S. 47F(I) had deliberately cut himself(Self Harm Actual, -9CH905). Operatons Manager
dispatched CSM F^tonand CSO N. .--ht0 ,o0m 3, .LWB weretalkinglothe
client through a gap between the wardrobe and the door. It was observed that the bunk bedhad
been pushed againstthe internalside of the wardrobe, preventing entry by pushing overthe
wardrobe. At approximately 23.0hrs Operaiions Manager^.. returned to the room after
briefing D/AC and Serco Senior Management. Clients' 47F(I) ands.s. 47F(,)
, friends o1S- 47F(,) were at the room. The friends were apparently assistings. 47F(,)In
attempting to gels- 47F(I) to stop self haming and to remove the barricade. The conversationwas monitored by Operations Manager. ^ through LWB interpreting. It was observedIhat

three clients forced open the door, butthe room was accessed and client'' ( ) wasfoundIn
the bathroom hanging by the neck with a rope fashioned by strips of plaited bed sheet. A100phad
been formed at one end, knotted off and the other end fed through the loop. Clients' 47F(,)had
placed this loop over his head and around his neck. The other end of the rope was secured bya
reefknolto the shower curtain rail. Therewas an overturned chairin the bathroom. though atthis
time ills riot understood if it was used by the chant in facilitation of self harm. clientsS. 47F(I)s. grabbed clients' 47F(^!^ supporting his weight. Operations Managerands.

with the NT Police at approximately 2000hrs for Royal Darwin Hospital.
