Health Disparities Health disparities refers to differences between groups of people. These differences can affects how frequently a disease affects a group, how many people get sick or how often the disease cause death. Many different populations are affected by disparities (Medline, 2013) . 5. What is Diabetes Diabetes is a chronic disease that occurs either when the pancreas does not produce enough insulin or when the body cannot effectively use the insulin it produces. Insulin is a hormone that regulates blood sugar (World Health Organization , 2013). Type 1 sometimes called Juvenile Type 2 sometimes called Adult Onset 6. Disparities In Ethical and Racial groups The most common reported health disparities are seen in cardiovascular disease, cancer and diabetes. Other illness include CVA (cardiovascular accident), unintentional injuries and HIV/AIDS. According to the former surgeon general , David Satcher, (2000), mortality rates in heart disease are more than 40% higher in African Americans in comparison to whites. Hispanics also experience higher mortality rates in diabetes and heart disease and are twice as likely to die from the disease than non Hispanic whites. 13. Statistics According to the 2000 census data, more than 25% of the U.S. population is composed of ethnic minority groups: 12.1% African Americans, 9% Hispanic, 2.9% Asian, and 1.0% Native Americans. If current birth and immigration trends continue, it is expected that the Hispanic population will increase by 21%, Asian 22%, African American 12% and the white population 2% by the year 2040 (US Bureau of the Census, 2000). These demographics changes combined with the fact that many minorities are overly burden with disease suggest that health care systems in the future will experience a much ore diverse clientele and sicker population. 14. What Is Heart Disease? Heart disease is a structural or functional abnormality of the heart, or of the blood vessels supplying the heart , that impairs its normal functioning. 15. Informatics is the science and art of turning data into information. Health disparities are the difference in health outcome, its determinant between segments of the populations. Communication between health care leaders, physician, nurses, and patient are facilitated through informatics. Patients are fully involved in the management of their own health needs . Hence, eliminating health disparities. Informatics nurses are key contributors to a working knowledge about low evidence-based practices designed in information system supported and enhanced clinical processes and decision making to improve patient safety and outcome. 20. Clinicians used informatics to reduce health disparities. With informatics, patients data is stored in Medical Record Format (MRF). Patients can access their own information easily A patient has direct access into their own personal information and can keep track of their follow up appointment. Thus reducing health disparity gaps through health maintenance. Health care professionals have access to online resources. Evidence base information research can be easily retrieved for patient education. When patients are educated on diseases processes health disparities will be reduced Through health informatics, information across culture data is available in delivery of healthcare improvements. 21. Effectiveness of Health Informatics on Disparities Through health informatics, information across culture data is available in delivery of healthcare improvements. Health informatics helps to reduce disparity gaps through innovative research in understanding behavioral and social factors that trigger health disparities. Informatics approaches helps researcher to understand information needed to alleviate health disparities. In terms of conducting public health research, biomedical research and clinical research. The use of community based modules, information based design in research provides information and knowledge in understanding health disparities. And therefore emphasis the fact that health informatics Is valuable in reducing disparity gaps among segments of population. 22. Factors associated with healthcare disparities. Health disparities, inequalities are important indicators for community health Informatics provides information for decision making, intervention, and implementation used to reduced preventable, morbidity and mortality rate. Attributed by social, demographics, environmental, and geographic factors. Income, education, race and ethnicity are major factors that increase disparities gaps. Healthcare Informatics promotes electronic health care records used to improve healthcare services. Informatics data can be easily accessed, interpreted by healthcare staff. 23. CONCLUSION The future of integrated efficient healthcare services which offers optimal patient centered care, depends on proactive information sharing The relationship between patients, clinicians, delivery accurate information is essential in decreasing health disparities. With increasingly complex patient health needs, the number of healthcare professionals that any patient might see is likely to increase. Therefore, optimizes the gaps in health disparities. 24.
