Graduation Project “School-to-Career Experience”
Ontario High School Class of 2016
Requirements
The Oregon Department of Education has mandated all seniors seeking a diploma must
provide evidence he/she has met or exceeded the Career-Related Learning Standards and Extended
Application Standards. These standards are fundamental skills essential for success in employment,
college, family, and community. It is Ontario High School’s belief that the Career-Related Learning
Standards and Extended Application Standards will be most meaningful when demonstrated through
integrated, interdisciplinary approaches and hands-on activities such as accomplishing a task or
discovering a solution to a problem in the classroom or career-related learning experience.

Paragraph 1: Explain how Ontario High School has helped prepare you for college or career. Choose two of the nine essential skills
(see listed above) and provide examples of how you have demonstrated these skills in high school. Explain how these skills will be
useful to you after graduation.
Essential Skill #1: Read and comprehend a variety of text
Essential Skill #2: Write clearly and accurately
Developed these skills in all subjects, but especially in English classes doing the following projects:
o Read and annotated multiple texts
o Informative reading work samples 1 time per quarter
o Literary analysis
o Socratic Seminars over readings
o Conducted research
o Wrote in the explanatory / informative and argumentative modes of writing (1 work sample minimum per quarter)

o I plan on entering the English teaching field and will need to have a strong grasp of each skill so I am able to teach
others
o All aspects of life require careful reading, writing, and critical thinking skills
Paragraph 2: Describe your experiences from the community service or job shadow assignment. Explain where you were and who
you worked with. Explain what you learned or did (be specific). Add specific details, examples and personal insight about what you
got from this experience and how it will connect to your future plans or goals.
Where: I job shadowed at Ontario High School located at 1115 W. Idaho Avenue; Ontario, Oregon
Who: Job shadowed Ms. Poff, a senior English teacher with 6.5 years of experience
I observed how many meetings a teacher has, the great need to multi-task: take roll, do warm-up, listen for on-task behavior,
record grades, continually provide examples, feedback, make copies, plan engaging lessons, and ensure all students are on-
track to pass English so they can graduate
Personal Insights: teaching is much more difficult and time-consuming than I first thought
o Teachers care deeply about each child
o They work ALL the time—at home, on the weekends, etc. to make sure they are giving 100% to their classes
o The rewards are seeing students progress and succeed—teachers are in this profession because they care about kids,
not for the money
Connections to future goals: This job shadow confirmed my desire to go into the teaching field. It may be a lot of work, but
it is worth it.
Paragraph 3: Describe your short-term goals. Describe your personal, educational or career plan in terms of what you hope to
accomplish in the first year or two after graduation. What specific challenges do you need to start thinking about? What do you need
to do to ensure that you are successful? What do you need to learn more about as you prepare to meet your short-term goals?
Personal Short-term Goals:
o Plan on moving into my own apartment with a roommate
o Find a part-time job working with kids to help fund my college courses and to provide me with experience working
with kids
12
Educational and career short-term goals: Upon graduation from high school, I will attend Portland State University to
work on my Bachelors in Education.
Challenges to be faced to achieve goals:
o I need to work, manage time between work and school
o apply for grants, FAFSA, and scholarships to help pay for my education
Ensuring success:
o Make it to class daily
o Complete all assignments before playing
o Job shadow educators
o Ask professors questions when needed
Additional learning required to meet goals:
o Teaching strategies
o Classroom management techniques
o Confidentiality laws
o Lesson planning techniques and requirements
o Public speaking
Paragraph 4: Describe your long-term goals. Explain your personal, educational or career plan in terms of what you hope to
accomplish in the first five to ten years after graduation. What specific challenges do you need to start thinking about? What do you
need to do to ensure that you are successful? What do you need to learn more about as you prepare to meet your long-term goals?
Personal long-term goals:
o Start a family
o Live independently
Education and career goals:
o Graduate with my Master’s in Education, specifically teaching English for grades 9-12
o Find a job as a high school English teacher
Challenges:
o Finding a teaching job in the competitive work field—especially when most employers are looking for candidates
with a minimum of five years teaching experience
o First year teaching and learning the ins and outs of teaching will be challenging, but will set me up for a strong
future in education
Ensuring success:
o Develop a strong, detailed resume and teaching portfolio
o Practice good interviewing skills
o Interview educators in the field to ensure I am well-versed in issues dealing with education
Paragraph 5: Write a conclusion that explains how your education has helped prepare you for your future goals. Restate main ideas
from the paragraphs above. Why is it important for you to reflect on what you have gained from your education? What skills do you
need to continue working on after you leave high school? Is there anything you wish you had done differently in high school? What
advice do you have for future high school students?
How education has helped prepare me for future goals:
o Essential Skills:
Developing the skills of reading and writing throughout my high school years will be invaluable throughout
my life—both in life, in college and as a high school English teacher
o College plans:
My studies at Portland State University will prepare me to be a skilled educator
o Career preparation:
With a Master’s in Education, a good resume, a strong teaching portfolio, and being well-practiced in
interviewing techniques will help me reach my career goals
o Skills needing continual work and improvement:
I will continually need to better my skills in reading, writing, public speaking, and technical skills as they
will all be integral in the education field
o What I wish I had done differently:
13
Not procrastinated and ensured I was passing all my classes each year. I wish I had taken more college
credit classes and been part of more clubs and sports.
o Advice for future students:
Do not procrastinate in school
Try your best on everything
Attend more extracurricular activities
Make good friends who will encourage positive things: college, graduation, good grades, etc.
Take responsibility for your own education and life—do not blame others for your choices

JOB SHADOW/COMMUNITY SERVICE PACKET
Job Shadow Student Information Purpose
The purpose of a job shadow is to provide students with an opportunity to investigate a career path of
interest. If you choose to conduct a job shadow, you will learn what someone actually does on the
job—within the scope of a day (or two). It will likely be your first opportunity to see the many attributes
of a career beyond what school-age students do for employment. Please understand that it will be
your responsibility to make contacts and follow through with all steps in the process. Keep in mind
that your focus—aside from the project itself—is communication; organization; problem-solving;
and teamwork.
Job Shadow
1. Decide which career you are interested in and a business/person to shadow. The Host CANNOT be
a relative or another student.
2. Narrow down the choices, and make contact (phone or in person) until accepted.
3. *See: Helpful Hints page and Responsibilities page.
4. Conduct Job Shadow (day) and take notes for journal—the format is up to you. The more detail you
include, the easier your final reflection (essay) will be to write! See suggestions and example.
5. Give your cooperating host the Business Host Evaluation form from your pack and wait for her/him to
fill it out. (PART OF YOUR GRADUATION PROJECT NOTEBOOK)
6. Type your notes in the form of a journal. (PART OF YOUR GRADUATION PROJECT NOTEBOOK)
7. Write a thank you letter to your host---print 2 copies, send one to your host business and one to put in
your graduation notebook. (PART OF YOUR GRADUATION PROJECT NOTEBOOK)
8. Turn in the following to your Home Room teacher (as soon as possible when school begins if you
complete this in the summer):
- Journal
- Business Host Evaluation form
- copy of Thank you letter
15
Community Service Student Information Purpose
The purpose of community service is to provide students with an opportunity to give back to the
community while investigating a career path of interest. For successful community service, you will
need to: (1) locate or create a service project—related to your career interest, (2) get it approved, and
(3) follow through with the remaining steps in the process as outlined. Keep in mind that your
focus—aside from the project itself—is communication; organization; problem-solving; and
teamwork.
Community Service
1. Decide what service you are interested in and contact an adult business/person to oversee or “Host”
your project. The Host CANNOT be a relative or another student.
2. Narrow down the choices, and make contact (phone or in person) until accepted.
3. Make a plan regarding time and resources to complete your service project.
4. *See: Helpful Hints page and Responsibilities page.
5. Conduct community service (day) and take notes for journal—the format is up to you. The more detail
you include, the easier your final reflection (essay) will be to write!
9. Give your cooperating host the Business Host Evaluation form from your pack and wait for her/him to
fill it out. (PART OF YOUR GRADUATION PROJECT NOTEBOOK)
10. Type your notes in the form of a journal. (PART OF YOUR GRADUATION PROJECT NOTEBOOK)
11. Write a thank you letter to your host---print 2 copies, send one to your host business and one to put in
your graduation notebook. (PART OF YOUR GRADUATION PROJECT NOTEBOOK)
12. Turn in the following to your Home Room teacher (as soon as possible when school begins if you
complete this in the summer):
- Journal
- Business Host Evaluation form
- copy of Thank you letter
16
Helpful Hints and Responsibilities
1. Make the Phone Call to Set-Up your Job Shadow:
2. Introduce yourself and explain your plan in a business/grown-up manner. Here’s a possible script.
3. Two days prior to meeting with your host, call to confirm the hours and time, and ask about the proper
clothing/attire. Also ask if there are any other special instructions that you will need to know.
4. It might be helpful to interview the person who is your host after your experience. Here are some possible
questions you might ask—be sure to take notes in your journal so you will remember the conversation!
4. Journal (take notes about)* your experience to include: a) Project description
b) Time spent (total)
c) Date(s) of work
d) People involved
e) What you accomplished: communication; personal management; teamwork; career development
f) New things you learned
g) What went well
h) What challenges you had
i) How you feel about the experience
j) Anything unique or funny
*Journal entries can be brief but should be well written. See example.
This document will “drive” your final written reflection, so it should be detailed enough to use as a
reference for remembering. You may choose how to format your journal.
5. Upon conclusion of your project, give your cooperating host the Business Host Evaluation form from
your pack and wait for her/him to fill it out.
6. Make sure you get your host’s address. Write it down! And make sure you have your host fill out the
HOST EVALUATION.
7. Write up your notes in the form of a journal—word processed.
TAKE PICTURES! If it is appropriate to your project, pictures are worth a thousand words.

Journal: Job Shadow (identifies which project you are doing)
June 30, 2013 (date for written journal completion—not actual job shadow/com. service date)
Project Description
I was able to complete my job shadow with Dr. Andres Cohen who is a neurosurgeon in Brooklyn, New York. Neurosurgeon
was one of the career paths I chose to investigate as indicated by CIS assessments of skill and interest. Dr. Cohen included
me in the following routines:
- Spinal surgery
- Physical therapy
- Surgery follow-up
- Hospital rounds
Time Spent
I spent a total of 23 hours over the course of two days with Dr. Cohen. That was above the requirement!
Dates of Work
June 2nd and June 3rd of 2013 were the days I shadowed Dr. Cohen.
People Involved
The people involved were patients (who must remain nameless for privacy reasons); The director of The Brooklyn Hospital,
Dr. Richard Becker; Dr. Anders J. Cohen, the head of the neurological department; Ruthann Crivitz, R.N.; and several
hospital staffers.
Accomplishments
I was able to check in with the doctor to join his early morning hospital rounds. I watched from a viewing room when he
operated on a young girl’s spine. He also allowed me to participate in debriefing the family after surgery. Of course I was
not allowed to do anything medically, but Dr. Cohen did allow me to make predictions or educated guesses about problems
concerning patients. I left his office at 5:30—after having met him at 6:00 a.m. for morning rounds. Both days I left
exhausted. I learned a lot about organizing my time and making plans to accomplish this.
Learned
I learned about: preparing and scheduling surgery; what surgery looks like from an outside perspective; how charts are
kept; the importance of talking/listening to patients and their families; how drug records are kept and updated; that it takes
a long time to complete one day’s work.
What Went Well
I was able to make some good guesses about what was wrong with one patient, and I loved watching the surgery.
Challenges
Getting up at 5:00 a.m. to make rounds was the greatest challenge.
Feelings About the Experience
I realized that—other than getting up early—I loved everything about the day’s work of a neurosurgeon.
Unique or Funny Experience
Because there was an emergency surgery, my two days were pretty serious; however, when we were walking down the hall
after surgery, there was a little old man running away from the nurses with his derriere hanging out!

419 S. Beaver-duck Road
Ontario, Oregon 97914
Salutation Dear Mr. Smith: Body of letter Thank you for allowing me to use your shop for my community service project. The time spent
was extremely valuable in that I was able to make a mess without worrying that someone would
be upset—as I learned how to direct young elementary students in building kaleidoscopes. I
learned a great deal from you, and I hope to follow in similar steps by opening a kaleidoscope
shop of my own one day. I appreciate the manner in which you approach customers and students.
Your demeanor is kind and makes it easy to approach you.
Sincerely,
Sally Student
