As part of a Health and Recovery Plan (HARP), Behavioral Health Home and Community Based Services (BH HCBS) give eligible adults with behavioral health challenges the support they need to live independently in their community.* BH HCBS can provide: REACH YOUR LIFE GOALS! Skill-building services to help you manage your money and improve your social skills. Daily living skills to help you regain life skills you once had but may have lost due to a behavioral health issue. In-home care when you are not able to go to your clinic or other mental health provider. Peer support services from others who are also living with mental health and/or substance use challenges. Peer specialists and peer advocates have “been there.” By sharing their own life experiences, they may be able to help you cope and advance your recovery. Assistance to help your family of choice develop the knowledge and skills to support your recovery. Educational support to help you complete formal educational programs and meet academic goals, such as entering a trade school, or helping with high school equivalency test prep. Employment support services to help you get or keep a job. Respite services, which offer safe and supportive residential environments during an emotional crisis. Service providers can help you find community resources, get care referrals and manage health issues. They can also offer peer support and provide other tools on how to avoid future emotional crises. Intensive crisis respite services when you need the specialized support of therapists, psychiatrists and other staff in a safe residential setting. ACCESSING SERVICES STEP 1 After you are enrolled in a HARP, you and your care manager discuss your goals. STEP 4 Your care manager helps you select a provider and make an appointment. STEP 2 Together, you create a care plan to help you meet your goals. STEP 5 You and your provider create a service plan that fits your goals. STEP 3 Your care manager contacts your insurance company for approval and to locate BH HCBS. STEP 6 Your care manager continues to work with you, your provider(s) and your insurance company. *You must be enrolled in a HARP and complete an assessment to find out if you qualify for BH HCBS. IS A HARP THE RIGHT CHOICE FOR ME?Talk to your mental health provider or substance use counselor about HARP. They can help you find out if you are already enrolled or how to enroll if you are eligible.

EligibilityYou must be 21 or older to join a HARP, be insured only by Medicaid, have a chronic mental illness or substance use issue, and be eligible for Medicaid managed care.
