CO-CHAIR REPORT — DR. KEN BURNS

When our Division originally formed, we focused on bringing our GP community together. In years past, we had the hospital as a meeting place but that no longer occurs. Our Division’s events are meant to foster new friendships as well as renew old ones between colleagues. I believe this focus is still critical. Without a sense of community and commitment we will lack the ability to support each other and our community in a caring, compassionate and competent manner.

As the common motto states, “United we stand, Divided we fall”

When greed dominates gratitude and selfishness eliminates selflessness, we begin fractionating and decaying our sense of community.

Your Board has demonstrated their dedication to our medical community through their willingness to give of their professional and personal time. They have addressed many issues important to our members and our Ridge Meadows community.

Your Division’s administrative team has provided the backbone that has allowed us to move forward with all our activities

Finally your treasurer, Dr. Virin Bhatti, your vice chair, Dr. Ward Tinney and especially my Co-Chair, Dr. Ursula Luitingh have been outstanding. They have tirelessly dealt with every issue and concern arising from our Division’s initiatives.

I have been unbelievably lucky and I know it.

I have been able to witness the creation and maturation of your Division of Family Practice. I have felt great pride in your support of our Division and that support has been the envy of many Divisions around the province. I pray that this support will continue to grow as the Division faces new challenges. Your Division’s focus on supporting our members’ ability to provide excellent primary care to our Ridge Meadows community should never cease.

As the Co-Chair over the last year, I have experienced sadness and satisfaction.

There has been sadness with the death of Dr. Clarence Fernandes and Dr. Marco Terwiel. They were both amazingly dedicated colleagues and leaders in our community.

There has been satisfaction with the approval of our “A GP for Me” project that has involved a huge amount of work over the last 16 months. We now have the resources to provide support to our members to enable them to provide excellent primary care to many more patients in the Ridge Meadows area.

CO-CHAIR REPORT — DR. URSULA LUITINGH

We are approaching a new year and with it comes new challenges and new outcomes. As your chair, I hope to get a better understanding of how we can connect to you, our Division.

We have worked hard to engage our GPs at events like the AGM, stakeholder meetings and the Golf Tournament. But I would like to hear each member’s take on our Division and have a conversation where we have the opportunity to communicate and connect those thoughts.

Communication is an integral part of relationships and we have also worked extremely hard on building that with our community. I would like to raise our profile and awareness in our community by using the local media to celebrate our successes and live our vision of being an influential organization in the community; an important stakeholder.

Divisions have evolved over the last 4–5 years and things are changing all the time. We have excellent staff who help manage that change and really do a lot of the work while we do what we know best — take care of our patients.

Our Executive Director Treena Innes, has done a great job in supporting each of us. She carries the responsibility to make our dreams come true, so to speak. I am privileged to have her support while I lead the Board.

Our admin team: Leanne Regan, Ashley McDonald and Tracy Miyashita are equally invaluable and have worked very hard to support our initiatives and manage the small and big things at the office. Thank you all.

I would also like to thank Dr. Laura Kelly for her contributions to the board over the last three

years. She brought her open-mindedness and honesty into our board room and it has been great working with her. She will continue to look after our physician wellness and contribute to our monthly e-news with tips and advice for us.

Dr. Wilhelm Warmenhoven has also been such a strong leader and his presence on the Board will be missed.

And all of this would not have been possible if not for our fearless leader, Dr. Ken Burns. He has been instrumental in getting the Division going, along with many others who he convinced to join. He has a passion for family medicine and has interest in all areas of concern. These are big shoes to fill, and rather than trying to fit into them, I am hoping to pick a new pair to walk in. He leaves behind an important contribution to our Division and hopefully, we will build this foundational work to the next level and continue to be looked at as a Division that leads its peers and supports the family physician membership.

DIRECTOR REPORT — TREENA INNES

This past year’s memorable highlights include:

• Record attendance at our member and AGM events

• Our first “Meet the Community” event where we included SP, GPs and MOAs to meet Ridge Meadows community organizations that can help your patients

• The launch of Pathways, an online SP referral database tool for our GPs and MOAs

• A medical community resource guide for our community

• Planning and Assessing our community’s needs through the A GP For Me initiative, which led to the approval of a $650,000 implementation proposal to improve attachment

• MOA network development

• The start of the Shared Care Child and Youth Mental Health Initiative

We now unfold the next chapter of the Division as we transition from our hard-working, incredibly dedicated Chair, Dr. Ken Burns, and hand over the leadership reins to Dr. Ursula Luitingh. The foundation and leadership that

Ken brought to the Division has been instrumental in the Division’s success. Since day one, Ken’s determination to have our GPs join the Division movement has created a legacy and the necessary foundation that enables the Division to bring in the next generation of leadership.

The members and Division staff bring faith and belief to Ursula as our next Division leader. We know she will create another important legacy for the Division with her unwavering passion and vision for our GPs and patients.

Over the next few years, our Division will be working towards building strong governance with our Board of Directors, which in turn will create strong sustainability and growth of the work we are all doing to create positive change for our medical community.

A very deep heartfelt thank you to our Division staff: Leanne Regan, Ashley McDonald and Tracy Miyashita. The work ethic and drive that this staff team brings to the Division, in honour of supporting our GP members to meet our vision, is insurmountable.

Each year as we take the time to reflect on writing this Annual Report, I am so very proud of our Division and the amount of positive change we are bringing to our medical community from year to year.

You, our members, continue to tell us how happy you are that the Division has re-created the “hospital lounge” as a means to stay connected with each other. From new members to our community I hear time and time again how they value our collegial and embracing medical community culture.

I am reminded at every board meeting, advisory committee and member engagement event how inspiring it is to see you all sharing your time so generously and providing your wisdom and direction on our initiatives.

VISION STATEMENT:

Ridge Meadows Division of Family Practice is an influential organization that provides leadership to achieve optimal health for our community and provide support to our primary care physicians.

OUR MISSION:

Acknowledge, support, protect and enhance the role of primary care physicians by having an organization that has meaningful communication and partnership with Health Authorities, Ministry of Health and community at large in order to provide improved patient care and physician satisfaction.

OUR ORGANIZATION:

The Ridge Meadows Division of Family Practice (RMDFP) is a nonprofit society that has been incorporated since 2010 serving the Maple Ridge and Pitt Meadows area. RMDFP provides approximately 87 members with a strong voice to ensure that our members are at the forefront of positive change in our health care system for patients.

A GP FOR ME

GOALS/DESCRIPTION:

Attach unattached patients in Ridge Meadows.

YEAR HIGHLIGHTS:

After submitting a proposal for the request of funding for assessment and planning for our community, our Division was approved for $325,000 in funding to begin this work. We contracted Siena Consulting to lead this project with our hard-working A GP for Me Steering Committee and Division Director.

Through collaboration with the Fraser Health Authority, patient groups, community service organizations, First Nations, specialist physicians and allied health care professionals, the Ridge Meadows Division of Family Practice has been able to complete the Assessment and Planning phase, and we presented our Implementation Plan to the GPSC in July.

We discovered our community has an unattach-ment rate of 17%, approximately 16,000 people. 10,000 of the 16,000 unattached are currently looking for a GP and cannot find one.

Predictions estimate the population of Ridge Meadows growing by 19% in the next 10 years.

In that same time, 36% of our Division’s current 65 GPs will be retired or semi-retired.

Also concerning is the fact that GPs are spending anywhere from 1-3 days per week on non-clinical work that could be performed by a

skilled MOA and on clinical tasks that could be performed by an RN or LPN.

SUCCESSES:

We proposed three main initiatives that will:

1. Make a difference for our priority populations—including new moms, homeless and mentally ill;

2. Build capacity to provide team-based care involving an RN and/or a Super MOA and

3. Expand physician supply by recruiting and retaining two new GPs by the end of March 2016, attaching 4,880 patients.

Our plan focuses on three strategies:

1. We will implement a referral and attachment process (initially) targeting mothers and their babies and then expanding it to the wider community. We plan to attach 640 new patients by March 2016. We will also augment our current NP clinic to support the attachment of 40 homeless and/or mentally ill patients.

2. We will launch a multi-disciplinary team model within GP practices that will attach an additional 1000 new patients by March 2016

3. And, by focusing on recruitment and retention of two new GPs, we will strengthen locum coverage, avert a crisis situation and attach 1600 new patients and re-attach another 1600-2000 patients from a retiring practice.

Our community has been approved for $650,000 in funding until March 2016 to achieve these goals.

COMMUNITY SERVICES PARTNERSHIP

YEAR HIGHLIGHTS:

Maple Ridge Community Services provides numerous services to our patients, from children to seniors, at no cost or very low cost. We have developed a simple referral and feedback process, just one phone call or fax for all services.

We have been trying to promote these services to our Division members for the benefit of our patients. I must say we have not been very successful in generating referrals from family physicians to Maple Ridge Community Services. Now we are trying to reach the patients directly so they can request a referral from their family physician. A CD promoting these services is now available for family physicians to display on their waiting room televisions.

Many thanks to Treena Innes, Dr. Ward Tinney, and Vicki Kipps for their time and effort in improving the effectiveness of this program.

SUCCESSES:

In the New Year the Division hosted a “Meet the Community” event where over 100 guests participated. GPs, SPs and MOAs were able to engage in meeting community organizations and learn how their services can help patients.

HEALTH AND WELLNESS

GOALS/DESCRIPTION:

1. Provide education and venues for connecting with colleagues.

YEAR HIGHLIGHTS:

The physician wellness program continues to be active with the Division board, and presented the workshop on “Happiness” at the AGM last September. In October, Dr. Kelly attended the Canadian Conference on Physician Health in Calgary in October and returned with some ideas and inspiration, including increasing education and reminders for doctors on keeping themselves well. Research articles on nutrition and reminders to snack during the workday were distributed with healthy granola bars to each doctor’s hospital mailbox.

SUCCESSES:

In December, Dr. Kelly, along with Dr. Ruffo and Dr. Froese from the White Rock Division of Family Practice, submitted an abstract to the International Conference on Physician Health to be held in London, UK this fall and was accepted for presentation. This abstract outlined our local community’s “Physician’s Advocating Wellness” (PAW) experience over the past seven years, and reviewed both the inspiration and pitfalls of starting such a community-based program.

LOOKING FORWARD:

Unfortunately, the Fraser Health Authority decided in March that our $10,000 per annum funding for programming would not be continued. The Ridge Meadows Division of Family Practice board renewed its commitment to sustaining a physician wellness focus in our community, recognizing the importance of a healthy physician community. Since that time the board has supported the walk/cycle event, the annual golf tournament and ongoing education pieces in the monthly e-newsletter. We welcome ideas and assistance in developing new strategies for supporting our local doctors, and maintaining our collegial physician community in Maple Ridge and Pitt Meadows.

HEART FUNCTION CLINIC

GOALS/DESCRIPTION:

1. Give patients rapid access to Heart Clinic.

YEAR HIGHLIGHTS:

The Heart Function Clinic is now in its third year of operation, averaging about 80 patients a year being served. Connection through the electronic medical record continues to be a challenge, and patients rarely bring their “folder” of updated information from clinic to the Emergency Department or their doctors’ offices.

SUCCESSES:

The RM Division was involved in advising at the outset of the clinic, and has reported back to the clinic staff about ongoing operations.

LOOKING FORWARD:

Last autumn, the Division Board of Directors made the decision to close its ongoing involvement with the Heart Function Clinic, but would be pleased to advise and communicate to our members on a when-needed basis going forward.

YEAR HIGHLIGHTS:

After a few hurdles last year to overcome, the Home Health Initiative is back running smoothly. The problems were caused by staffing shortages in terms of the number of case managers with lack of communication. There is now a full complement of case managers who have been contacting all our members to hopefully set up a communication schedule that works for everyone. The Division members are encouraged to speak with their case managers and work out a system that works for them. Remember that the contact doesn’t need to be face to face. Phone contact is acceptable both in terms of patient care and satisfying billing requirements. If anyone has forgotten the billing codes, the Division can help.

SUCCESSES:

The first few months after the implementation of this initiative provided quite a few success stories that we were able to share not only with each other but with Board Members from other Divisions. Hopefully, now that we are up and running again, we will have more successes to share.

INTEGRATED HEALTH NETWORKS/PSYCHIATRY

GOALS/DESCRIPTION:

Improving the journey of complex psychiatric patients back into the community and care of their family physician from Mental Health Unit

YEAR HIGHLIGHTS:

This project tested and refined a standardized process for discharging/transitioning stable clients from community mental health centres to family practitioners.

This process is intended to support effective communication, collaboration and client care during this transition period.

SUCCESSES:

The successes completed were to ensure completion of the pilot, membership communication/feedback and to simplify a flow chart process for the return of patients from mental health back to the GPs.

YEAR HIGHLIGHTS:

Ridge Meadows Maternity Clinic was created in 2002 to meet the growing demand for primary obstetric care in the Ridge Meadows area due to increasing numbers of deliveries at RMH and decreasing number of physicians providing labour and delivery care.

Supporting our maternity clinic as an essential part of health care services in the Ridge Meadows area has always been a focus of our Division. We feel confident that we are addressing this goal, as the maternity patient volume has continued to increase over 12 years. The total number of babies delivered at Ridge Meadows Hospital in 2001 (the year before RMMC opened) was 542, whereas the total number of deliveries for 2013 was 792. We thank all of the referring physicians in Ridge Meadows and surrounding communities for their support.

SUCCESSES:

As part of our A GP for Me work, since August 2013 we have been working on developing a matching tool that will facilitate the attachment of “Mother-Child Dyads” with a local GP in our community. We have created relationships with Ridge Meadows GPs, which allowed us to start attaching patients in January 2014. From January to July 2014, we successfully attached approximately 125 patients to a local family physician. Our goal is for every one of our patients, and their families, to be attached to a GP before they are discharged from our care at postpartum visit.

We have been able to hire a manager who is responsible for this project.

LOOKING FORWARD:

We are composed of eight local family physicians with a passion for providing compassionate, competent primary obstetric care: Dr. Len Hatlelid, Dr. Cilliers Marais, Dr. Paul Sidhu, Dr. Simone Loch, Dr. Daniel Wong, Dr. Ken Burns, Dr. Marla Connor and Dr. Virinder Bhatti.

We are currently looking for a ninth physician to join our primary maternity care team.

We look forward to working with interested members of our Division to advance this work.

GOALS/DESCRIPTION:

YEAR HIGHLIGHTS: 

This year, the MOA Network really took shape. We currently have over 70 SP and GP MOAs in our network who are actively communicating with us about what they would like to see come to Ridge Meadows in order to benefit their job performance and improve patient care. We have been able to increase our communication with MOAs throughout Ridge Meadows through a variety of avenues, as well as offer them several workshops based on feedback we have received. We are also actively investigating further opportunities for everyone to come together and create a strong MOA community.

With the implementation of Pathways, we were given a great opportunity to meet directly with many MOAs and show them that we are here to support them. The Division is going to be offering an increasing number of opportunities for participation in many Division events for our MOAs, and with that, our hope is that this network will continue to grow and give our MOAs the chance to improve their relationships with each other, our membership and the Division.

SUCCESSES: 

We held two very successful Workshops this year, including a two-day first aid re-certification workshop and a second tour of the Radiology Department at Ridge Meadows Hospital. We were able to highlight how our MOAs can better communicate with Radiology in order to improve wait times by providing them the chance to speak with the technicians as well as the main unit clerk. We also added several MOAs from the specialists’ offices in our community who are able to offer a greater understanding when it comes to referrals and booking processes within those offices.

LOOKING FORWARD:

We have booked a workshop with the Public Health Unit regarding vaccinations scheduled to occur in early fall, as well as a presentation from Alouette Addictions in early 2015. We are in the planning stages of a conflict resolution seminar slotted for this Spring, and have contacted MOA Association of BC to see what kind of resources are available to our MOAs on a provincial level. We will also be rolling out an improved communication plan with our MOA Network this September, in order to create a better method of connecting our MOAs to each other and the Division.

NURSE PRACTITIONER 1 — HOMELESS/MENTALLY ILL

YEAR HIGHLIGHTS:

I continue to be very proud of the work that the Board has done to provide primary care to the disadvantaged people who use the services of The Caring Place. Janet Rosenfeld has been successful in engaging this very difficult to treat population. I would like to offer a special thank you to Dr. Liz Zubek and Dr. Paul Beckett for their help with mentoring Janet. I would also like to thank The Caring Place for their generosity in providing the space and administrative help required for the day to day functioning of the clinic.

SUCCESSES:

Janet has successfully reintroduced primary care to many people who have fallen out of touch with this very important first step to good health. There is still a lot of work to do and we need to remain positive. The parameters by which we measure success in this special group of patients are by necessity more variable. Janet has been able to help several patients back to a point where they are ready to return to a more traditional family practice.

LOOKING FORWARD:

Speaking of returning to more traditional family practices, Dr. Zubek has attached a patient from the clinic and Janet has several other patients that she would like to see attached to one of our members. Treatment of these patients can be very rewarding and if anyone is interested, please contact Dr. Al Neufeld or Janet.

All of us have patients who cause us a great deal of difficulty. Often these patients have mental health issues and are on the road to becoming homeless and doctorless. Our nurse practitioner has a special set of skills and interests which may be very helpful in dealing with these patients. Before you end your relationship with these patients, consider a referral to Janet who would love to work collaboratively with you.

This can be a win-win-win with patient, doctor and nurse practitioner all benefiting.

We also look forward to having this initiative become part of our A GP for Me work, so we can implement a strong attachment and referral tool with evaluation.

YEAR HIGHLIGHTS:

The Division adopted Pathways in early 2014 to benefit our members and their practice. Since the launch approx. 40% of physicians have been utilizing this resource.

Pathways has been a great way to download referral forms, obtain information about wait-times, discover focus areas of specialists, and access patient instructions.

SUCCESSES:

The Division successfully launched Pathways on May 12 and usage continues to grow. Ridge Meadows was the third division to adopt this resource and since the launch seven more divisions have come on board. Physicians in our community have access to over 200 written resources on various topics and can view the pages of hundreds of specialists located throughout the lower mainland covering 36 specialty areas. This resource will greatly improve referrals to specialists and help physicians enhance service to patients.

Next steps for Pathways in Ridge Meadows include the addition of community resources and a listing of family physicians. Dr. Cilliers Marais, Dr. Ward Tinney and Ashley McDonald have joined the Pathways provincial group to support the growth of Pathways across BC.

PITO / ELECTRONIC MEDICAL RECORDS / COMMUNITY OF PRACTICE

GOALS/DESCRIPTION:

The Ridge Meadows Community of Practice received $31,000 in January 2014 to focus on the following goals:

1. Develop electronic resources to improve SP/GP communication, referrals and support to patients.

2. Support optimization of EMR usage in clinics.

3. Increase EMR adoption rate of SPs by 10% (from 54% adoption).

YEAR HIGHLIGHTS:

The Community of Practice (CoP) received funding from January – March 2014 for local initiatives that would enhance EMR use. For the duration of the funding period, the CoP facilitated advisory planning sessions as well as user learning sessions on dermatology and pathways.

Highlights include:

• Implementation of Pathways which will enable sharing of electronic forms, resources and improve referrals.

• Development of electronic requisition forms, adapted to each EMR vendor.

• Increase in adoption rates by both SPs and GPs.

SUCCESSES:

• The Division was successful in submitting a funding proposal for $93,000 for PITO initiatives (received $31,000 of total funding prior to March 31).

• Ridge Meadows has a physician adoption rate of 83%, which exceeds the provincial benchmark of 82% and the Fraser Health region average of 77%.

• PITO funding supported the implementation of Pathways, which facilitates faster referrals, access to electronic physician and patient resources, as well as access to electronic referral forms.

• Development of an electronic requisition for RMH imaging that has been adapted to Plexia, Osler and Intrahealth users.

LOOKING FORWARD:

As PITO ceased to exist beyond March 2014, the EMR focus has transferred to the PSP Technology Group. The future efforts will shift to Post Implementation Support (PIS) for EMR users to help them obtain a functional capacity of “Meaningful Use – level 3”. More information will be available from PSP-TG and Doctors of BC.

YEAR HIGHLIGHTS:

Physician recruitment continues to be prioritized by the Division members. Continuous efforts are made to welcome and engage new GPs to our community with strong GP mentorship, spouse welcome lunches and connections to the community.

In honour of helping increase GP capacity in our community, Fraser Health recently awarded an IMG for our community starting in spring 2015 for two years. Recruitment of this IMG will begin in the fall.

SUCCESSES:

In the last 12 months, we welcomed four new locums and three new GPs to our community. We have also been listening to the provincial Divisions of Family Practice office as they begin to support a cross-divisional working group that is looking at recruitment, locum, and practice coverage efforts throughout BC and has developed some tools and resources to support all divisions’ recruitment and retention efforts.

LOOKING FORWARD:

We are very excited to improve the capacity on our Division’s physician recruitment work within the A GP for Me program. Additional Division Director and administration time will be dedicated to this important work, enabling us to be proactive in our efforts. SHARED CARE ACUTE CARE PROJECT

GOALS/DESCRIPTION:

YEAR HIGHLIGHTS:

The Ridge Meadows Division of Family Practice submitted a proposal to the Shared Care Committee (a joint Doctors of BC/Ministry of Health collaborative committee) in early 2014 to focus on improving communication between family physicians, hospitalists and ER physicians with respect to acute care admission and discharge. Early discussions to identify preliminary challenges took place in January 2014 and participants expressed interest in moving forward with a project.

SUCCESSES:

We are pleased to announce that we were successful in receiving $159,000 to begin a Transitions in Care project to focus on acute care communication. The project was officially launched in June 2014 and is expected to conclude in December 2015.

LOOKING FORWARD:

A stakeholder engagement meeting is planned for October 2014 to identify challenges and solutions.

Division members have identified this as a priority. In the new year, a provincial initiative was approved to roll out through the Shared Care Program. Our Division was quick to react and start to work with the Shared Care team to begin inquiry for our community.

SUCCESSES:

Unfortunately this initiative has been put on hold until Fraser Health works through the necessary partnership agreements, including Ministry of Children and Family Development. Our Division will remain engaged by attending an upcoming collaborative conference in the fall and continuing to follow up to show our Division readiness to commence once Fraser Health is ready.

SHARED CARE RADIOLOGY

GOALS/DESCRIPTION:

Improve journey of patients requiring imaging services through timely appropriate tests with two-way information transfer and patient involvement.

YEAR HIGHLIGHTS:

Key project activities included the development of an electronic requisition form, which is available on Pathways as well as EMR systems in each clinic. A total of 46 MOAs attended the MOA Tour and Talks held in the Imaging Department to gain a better understanding about the referral process and pre-tests that are required. A referral process card was also developed that includes approximate wait times, department numbers and referral process for urgent and non-urgent requests. Two physician education workshops were held with a total of 65 family physicians and specialists. Topics included managing patients with shoulder pain, red flags and imaging guidelines. Thank you to Dr. Andrew Mason, Dr. Stuart Cocquyt and Dr. Won Kim for facilitating these sessions.

SUCCESSES:

An evaluation of the project indicates that:

• 88% of physicians noted an improvement in overall communication between their office and the Imaging Department.

• 77% felt that there was an improvement in the time it takes to get results back.

• 47% strongly agreed and 53% agreed that they will make more informed referral decisions as a result of the physician learning sessions.

• 100% of MOAs in attendance indicated the tours enhanced their knowledge about the referral process and patient preparation for imaging tests.

LOOKING FORWARD:

The Imaging Department plans to continue to offer MOA Tours to maintain communication with GP offices and we will continue to advocate for a computerized booking system for imaging which would decrease the processing time for booking appointments. Thank you to all those that participated in this initiative. We look forward to continuing the great relationships that have been established with radiology staff.

NON PHARMA CME — DR. VIRIN BHATTIThis is a new Initiative which was requested by the membership. We completed our first CME event, entitled Opioids in Chronic Non-Cancer Pain, in June. The event provided a lot of nonbiased information on the nature and proper use of opioids.

Our next CME event is on October 8 and focuses on Oral Anticoagulants in Atrial Fibrillation.

We would like to improve the attendance on these events. Please provide your suggestions regarding what time of the day and which days of the week would make it easier for you to attend.

MESSAGE FROM TREASURER — DR. VIRIN BHATTI

The division had another successful year with a balanced budget. Audit was completed without any problems.

The Division received infrastructure funding of $249,000 to carry out the initiatives as prioritized by the members. Shared Care funding of $110,744 was carried over for the Radiology Project. A GP for Me funding of $325,000 was received for the assessment and planning.

Balancing the budget was challenging because the A GP for Me planning phase was extended and we had to come up with funding for three extra months of planning. As a result we were not able to contribute to the provisional data collaborative. The next year is going to be even more challenging with our work on the A GP for Me initiative becoming more complex and difficult to budget for.
