ABOUT THIS POSITIONCHANGING MEDICINE. CHANGING LIVES. 5

The HCCC received NCI-designation in 1999 and comprehensive designation from the NCI in 2000. It has undergone successful competitive renewal of this designation twice since that time. The HCCC is organized in two different dimensions. The formal research programs of the HCCC are based on scientific discipline and include cancer immunology and immunotherapy, cancer signaling and experimental therapeutics, cancer genetics and gell growth, tumor imaging, free radical cancer biology, and cancer epidemiology.

In addition, a number of cancer type specific muliti-disciplinary oncology groups (MOGs) have been developed and are active in both clinical and research efforts. The HCCC has a robust Clinical Trials Support Core that includes 35 full-time staff who assist in protocol development, management, and the conduct of trials. The Clinical Trials Support Core also supports bedside to bench translational research including the prospective clinical registries of the HCCC. These are known as the Molecular Epidemiology Resources (MERs), work closely to the HCCC Tissue Procurement Core and biobanks, and have enrolled over 4,500 subjects in a variety of cancer types.

Endowed Positions and ProgramsThe Holden Endowment to support Experimental Therapeutics was established in 2000 to support research geared towards advancing knowledge of cancer and application of this knowledge to the discovery and development of new cancer therapeutics. This research is expected to include the design and synthesis of novel chemicals and testing of active agents in humans.

The endowment consists of two components.:

Holden Family ChairThis fund supports a chair in the University of Iowa Roy J. and Lucille A. Carver College of Medicine for the Director of the Roland W. Holden Family Program for Experimental Cancer Therapeutics.

Roland W. Holden Family Program for Experimental Cancer TherapeuticsThe Program itself is supported by the Roland W. Holden Family Endowed Fund.

Support for the clinical research infrastructure of the Holden Comprehensive Cancer Center comes from other sources so support from the Roland W. Holden Family Program for Experimental Cancer Therapeutics can be directed towards specific research projects in Experimental Cancer Therapeutics.

Dr. Raymond Hohl was the founding Director of the Program. Dr. Hohl’s efforts focused on the RAS pathway and novel approaches to targeting the isoprene intermediates in the cholesterol biosynthetic pathway. His efforts included extensive collaborations with synthetic chemists in the Department ofChemistry as well as a broad range of investigators across the Cancer Center. The next recipient of the Holden Chair will be expected to leverage the infrastructure and contributions developed by Dr. Hohl while also applying their own research expertise to goals of the Holden Program in ExperimentalTherapeutics.

Holden Comprehensive Cancer CenterEfforts to form a comprehensively organized program

in cancer began in the 1970s when former Dean John

W. Eckstein, MD charged the College of Medicine’s

Committee on Cancer with the responsibility of reviewing

and developing research, demonstration, and education

programs in cancer.

Richard L. DeGowin, MD began the effort by working with faculty members in the College’s of Medicine, Nursing, Dentistry, Pharmacy and Liberal Arts and Sciences and after much work, a plan was implemented for the creation of a more organized cancer program at the university. Following presentation to the Board of Regents, State of Iowa, in 1980 the new program was conferred special “center designation, ” and DeGowin became the Cancer Center’s first director.

Since 1980, the Cancer Center has experienced tremendous growth and achieved the highest level of recognition status given by the National Cancer Institute—comprehensive—in 2000. To reflect this achievement, and in recognition of a landmark, $25 million gift from four generations of the Holden family of Williamsburg, Iowa, the Cancer Center was renamed the Holden Comprehensive Cancer Center.

Holden Family SupportThrough years of visionary support for the Holden Comprehensive Cancer Center at the University of Iowa, the Holden family has helped establish the UI as a major center for cancer research, patient care, and education. The Holden Comprehensive Cancer Center was so named in 2000, when four generations of the Holden family made a landmark gift of $25 million to support what was then the University of Iowa Cancer Center. In the 13 years since the Holden gift, world-class research scientists, physicians, and other health care professionals have worked tirelessly to advance research that helps prevent, detect, and treat cancer for our patients. The Holden family gift was made in honor of the late Roland W. Holden, who died in 1995. Holden founded Holden Foundation Seeds Inc., a corn seed research and genetics company based in Williamsburg, Iowa, 30 miles west of Iowa City, in 1937. His family chose to celebrate his legacy by supporting research and treatment that may eventually cure the disease that took his life.

Contributors to the $25 million gift included Ruby Holden; Ronald, Arlene, and Kathy Holden; Susan and David McCurry; Karol Holden; Mary Ann and Curtis Blythe; and the Roland and Ruby Holden Foundation.

The gift, one of the largest received for UI Health Care initiatives, has supported a full range of critical programs within the Holden Comprehensive Cancer Center, including:

RESEARCH

Cutting-edge research, new cancer investigators, and studies that have translated laboratory discoveries into innovative patient treatment options.

The Clinical Trials Support Core, which include trials that come from our own labs, collaboration with other cancer centers, and partnerships with biotechnology and pharmaceutical companies. Treatments that would not otherwise be available in Iowa are available to our patients because of these trials.

PATIENT SERVICES AND CARE

The UI Palliative Care Service—a team of nurses, social workers, chaplains and physicians that assists terminally ill patients and their families. This interdisciplinary program provides emotional and spiritual support and help in planning for care outside the hospital, as well as pain control and symptom management.

Cancer Immunology and Immunotherapy

ABOUT THE PROGRAM

The Cancer Immunology and Immunotherapy Program consists of faculty investigators who are linked by common research interests in the functioning of the immune system, and how that system can be manipulated to help prevent or treat cancer and complications of cancer. The investigators in this program are working on problems that extend from fundamental molecular questions to those involving how recent advances in basic immunology can be translated into clinical utility for the treatment of cancer.

Although many of investigators in the Cancer Immunology and Immunotherapy Program are pursuing related questions, the individual research programs are diverse, allowing researchers in this program to attack basic biologic issues, and develop new clinical therapies, from many angles. Model systems being studied range from an evaluation of signaling properties of T and B cells, to increasing the efficacy of vaccines and immunotherapy in living animals, through the conduct of novel clinical trials related to cancer immunotherapy. Clinical applications, as a direct result of more basic investigations, have arisen from within the group and it is expected that this will continue.

Because of the diversity of research interests reflected within the membership of the Cancer Immunology and Immunotherapy Program, a wide range of technologies and approaches are available to investigators. Each of our investigators benefits from interactions with others in the program, as well as members of additional Cancer Center programs, resulting in a synergism yielding new insights, perspectives, and collaborative efforts.

Tumor Imaging Program

ABOUT THE PROGRAM

The goal of the Imaging Program is to foster interdisciplinary research that integrates modern medical imaging techniques into basic, translational, and clinical cancer research studies. The methodologies will include computed tomography (CT), magnetic resonance imaging (MRI), ultrasound, positron emission tomography (PET), gamma scintigraphy, optical imaging, and autoradiography. Metabolic imaging extends the capability of imaging well beyond anatomy. It includes imaging blood flow, vascular permeability, metabolic rates (glucose, oxygen, DNA synthesis, protein synthesis, membrane synthesis), receptor distribution and density (estrogen, testosterone, somatostatin), apoptosis, angiogenesis, cell trafficking, and reporter gene activation. Members of the imaging program are working in all these areas. The importance of having an Imaging Program is that it brings these investigators together to collaborate, and share ideas, imaging strategies, and data analysis methods.

The Imaging Program has the following specific areas of focus:

• Metabolic assessment of response to therapy

• Iodine symporter as a reporter gene and as a novel therapy

• Cell trafficking

• Development of new radiopharmaceuticals

• Development of novel analysis methodologies

Free Radical and Cancer Biology Program

ABOUT THE PROGRAM

For many years it has been recognized that many agents used to treat cancer (i.e., radiation and chemotherapy agents) cause oxidative stress that contributes to the mechanism by which they kill cancer cells as well as damage normal tissues. Recently, it is becoming increasingly clear that metabolic oxidation/reduction (redox) reactions are also altered in cancer vs. normal cells. The current consensus is that cancer cells may exist in a chronic state of metabolic oxidative stress that may represent a significant underlying mechanism contributing to malignancy. A unifying goal of the investigators in the Free Radical Cancer Biology Program is to utilize a comprehensive understanding of redox biology to develop novel biochemical rationales for improving cancer therapy taking advantage fundamental differences in oxidative metabolism between cancer vs. normal cells.

The researchers in the Free Radical Cancer Biology Program (FRCBP) focus on studying the biological consequences of inherent differences in redox metabolism between cancer vs. normal cells to determine the role that redox reactions play in cancer phenotypic changes relevant to cell growth, differentiation, signal transduction, metastasis, functional molecular imaging, and cancer therapy. A unifying goal of the program is to utilize a comprehensive basic science understanding of cancer cell redox biology to develop novel biochemical rationales for improving cancer therapy. Three overlapping themes exist within the program.

Theme 1Understanding the role of redox biochemistry in regulation of genetic and epigenetic gene expression pathways relevant to cancer biology. Within this theme are focus groups exploring redox and gene expression globally, as well as gene expression of key redox molecules such as glutathione, thioredoxin, SOD, and AP2.

Theme 2Understanding the role of redox regulation in cell biology. Within this theme are focus groups exploring the effects of intracellular redox environmental changes on the cell cycle, and on invasion and metastasis.

Theme 3Exploring the role of oxidative stress in cancer therapy outcomes. Within this theme are focus groups exploring the role of manipulating oxidative stress to improve cancer therapy as well as using imaging of oxidative metabolism as an approach to monitor and guide therapy.

Cancer Epidemiology Program

ABOUT THE PROGRAM

The overall goal of the Cancer Epidemiology Program is to save lives by increasing our understanding of what causes cancer, promoting cancer prevention behaviors and early detection and treatment, and developing improved diagnostic tests and treatments. When cancer cannot be prevented, we are also interested in research that will extend the quantity and quality of

life for the cancer survivor.

A major strength of the Cancer Epidemiology Program is its extension beyond the walls of the medical center, as its primary research population is residents across the state of Iowa. This is based on the role played by the University of Iowa in public health across Iowa, including cancer epidemiology. Members of the Cancer Epidemiology Program

are responsible for the State Health Registry of Iowa (SHRI), Iowa’s statewide cancer surveillance program that is part of the Surveillance, Epidemiology, and End Results (SEER) Program of the National Cancer Institute (NCI). This resource allows researchers in the Cancer Epidemiology Program to perform outstanding population-based cancer research. Information gained from these studies can then be applied to other states and other populations.

The Cancer Epidemiology Program has three focus areas.

• Cancer Etiology

• Cancer Health Services and Outcomes

• Cancer Prevention and Control

Cancer Genomics and Cell Growth Program

ABOUT THE PROGRAM

Research in the Cancer Genomics and Cell Growth Program focuses on genetic and molecular events controlling normal versus malignant cell proliferation, differentiation and survival. The overall goal of the program is to define biologically significant genetic and molecular alterations in tumor cells so that information can be used to more effectively detect and treat human cancers. There are two major overlapping themes within the umbrella of the Progam’s research.

Theme 1 Genomic organization and cancer gene expression/analysis

Theme 2 Cell growth, differentiation, survival and transformation.

Major accomplishments of the Cancer Genomics and Cell Growth Program over the past funding period are best reflected by its identification and characterization of novel cancer genes and/or biomarkers, development of novel animal tumor models to discover and assess cancer gene function in vivo, and increasing collaboration as demonstrated by coalescence of program members into effective working groups. Key examples of intraprogrammatic research collaborations include a) studies revealing the role of chromatin binding proteins (such as RPA, DNA polymerases, HP1, transcription factors) in gene expression and how that impacts the cancer phenotype, b) studies defining basic mechanisms of viral infection and replication, and how that leads to cell transformation, and c) the discovery and subsequent functional analysis of novel genetic pathways of carcinogenesis. There are numerous past and present productive collaborations both between members of the Program, and with members of other Cancer Center programs. There are currently 16 NIH and NCI funded projects involving multiple Program members and/or HCCC members from other programs serving as co-Investigators. The program consists of 36 members from 12 departments (8 basic science and 4 clinical departments) and 5 Colleges. Peer-reviewed, research funding for this program totals $8,449,398 with $2,583,657 coming from the NCI.

Members of the Program have 300 cancer-related publications with nearly 30% collaborative publications (17% intra-programmatic, 12% inter-programmatic) during the last grant period.

SERVICES

Investigators who are interested in obtaining human tissue samples from the Facility should begin by contacting the TPC office to discuss the project and determine if the core can potentially meet the project tissue requirements.

If the discussion concludes that the request is ready to advance to the stage of making a formal application, the investigator will be directed to the core web site to complete an online application.

Completed applications will be forwarded for review to the Tissue Utilization Committee. The Committee will recommend approval or disapproval. Priority will be given to those studies that are supported by peer reviewed funding or foster collaborative work. A signed IRB approved usage agreement is required.

The Tissue Procurement Core has worked with the HCCC to develop a federated system of disease-specific tissue biorepositories. In this system, all repositories use the same informatics system supported by the HCCC and institution (we have recently licensed and are currently transitioning to Biomatrix). This allows investigators across the institution to know what samples are available for research. Each of the federated biorepositories retains oversight on use of samples from their own biorepository with the expectation they will provide qualified investigators access to samples. Once a year, each repository submits an application to the Tissue Utilization Committee describing requests for tissue that have been approved or denied and other information to assure equitable access to samples. This system has been working well and combines the incentive that comes from local control with the efficiency, quality assurance and transparency that comes from a more centralized approach.

Roy J. and Lucille A.Carver College of MedicineState-supported medical education in Iowa traces back to

the winter of 1850-51, when the state legislature recognized

the Keokuk College of Physicians and Surgeons as the

official medical department of the State University of Iowa.

The Keokuk College had begun operations a year earlier as the College of Physicians and Surgeons of the Upper Mississippi in Davenport, graduating 15 physicians before its move to Keokuk. It would continue to operate the state-sponsored medical college for another 20 years, and as an independent institution for an additional 40 years.

In the late 1860s, a prominent Davenport surgeon named Washington Freeman Peck initiated efforts to create a medical college in Iowa City. With support from Judge John F. Dillon (a patient of Peck’s and a graduate of the original Davenport medical college) and the Honorable John P. Irish (Iowa City newspaper editor, state legislator, and a member of the university board of trustees), the Iowa City medical department gained approval as the official university medical college in 1870. The UI college was a founding member of the Association of American Medical Colleges in 1876.

The college opened doors for its first class on Sept. 20, 1870. The faculty consisted of eight professors, including Washington Peck as dean and professor of surgery, and Judge Dillon as a professor of medical jurisprudence. The first class consisted of 37 students, including eight women. The Iowa College of Medicine thus became the first public institution in the country to admit women to its class. The curriculum for the first class consisted of a two-week course of lectures followed by 16 weeks of clinical training.

The College Today

The University of Iowa Roy J. and Lucille A. Carver College

of Medicine is a nationally ranked medical school with

exceptional balance across its three missions of research,

education, and clinical care. It was founded in 1870 and

was the first public medical school in the country to admit

women on an equal basis with men.

The college’s annual budget is more than $600 million, and it employs over 1,000 faculty and more than 1,700 staff. The faculty teaches 640 medical students, 972 resident and fellow physicians, 200 associated medical science students, and 5,000 undergraduate students.

U.S. News & World Report consistently ranks the college among the nation’s best. In 2012 the college ranked 16th among medical schools in primary care and 28th in research. Its physician assistant training program ranked second and its physical therapy program ranked fifth. Other top rankings included rural medicine (9); family medicine (16); and internal medicine (17).

The buildings housing the Carver College of Medicine are the centerpieces of a recently redesigned health sciences campus which includes a pedestrian-friendly commons and a functional and stimulating environment for students, faculty, and staff. Among the college facilities are three of the most forward-looking building projects in UI history—the Medical Education and Research Facility, Carver Biomedical Research Building and Pappajohn Biomedical Discovery Building.

students learn to become

accomplished clinicians and top-

flight researchers. Students who

come to Iowa to study medicine

join a community where case-

based learning is the basis

of their education.

With its emphasis on problem-solving skills, early exposure to patients, and enhanced community-based experiences, UI medical students typically have earned impressive scores on Step 1 of the U.S. Medical Licensing Examination and successfully “match” with top-quality residency programs.

Each of the medical and physician assistant students belongs to one of four “communities” in which he or she interacts with students in each year of the programs. This provides students with a more intimate, personal, and supportive learning environment.

Education and training from the college extends far beyond medical students and the University of Iowa campus.

It falls broadly into seven categories listed at right.

Education

Programs of Education at the Carver College of MedicineUndergraduate Medical EducationThere are 583 medical students in the Carver College of Medicine. Students may elect to pursue a distinction track in humanities, global health, teaching, service, or research. The Carver College of Medicine undergraduate medical program ranked 7th by the American Academy of Family Physicians for graduating the greatest percentage of students and in 2010 was ranked 10th in the nation by the American College of Physicians for accomplishing the social mission of academic medicine—graduating students who practice primary care, work in underserved areas, and represent the diversity of the population.

Graduate Medical EducationAt any given time, there are more than 700 doctors participating in graduate medical education at the University of Iowa. The impact of UI Health Care’s education mission on Iowa is significant, one-half of Iowa’s 5,000 doctors did some or all of their training at the University of Iowa.

Other Health Professional EducationIn addition to medical students, UIHC trains physician assistants and physical therapists and offers programs in dietetics, nuclear medicine technology, perfusion, radiation sciences, radiation therapy, radiologic technology, diagnostic medical sonography, and other health professions.

Undergraduate Science EducationAbout 5,000 undergraduate students from across the UI take courses in physiology and biophysics, biochemistry, anatomy and cell biology, and other basic sciences at the Carver College of Medicine.

Graduate Education in ScienceThe college’s research laboratories are the training grounds for more than 250 graduate students who pursue biomedical research under the mentorship of Carver College of Medicine faculty. The University of Iowa grants 60 doctorates each year to graduate students in the basic sciences. Routinely, over two-thirds of UI medical students graduate after participating in at least one research experience.

Clinical Training for Health ProfessionalsUniversity of Iowa Health Care provides professional education and training for a range of health care professionals, including nursing students from the UI College of Nursing and local community colleges, as well as UI Pharmacy students who gain clinical experience at University of Iowa Hospitals and Clinics.

Continuing Medical EducationUI Health Care practitioners present 200 CME programs annually to more than 5,000 physicians and an equal number of other professionals. Education takes place well beyond the campus in Iowa City. University of Iowa Health Care also participates in an education consortium in six Iowa cities: Des Moines, Waterloo, Mason City, Sioux City, Davenport, and Cedar Rapids.

Medical students fan out across the state to learn from local doctors who serve as the Carver College of Medicine’s 700 community-based faculty. In addition, Iowans who are interested in health care issues can participate in Mini-Medical School programs, community seminars, and obtain information about diseases, treatments, and other topics through programs such as the Cancer Information Service.

University of Iowa Hospitals and ClinicsThe first fully autonomous University Hospital opened in

1898 in the building now called Seashore Hall. The building’s

“H” shape, which kept patients segregated in separate wings,

was evidence that modern ideas about disease transmission

were beginning to inform medical science in Iowa.

In 1919 a Children’s Hospital opened on the west side of the Iowa River. This building was the first in a new west side medical complex, and its opening was marked by an official name change from “University Hospital” to “University Hospitals.”

A 60-bed Psychopathic Hospital was added in 1921 and, with matching funds from the Rockefeller Foundation, a new, modern General Hospital was opened in 1928.

The seven-story building had 750 beds, a 300-seat surgical amphitheater, and a 143-foot Gothic tower made of Indiana limestone. Thirteen hundred and 50 feet of tunnels connected the building with the adjacent Medical Laboratories and the Psychopathic and Children’s Hospitals, making the new hospital the center of the 80-acre medical complex.

In the 1970s, a long-range, phased program began to modernize patient care facilities in this state-owned institution without requiring state-appropriated capital funds. Today’s 692-bed facility provides patients with access to world class health care.

The University of IowaFounded in 1847 as Iowa’s first public institution of higher

education, the University of Iowa (UI) has won international

recognition for achievements in the sciences, humanities,

and the arts.

The UI was the first public university in the United States to admit women and men on an equal basis and the first institution of higher education to accept creative work in theater, writing, music, and art as theses for advanced degrees. It established the first law school and the first educational radio station west of the Mississippi, broadcast the world’s first educational television programs, and developed and continues to hold preeminence in educational testing.

The Carver College of Medicine is one of 11 colleges at the University of Iowa.

The university both provides and attracts a wide variety of cultural opportunities, Big Ten athletic events, and a number of business endeavors resulting from scientific and educational research that originated at Iowa.

The University of Iowa is a major national research university with a solid liberal arts and sciences foundation. Respected for its excellence in education, passion for discovery, and tradition of public service, the university offers a close-knit intellectual community that draws students and scholars from around the world.

StrengthsThe UI has world renowned research programs in genetics, hydraulics, and speech and hearing, and has recorded major innovations in agricultural medicine, biocatalysis, biomedical engineering, biomedical sciences, and pharmacology education. Its graduate programs in audiology, printmaking, creative writing, speech-language pathology, and nursing service administration are first-ranked. Iowa scientists,

including James Van Allen, have been pioneers in space research, designing and building research instruments for more than 50 successful U.S. satellites and space probes. The University of Iowa operates one of the nation’s most advanced and comprehensive university-owned teaching hospitals. It also has developed the most technically advanced driving simulator in the world.

Some NumbersMore than 30,500 students enroll at the UI each year. Some 55 percent come from Iowa, 25 percent from adjoining states, and 9 percent from the remaining states. International students from 100 countries make up 10 percent of the university’s enrollment. The faculty numbers about 1,700 and there are about 13,000 staff. The total annual operating budget is about $2.8 billion, and there are more than 120 major buildings, most of them within walking distance of one another.

Adding to the population are more than a million visitors each year who come to enjoy cultural events and art exhibits, to attend Big Ten athletic events, and to participate in the many conferences and educational programs scheduled at the university year-round.

Faculty include three Pulitzer Prize winners; five former U.S. Supreme Court clerks; two National Medal of Science recipients; 11 members of the American Academy of Arts and Sciences; 16 Institute of Medicine members; four members of the National Academy of Sciences; four Howard Hughes Medical Institute investigators; and members of other prestigious scholarly academies.

Academic HighlightsWith more than four million volumes in its Main Library and nine specialty libraries, the UI collection ranks 18th among public research universities. Its 200,000 rare books and 500 historical manuscript collections include works by Walt Whitman, the Iowa Women’s Archives, and the Szathmary culinary arts collection.

The pioneering Iowa Writers’ Workshop is one of the most respected and competitive programs of its kind. The Workshop, the International Writing Program, and other initiatives make the UI a premier center for creative work in fiction, nonfiction, poetry, and drama.

Major interdisciplinary research centers at the University of Iowa include the Holden Comprehensive Cancer Center, the Carver Family Center for Macular Degeneration, the Cardiovascular Research Center, the Center for Bioinformatics and Computational Biology, and others.

The UI Alumni Association connects the university with its 227,000 alumni around the world.

After it was engulfed by flood waters in June 2008, the internationally acclaimed Hancher Auditorium, the university’s performing arts center, was forced to evacuate. Until it is rebuilt in a new location, Hancher is continuing to offer a range of works in venues throughout the area.

Also impacted by the flood, the Museum of Art’s 12,000 piece permanent collection is in storage until a new home is built. Its collection is ranked in the top 15 of public universities, and its Stanley Collection of African Art has been called one of the finest in the world.

The Department of Intercollegiate Athletics oversees 12 women’s and 10 men’s varsity squads and facilities including Kinnick Stadium and Carver-Hawkeye Arena.

Iowa City CommunityThere’s a reason people relocate to the Iowa City area.

Picture a top-notch educational institution offering some of the nation’s best (and largest) health care facilities, a variety of art and theatrical attractions, an internationally renowned writers’ workshop, and research programs that are changing our world. Combine this with a welcoming community surrounded by the picturesque forests, lakes, and rivers of Eastern Iowa. It’s not hard to see why the Iowa City area is often ranked as one of the most livable communities in the nation.

In summertime, Iowa City sponsors weekly downtown jazz and pop concerts, and year-round major poets, writers, artists, historians, scientists, and others speak or perform at university or local venues.

Excellent public schools, safe and comfortable neighborhoods, and a highly educated population mean that Iowa City frequently appears high on “best-place-to-live” listings in national magazines.

The nearby countryside, scenic state parks, and the Iowa River provide many opportunities for walking, biking, and boating. Twenty miles to the north is Cedar Rapids, Iowa’s second-largest city, home to the Eastern Iowa Airport and a population of about 120,000. And Chicago, St. Louis, and Minneapolis/St. Paul are within easy driving distance.
