Search for a Patient Click Main Menu > Search for a patient > Patient Search

Enter any of the following:

− CHI number

− Surname

− DOB

− Postcode

− Diabetes Type then select “Search All Patients available to me”

On the search results page select the patient by clicking on their name. You will then be taken to their patient summary page. Go to the “Forms” drop down menu at the top and click the option “Foot Screening Tool”:

You will then be taken to the data entry form for foot screening. This pre-populates certain information based on previous screening results and today’s date. Click on “Keep Pre-populated Form” to proceed using the pre-populated data.

This will then allow you to record the new foot screening results more quickly by only altering those fields where the information is different from the pre-populated values. To record any new data, simply click on the relevant field and enter the new data. The screenshot below shows the entire foot screening tool form at a glance:

Once you have finished recording the foot screening data save the form by clicking on “Save” at the top of the screen, next to the “Forms” drop down menu.

Identifying Patient Lists by Foot Risk Category Click Main Menu > Population Overview > Foot Screening

On the next screen click on the column header “Foot Risk Calculation” to sort all the patients on the list by their current foot risk category.

By default the system only displays 20 records per page. You can select to display up to 500 records on one page. To print the patient list, or copy and paste it into a Word document or Excel spreadsheet, it will be best to display the maximum 500 records per page. Scoll to the bottom of the page and select “500” from the dropdown menu on the bottom right of the screen.

To print the list click on the print icon within the SCI-Diabetes menu bar:

This will open up a pop-up window with a printer-friendly version of your patient list. You can then print this by clicking on Internet Explorer’s print icon, or selecting “print” from Internet Explorer’s menu (this may vary slightly depending on which version of Internet Explorer you have installed on your computer).
