Rectification arrangements have been made immediately after the inspection was under taken.

Issues Inspected Rectification Arrangements

Evidential leakage in proximity of distribution board

Debris netting in general to be repaired

After a thorough observation and scrutiny by safety practitioner and site supervisor, they have all agreed that the seemingly chemical leakage was not factual but rather the alteration of coloration of the particular area of soil from its proximity is because the soil was found to be the excavated soil from the pit.

Meshwork was replaced and adhered to best serve its purpose and that is to block any debris from falling down on people working on the excavated area. Supervisor was tasked to check the condition of the meshwork on daily basis so as prevent any accidents directly associated to the hazard.

Saw to be PAT tested. Machine lacking guard or riving knife

Finger gaps in electric fans, particular front of fan which is hand span

Wood saw was isolated and safely kept in order not to be mistaken as functional. All men were instructed that this machine is no longer to be used and to be disposed of from the site as soon as the chance is under way.

Distribution board without door panel and switch cover missing

Possible oil spill without spill kit in place

The distribution board was covered and electrical hazard signage was placed on its cover as warning sign. Indications of signages were discussed during the first toolbox talk

Shock absorbing lanyards with double harness are used on site. Damaged and improper safety harness are not used until fixed.

The spilled liquid or material was not of oil characteristics. Spill kit has already been issued for an MR and is just waiting to be delivered on site soon. As for temporary measure, bucket with sand are used as substitute for the kit until the ordered one has arrived.

Scaffold safety tag broken and not inspected for longer Scaffold tags are being inspected on weekly basis. that 7 days The latest was last 10th of June 2014 done by a

KELE certified scaffolding supervisor

All the rest of the issues considered of minor significance enclosed in the observation ref number (HSSE Site Observations Report no.01) herewith detailed and mentioned were rectified and were made sure to be not recurring. Proper Housekeeping and stacking of materials have been fully taken into account and shall not be in any way taken lightly as the consequences may lead to a number of uncontrolled near misses and dangerous occurrences. Electrical hazards such as cables lying on the ground unarmored are recalled from site to the yard. Cable stands were built to keep the wires overhead in order to prevent slip, trip and fall incidents and from getting damaged which will then cause conceivable electrocution. Individual safety induction has been done to all the staffs

Safety statistics board has been populated as per the date of project official commissioning

who has moved in. Mass induction is programmed to commence next week Tuesday June 24, 2014.
