rehabilitation Act. Information about the VPAT is available from the Information Technology Industry Council.

28 PRIOR TO COMMENCEMENT OF WORK, THE CONTRACTOR SHALL SECURE THE AREA SO THAT NO UNAUTHORIZED PERSONNEL ORCHILDREN SHALL GAIN ACCESS TO THE PROJECT AREA OR PROJECT STAGING AREAS.
29 THE CONTRACTOR SHALL BE RESPONSIBLE FOR PROTECTING THE EXISTING BUILDING(S) FROM WEATHER DAMAGE DURINGCONSTRUCTION. ALL DAMAGE SHALL BE REPAIRED TO THE SATISFACTION OF THE CLIENT AND PAID FOR BY THE CONTRACTOR.
30 CONTRACTOR SHALL BE SOLELY RESPONSIBLE FOR SECURING HIS/HER EQUIPMENT, SUPPLIES, TOOLS, ETC.
31 IT SHALL BE THE RESPONSIBILITY OF THE CONTRACTOR TO LOCATE ALL EXISTING UTILITIES WHETHER SHOWN HEREON OR NOT AND TOPROTECT THEM FROM DAMAGE. THE CONTRACTOR SHALL BEAR ALL EXPENSES OF REPAIR OR REPLACEMENT IN CONJUNCTION WITHTHE EXECUTION OF THIS WORK. ANY DAMAGE TO UTILITIES SHALL BE REPORTED TO THE CONSTRUCTION MANAGER AND CENTRALPLANT OPERATIONS IMMEDIATELY.
32 THE CONTRACTOR SHALL SAFEGUARD THE OWNERS PROPERTY DURING CONSTRUCTION AND SHALL REPLACE ANY DAMAGEDPROPERTY OF THE OWNER TO ORIGINAL CONDITION OR BETTER.
33 THE CONTRACTOR WARRANTS TO THE OWNER AND THE ARCHITECT THAT ALL MATERIALS AND EQUIPMENT FURNISHED WILL BE NEWUNLESS OTHERWISE SPECIFIED AND THAT ALL WORK WILL BE OF GOOD QUALITY, FREE FROM FAULTS AND DEFECTS.
34 CONTRACTOR TO PROVIDE PORTABLE FIRE EXTINGUISHER UNITS IN RECESSED CABINETS AS SPECIFIED BY LOCAL AUTHORITY HAVINGJURISDICTION. LOCATION AND TYPE OF UNIT WILL BE DETERMINED BY LOCAL AUTHORITY HAVING JURISDICTION. THE MAXIMUMFLOOR TRAVEL DISTANCE SHALL NOT EXCEED 75 FT. TO THE NEAREST EXTINGUISHER FROM ANY POINT IN THE BUILDING WITHOUTNEEDING TO GO UP OR DOWN STAIRS.
35 THE CONTRACTOR SHALL BE RESPONSIBLE FOR LEAVING ALL FINISHED SURFACES CLEAN AT THE COMPLETION OF THE WORK ANDSHALL REMOVE ALL EXCESS MATERIAL AND DEBRIS FROM THE JOB REGULARLY WHENEVER POSSIBLE, PREVIOUS TRENCH CUTS SHALLBE USED TO MINIMIZE PAVEMENT CUTS.
36 WORKMANSHIP SHALL BE OF THE HIGHER QUALITY AND NOT LESS THAN THE MINIMUM STANDARDS AS EXPECTED OF THE APPLICABLETRADE OR PROFESSION, COMPLETELY FINISHED, SAFE, NEAT THROUGHOUT AND PERFORMED BY COMPETENT AND EXPERIENCEDWORKMEN. CONSTANT SUPERVISION OF WORK BY CONTRACTOR SHALL BE MAINTAINED.
37 ALL NEW CONSTRUCTION MATERIALS SHALL BE 100% ASBESTOS FREE.
38 NO HAZARDOUS MATERIALS WILL BE STORED AND/OR USED WITHIN THE BUILDING WHICH EXCEED THE QUANTITIES LISTED IN CBCTABLES 307.1(1) AND 307.1(2)
39 CONTRACTOR'S ACCESS SHALL BE APPROVED BY CLIENT, INCLUDING MATERIAL STORAGE AND VEHICLE PARKING. CONTRACTORSHALL LIMIT STORAGE AND PARKING TO THE DESIGNATED AREAS.
40 ITEMS OF A MECHANICAL OR ELECTRICAL NATURE MAY NOT NECESSARILY APPEAR ON THE ARCHITECTURAL DRAWINGS. SEE THEAPPROPRIATE DRAWINGS FOR ITEMS OF THIS NATURE.

3. LOCATE EXISTING REINFORCING BY NON-DESTRUCTIVE METHODS PRIOR TO DRILLING. EXISTING REINFORCING SHALLNOT BE CUT OR DAMAGED.
4. HOLES FOR INSTALLATION OF THE POST-INSTALLED ANCHOR SHALL BE DRILLED USING A DRILL THAT HAS ACARBIDE-TIPPED BIT THAT COMPLIES WITH ANSI B212.15. A REBAR CUTTING DRILL BIT IS NOT ALLOWED.
5. CONTRACTOR SHALL USE APPROPRIATE EQUIPMENT AND METHODS AS REQUIRED TO PROVIDE DRILLED HOLES FORPOST-INSTALLED ANCHORS IN ACCORDANCE WITH APPLICABLE STANDARDS, MANUFACTURER'S RECOMMENDATIONS,AND QUALIFYING (ICC) TEST REPORTS. CARE SHALL BE TAKEN TO PREVENT OVERSIZING, OVALING, AND/OR BLOW-OUTTHROUGH THE BACK FACE OF THE DRILLED MEMBER. IF OVERSIZING, OVALING, AND/OR BLOW-OUT OCCURS, THEEMPLOYED EQUIPMENT AND METHODS SHALL BE DISCONTINUED. 4. MAXIMUM MOISTURE CONTENT FOR 2X FRAMING SHALL NOT EXCEED 19 PERCENT (19%) AT TIME OFCOVER-UP (COVER-UP INCLUDES APPLICATION OF PLYWOOD OR GYPBOARD). 2X FRAMING SHALL BESTAMPED “S-DRY” OR “KD”. FOR PROJECTS NOT GOVERNED BY DSA OR OSHPD, 2X FRAMING MAY BESTAMPED “S-GREEN”. MOISTURE CONTENT OF 2X FRAMING SHALL BE TESTED AND INSPECTED PER FRAMED LUMBERSPECIAL INSPECTION NOTES.
5. BOLT HOLE DIAMETER SHALL BE THE DIAMETER OF BOLT PLUS 1/16" UNLESS NOTED OTHERWISE. PROVIDE WASHERSUNDER HEADS AND NUTS OF ALL BOLTS AND LAG SCREWS BEARING ON WOOD. ALL BOLTS SHALL BE RE-TIGHTENEDJUST PRIOR TO COVER-UP.
6. FRAMING HANGERS, CAPS, HOLDOWNS, POST BASES, ANCHORS, CONNECTORS, ETC., SHALL BE AS MANUFACTURED BY"SIMPSON COMPANY" OR APPROVED EQUAL TO BE SUBMITTED TO STRUCTURAL ENGINEER FOR APPROVAL PRIOR TOINSTALLATION.

NAILS SHALL BE COMMON WIRE (UNO) AND THENUMBER OF NAILS CONNECTING WOOD MEMBERS SHALL BE PER THE PLANS AND DETAILS BUT NOT LESS THAN THEAMOUNTS SET FORTH IN TYPICAL WOOD FASTENING SCHEDULE IN THE DRAWINGS. GALVANIZED COMMON NAILS SHALLBE USED FOR ALL EXPOSED EXTERIOR WOOD. COMMON NAILS SHALL CONFORM TO THE SPECIFICATIONS OF NAILPROPERTIES SCHEDULE BELOW:
9. TOE NAILS SHALL BE DRIVEN AT AN ANGLE OF APPROXIMATELY 30 DEGREES WITH THE MEMBER AND STARTEDAPPROXIMATELY 1/3 THE LENGTH OF THE NAIL FROM THE MEMBER END.
10. PRE-DRILL ALL LAG SCREW HOLES WITH DIAMETER (D) FOR UNTHREADED SHANK, 40% TO 70% OF D FOR THREADEDPORTION. THE THREADED PORTION OF LAG SCREWS SHALL BE INSERTED IN ITS LEAD HOLE BY TURNING WITH AWRENCH, NOT BY DRIVING WITH A HAMMER. PROVIDE SOAP OR LUBRICANT ON LAG SCREWS OR IN LEAD HOLE TOFACILITATE INSERTION AND PREVENT DAMAGE TO LAG SCREWS OR WOOD SCREWS.

2. INSTALLATION SHALL CONFORM TO THE MANUFACTURER'S INSTRUCTIONS AND THE APPLICABLE EVALUATION REPORT,AND SHALL BE INSTALLED BY PERSONNEL TRAINED TO INSTALL THE TYPE OF POST-INSTALLED ANCHOR BEINGINSTALLED.
3. LOCATE EXISTING REINFORCING BY NON-DESTRUCTIVE METHODS PRIOR TO DRILLING. EXISTING REINFORCING SHALLNOT BE CUT OR DAMAGED.
4. HOLES FOR INSTALLATION OF THE POST-INSTALLED ANCHOR SHALL BE DRILLED USING A DRILL THAT HAS ACARBIDE-TIPPED BIT THAT COMPLIES WITH ANSI B212.15. A REBAR CUTTING DRILL BIT IS NOT ALLOWED.
5. CONTRACTOR SHALL USE APPROPRIATE EQUIPMENT AND METHODS AS REQUIRED TO PROVIDE DRILLED HOLES FORPOST-INSTALLED ANCHORS IN ACCORDANCE WITH APPLICABLE STANDARDS, MANUFACTURER'S RECOMMENDATIONS,AND QUALIFYING (ICC) TEST REPORTS. CARE SHALL BE TAKEN TO PREVENT OVERSIZING, OVALING, AND/OR BLOW-OUTTHROUGH THE BACK FACE OF THE DRILLED MEMBER. IF OVERSIZING, OVALING, AND/OR BLOW-OUT OCCURS, THEEMPLOYED EQUIPMENT AND METHODS SHALL BE DISCONTINUED. ADDITIONAL DRILLING SHALL NOT BE RESUMEDUNTIL THE SEOR HAS PROVIDED APPROVED REPAIR PROCEDURES. THE CONTRACTOR SHALL BE RESPONSIBLE FORTHE COST OF ALL SUCH REPAIRS. WHEN RESUMING DRILLING, THE CONTRACTOR SHALL MODIFY THE PROCEDURES ASNECESSARY TO PREVENT FURTHER DAMAGE.
6. HOLES SHALL BE CLEANED OF DUST AND DEBRIS, USING A WIRE BRUSH AND COMPRESSED AIR OR MANUFACTURER'SBLOW-OUT BULB (AS PER MANUFACTURER'S RECOMMENDATIONS) AS REQUIRED TO REMOVE PARTICULATE DEBRISAND TO ACHIEVE A RELATIVELY DUST-FREE SURFACE.
7. OIL, SCALE, AND RUST SHALL BE REMOVED FROM THE POST-INSTALLED ANCHOR AND HOLES SHALL BE DRY, PRIOR TOINSTALLATION.
8. THE EFFECTIVE EMBEDMENT DEPTH FOR POST INSTALLED ANCHORS SHALL BE AS NOTED ON THE DETAILS. FOREXPANSION ANCHORS, REFER TO THE APPLICABLE EVALUATION REPORT FOR THE CORRESPONDING MINIMUM HOLEDEPTH AND NOMINAL EMBEDMENT.
