BWC-CWDM multi-channel lasersource employs stabilized Fabry-Perot laser diodes. All quotations written or verbal are valid for 30 days fromthe date of quotation unless stated otherwise. Pricesare based on your requested specifications and quanti-ties, and are subject to change if any changes are madefrom the original request.

Prices quoted by B&W TEK are for F.O.B. factory.Special items will be individually priced per providedspecifications. All published prices are subject tochange without notice. An extra 20% handling fee oneach item is applied to international orders.

Purchase Orders and PaymentMethods

Purchase orders are accepted by mail, e-mail or fax.Hard copies of the purchase orders are required. Tominimize errors, please reference the B&W TEK quota-tion numbers. Provide also item description, purchaseorder number, ship-to and bill-to address and shippingmethod desired.

Open accounts of net 30 with approval of credit limit.Contact B&W TEK for further information.

Orders placed with MasterCard or VISA may be ac-cepted for US government customers only. Pleaseprovide account number, card holder, billing address, andexpiration date that are associated with the card to beused.

Orders may be shipped via UPS or Federal Express on aCOD basis. Cash, a money order or a bank or companycheck is required at the time of delivery.

Delivery and Shipping

Rush orders placed by phone (for items in stock at timeof order) will be shipped within 2 working days. Most

other standard items can be shipped within 6 weeks.Delivery times for special orders will be established perquotation.

FedEx is our default shipping carrier although UPS andairfreight are available. If not specified clearly on thepurchase order at the time of ordering Airborne expresswill be used by B&W TEK at the time of shipping. Uponrequest, the tracking numbers will be provided.

Returns and Cancellation Fee

A Return Material Authorization number (RMA) is re-quired for any returned goods. Original purchase ordernumber, date of shipment and serial number must beprovided before the RMA can be issued. All the returnsshould be shipped with the original packaging materi-als with the assigned RMA number(s) clearly marked.The restocking fee will be determined after the items areinspected at B&W TEK. No product(s) will be acceptedfor restocking after 45 days. A cost estimate will beprovided by B&W TEK for out of warranty items. Spe-cially designed products damaged by the customer maynot be returned.

Should it become necessary to cancel or modify ordersprior to shipment, a restocking fee of 15 to 50% of thetotal order amount will be charged by B&W TEK Tek forreturns to stock. Specially designed products damagedby the customer may not be returned.

International Customers

Please make payment in United States dollars to bedrawn on a United States Bank. Certain items may besubject to export control and require a validated exportlicense.

Warranty Terms and Conditions

B & W TEK’s end user products, OEM modules, andcomponents are warranted to be free from defects inmaterials and workmanship for a period of 12 months, 6months, and 90 days, respectively, from the date ofinitial shipment. This warranty does not extend toincidental or consequential damages and to damagecaused by negligent or improper handling in use, stor-age, nor for products for which the original identificationmarkings or labels have been removed, defaced oraltered.

Special contracts or contracts for nonstandard productsmay have modified terms of warranty and, in suchcases, the terms as stated in the individual contractmust be signed by the duly authorized officer of B & WTEK and will supersede the standard terms.

B & W TEK will make final determination as to cause orexistence of defect and, at its option repair or replacethe products, which prove to be defective during thewarranty period. Products replaced under warranty willbe warranted only for the balance of the warranty periodfrom the original supplied equipment.

This warranty extends only to the original purchaser ofthe equipment from B & W TEK. The purchaser mustnotify B & W TEK within 15 days of first noticing thedefect and promptly return the defective product uponreceipt of RMA number(s) before expiration of thewarranty period.

Products believed by purchaser to be defective shall bereturned to B & W TEK transportation and insuranceprepaid by purchaser. Repaired or replaced products willbe returned to purchaser by B & W TEK, FOB citydestination within the Continental United States, Trans-portation beyond these limits will be charged to pur-chaser.

Warranty Return Procedure

Review terms of purchase and date of shipment todetermine validity of warranty claim. Warranty claimshould only be made for products within terms of war-ranty policy.

Safety Warnings

The laser products described in this catalog emitvisible or invisible radiation power. They aresafe to operate only when the users follow allsafety measures:

a) Post warnings in the area where the laserbeam passes to alert those present.

b) Keep all unauthorized personnel out of thearea where the laser is operated.

c) Whenever the laser is running and the beamis not in use, it is a good practice to mechani-cally block the radiation path.

d) Never look directly into the laser beam pathor scattered laser light from any reflective sur-faces,

e) Never look directly into the laser source.

f) Maintain experimental setup at lower levelto prevent inadvertent beam-eye contact.

g) As a precaution against accidental expo-sures to the laser beam or its reflection, op-erators should wear laser safety glasses withsufficient attenuation at the laser emissionwavelength .

CDRH Compliant System

The BWB, BWN, BWT, and BWR series lasersystems, which are certified to be in compliancewith Class IIIa, IIIb, or IV, are equipped with key lockswitch, remote control connector, laser radiationemission detector, emission time delay (in thelaser driver), a beam attenuator (shutter), emer-gence switch (Class IV only) and appropriatewarning labels. The following is a consolidate labelwhich is located on the laser head. It includesaperture label, warning logotype label and certifica-tion/identification label designed to warn the user ofpotential hazard. There are three blanks will befilled with a specific laser wavelengths (350nm –1650nm), power level (1mW – 5W) and properlaser class (IIIA, IIIB and IV).

Non-CDRH Compliant OEM modules

The BWB, BWN, BWT, BWT, and BWR serieslaser are also provided to customers as an OEMcomponent to be incorporated into a system. Themodules will be provided to customer either laserhead only, or with a B&W TEK’s OEM driver or tobe incorporated with the drivers provided by cus-tomers. In the above mentioned “OEM modules”:

1) OEM versions are non-CDRH compliant de-vices. These modules are sold as a componentinto other equipment.

2) An OEM label is attached to the modules andstates “This product is sold as a component forincorporation into other equipment. The purchaserassumes responsibility to comply with US 21 CFR1040 with regard to the use of this laser and itsintroduction into commerce.”

3) B&W TEK will provide all the necessaryassistances to help customers achieve the re-quired CDRH compliance.

4) There are built-in safety features in OEM laserdriver. Such as the emission delay, emissionindicator and interlock which will enable purchaserto easily reconfigure the system to be in compli-ance with CDRH requirements.
