RICHARD FAIRFAX Deputy Assistant Secretary of Labor Occupational Safety and Health Administration Richard Fairfax was named to his new position by Assistant Secretary David Michaels where he began his duties on April 5, 2010. As Deputy Assistant Secretary, Mr. Fairfax is responsible for supervision of the work of the Occupational Safety and Health Administration’s 10 Regional Offices across the country and the Directorates of Enforcement and Construction. Immediately prior to this position, Mr. Fairfax served as the Director of Enforcement Programs and Acting Director of Construction Programs for OSHA. A certified industrial hygienist, he has worked for OSHA for 32 years as a field industrial hygienist, regional industrial hygienist and as a senior industrial hygienist before becoming Director of Enforcement Programs.

Mr. Fairfax also writes a column and serves on the Editorial Review Board for the Journal of Occupational and Environmental Hygiene.

HOWARD A. MAVITY Fisher & Phillips Howard A. Mavity is a partner with Fisher & Phillips, one of the nation’s oldest and largest law firms devoted exclusively to representing management in labor and employment matters. The Firm has approximately 230 attorneys in 25 offices.

Mr. Mavity has been with Fisher & Phillips for almost 27 years. He is the Co-Chairman of the Firm’s Workplace Safety and Catastrophe Management Practice Group. Howard is a graduate of both the undergraduate and law schools of Vanderbilt University. Prior to Fisher & Phillips, Mr. Mavity grew up in Dalton, Georgia and developed a background in construction, transportation, and materials handling. He devotes the majority of his time to managing workplace fatalities and catastrophes, union organizing drives, and other high-visibility matters, including whistleblower claims and internal investigations. Howard advises employers in handling day to day personnel matters, dealing with government investigations, and engaging in acquisitions, restructuring, and strategic planning involving labor and personnel issues. Mr. Mavity has handled approximately 400 fatality

cases and countless OSHA inspections in 45 states. He maintains professional relationships with federal and state plan administrators, as well as with the D.C. OSHA and Solicitors’ Offices. He coordinates the abatement and compliance efforts of nationwide manufacturers in higher hazard industries, and works extensively with construction contractors in multi-employer settings.

Mr. Mavity devotes a substantial amount of his time to working proactively with employers to train managers and develop effective corporate wide policies and programs in order to avoid safety, union, and legal challenges. Howard has handled well over 225 occasions of union activity in 25 states, as well as representing employers with amicable union relationships. He oversees comprehensive HR, Safety and Labor audits of his clients. Mr. Mavity has been involved in rulemaking and comments. He is a member of the Associated General Contractors (AGC)-National Safety Committee, the Georgia AGC, the National Alliance for Model State Drug Laws, the ABA-OSHA Section, the NFPA, and various Chambers of Commerce. He was featured as one of the “Georgia Super Lawyers” in the 2004, 2005, and 2006 Atlanta Magazine survey, and the 2006 Georgia Trend Magazine, Georgia Legal Elite.

Mr. Mavity addresses numerous business and legal groups each year, including the American Bar Association – OSHA Section, the Georgia and Tennessee Chambers of Commerce, the Georgia and Alabama Hospital Associations, the Carl Vinson Institute on Government, the Associated General Contractors of Georgia, Alabama, Arkansas, Missouri, Kentucky, Ohio, and Carolinas, the Associated Builders and Contractors of Alabama, the Independent Electrical Contractors Association, the Alabama Trucking Association, the American Public Power Association, the American Foundry Society (AFS), the National Rural Electric Cooperative (NREC) Association, the American Public Transit Association, the Texas and Arkansas Transit Associations, the Tennessee Valley Public Power Association, the American Foundry Institute, the Georgia Bar, the Associated Wire Rope Fabricators, the Alabama, Georgia, Central Florida and Metro Atlanta Autodealers Associations, the Florida Fruit and Vegetable Growers Association, the National Association of Healthcare Recruiters, the Alabama Council of Association Executives, the CPA Clubs of Georgia, the American Public Relations Society, Georgia Concrete & Products Association, the U.S. Poultry & Egg Association, various SHRM groups, and the American Society of Safety Engineers (ASSE). Mr. Mavity is active in the Atlanta Area Boy Scouts, United Way, the Marist School and Christ Church Presbyterian Church. He has coached and participates in adult and children’s baseball, cross-country, and karate programs. He has mentored Georgia Tech students. He is married to Karla Mavity, a nursing administrator with Children’s Healthcare of Atlanta. They have a daughter, Megan and a son, William. Mr. Mavity is a runner and has competed in martial arts and full contact fighting.

SUBPART CC QUALIFIED RIGGING TRAINING .................................................................................................. 8:00 AM - 12:00 PM Jim Leach - American Riggers Supply, Inc. This course will cover the training requirements as set forth in the new OSHA Construction Crane Standard, Subpart CC, which became effective November 8, 2010. This course will cover the elements of a rigging plan to ensure safe work during hoisting activities. It will also cover all types of rigging and materials, including slings and chains, and types of hardware used, and how to properly inspect them for defects and when to remove from service, manufacturers’ recommendations for load ratings, and emphasize the importance for locating the center of gravity for all loads and calculating the load angle of the rigging from horizontal to ensure the proper rigging is used. The intent is to address the various aspects of rigging with an interactive approach. I will be asking questions and expecting the attendees to do the same. Hopefully we will all learn something.

This training course will help participants to know and understand those responsibilities and requirements, as OSHA requires them. This course will also train employees on the dangers of scaffolds and how to minimize risks by knowing the correct ways to erect, maintain and use scaffolding equipment, show all possible responsibilities of a "Scaffold Competent Person”, ensure all participants have the knowledge to create a level and stable foundation, discuss power line hazards and distance or protection requirements, ensure proper access with appropriate ramps and/or walkways, and ensure all participants understand platform & planking assembly and hazards.

DENNIS BURKS, CSP, PE HNTB Companies Dennis Burks is the Safety Director for The HNTB Companies. He started his career as a member of the Laborers Union Local 1290 and then as a member of Operating Engineers Local 101 in Kansas City. Dennis received his Master of Science degree in Industrial Safety and his first safety position was with the Greater Kansas City Building and Construction Trades Council. He teaches part time for the OSHA Training Institute Education Center. Dennis is a past local chapter President of the American Society of Safety Engineers, a Certified Safety Professional (CSP), a registered Professional Engineer (PE), and the recipient of the Regional Safety Professional of the Year award from the American Society of Safety Engineers, Region IV.

CHARLIE CARTWRIGHT Lockton Companies, LLC Charlie has more than twenty years of business and management experience, primarily spent in the transportation, staffing, health care and restaurant management fields. His responsibilities included budgeting, personnel and creating and maintaining safe and productive work environments. Included in the latter were designing programs and implementing effective ways of reducing workplace injuries and the resultant lost time and expenses from them. In 2001 Charlie obtained his Bachelor of Science and Chiropractic Degrees. He is also a Board Certified Chiropractic Physician.

MARK S. DAMON Process Marketing Group Mark Damon has been involved with fall protection for over thirty years. He has served as Vice President of Rose Manufacturing Company and is currently President of Process Marketing Group, manufacturers’ representatives of safety equipment. Mark has served on various OSHA and ANSI fall protection committees, and also helped to draft fall protection standards for the U.S. Navy.

AJ GAJDOSIK, CSP, ARM-P Nationwide Insurance AJ began his career in safety working for a large oil and gas company in Houston, Texas as a Safety, Health and Environmental Representative. From there he returned to St. Louis and began work for an agribusiness insurance company servicing accounts in eleven southeastern states. Following this position, he worked for a monoline workers’ compensation carrier serving as a technical specialist responsible for all training activities and development of consultant resources. He has also worked for a large regional multi-line insurance carrier where he provided high level training and safety services to key accounts. He earned a Bachelor of Science Degree in Safety Management from the University of Central Missouri. AJ has earned a certificate in Human Performance Improvement through the American Society for Training and Development. He has also earned the Certified Safety Professional (CSP) designation conferred by the Board of Certified Safety Professionals and the Associate in Risk Management (ARM) designation with Public Entity (-P) certification from the Insurance Institute of America.

AJ joined the adjunct faculty of the Midwest OSHA Education Centers (MOEC) teaching a variety of OSHA Training Institute (OTI) courses including the OSHA Authorized Outreach Trainer courses. DWAYNE HARTMAN, CSP, CRIS Lockton Companies, LLC Dwayne is a 1994 graduate of the University of Central Missouri, where he obtained a Bachelor of Science degree in Safety Management. JURCYK McAnany, VanCleave & Phillips John received a B.A. from Creighton University in 1981 and a J.D. from Washburn University School of Law in 1984 where he was President of the Honors Fraternity and the Moot Court Council. John is AV rated in Martindale Hubble. JIM LEACH American Rigger’s Supply, Inc. Mr. Leach holds a Bachelor’s degree from Kansas University. March of 2011 marked his 40th year with American Rigger's Supply. For the last 25 years Jim has been manager of sales and marketing where his approach has been to educate end users regarding proper rigging. When it comes to lifting and moving loads, “what you don't know can hurt you”. Jim’s goal is to help prevent that from happening. He is currently a Crosby “authorized trainer” and a NCCCO “certified rigger”.

DEREK LEFFERT Missouri One Call System Derek Leffert has been the field manager for Missouri One Call System since August of 2007. Prior to coming to Missouri One Call, Derek was a lobbyist in the utility industry. Derek serves as a spokesperson for MOCS and travels the state extensively providing training and forming partnerships with stakeholders in the utility and construction industries. He holds a Bachelor's Degree in Business Administration with Dual Majors in Management & Human Resource Management with a minor in Political Science. He resides in Jefferson City with his wife and two young children.

GLEN MAUGHMER Kansas City Power & Light Glen has been a Journeyman Lineman for 23 years. The last 2 years he has spent training apprentice lineman and conducting safety and training for journeyman lineman and the general public.

BRENT MAYHEW Lockton Companies, LLC. Brent Mayhew currently works for Lockton Companies, LLC as a Senior Loss Control Consultant, focusing on both construction and general industry accounts. Prior to joining Lockton three years ago, Mr. Mayhew was employed with a National Contractors Association, whereby he managed various contractors and projects. Mr. Mayhew is a current EMT in the state of Missouri and conducts numerous OSHA, MSHA and other specialized training courses.

BILL MCDONALD, CSP OSHA St. Louis Area Office, Area Director Bill was selected as the Area Director, St. Louis Area Office in November 2001. Bill has earned distinction as a Certified Safety Professional (CSP). Prior to his selection as Area Director, he served as Assistant Area Director for 3 years and as an OSHA Industrial Hygienist for 10 years. Bill also served as an Industrial Hygienist for the U.S. Air Force for 5 years prior to joining OSHA. Bill has a B.S. in Industrial Hygiene from Central Missouri State University. Bill has been honored with many awards during his career with the Federal Government including the OSHA Leadership Award for 2005, and the OSHA Region VII Leadership award in 2005, 2006, and 2007. RAMONA MCENANEY, AIC, CRIS, LPCS Lockton Companies, LLC Ramona McEnaney is a Senior Claims Consultant with Lockton Companies, LLC for the past six years. In her current position she provides claims management evaluation, procedures and recommendations to help mitigate the financial impact of insurance programs. He is currently the safety director for Lytle Construction Inc. in Lee's Summit, MO. He has worked for Winn-Senter Construction, J.E. JIM A. PICKEL Cates Sheet Metal Industries, Inc. Jim has been in the Environmental and Safety field for over 29 years. He set up the first asbestos laboratory at the University of Kansas and designed the university’s Asbestos Abatement Training Trailers. He is certified in Chemical Microscopy for Polarized Light, Phase Contrast and Transmission Electron Microscopy (fiber identification and counting). He was the Laboratory Director for a local asbestos and environmental laboratory for 14 years and was a certified trainer in asbestos and lead abatement, for project supervisors, workers, inspectors, project managers and in project design. Jim has written several EPA and State approved training manuals in these subjects. He has been the Safety Director at Cates Sheet Metal Industries, Inc. for the last 6 years.

Mr. Reuter has worked in the construction industry for more than 30 years and has been employed with Travelers for over 25 years. Before he began his career with Travelers, his first few years were spent as a laborer, carpenter, welder and equipment operator building slip form grain elevators and working for a heavy contractor. After many years of providing risk control services to clients and managing other representatives, he became the Technical Services Director in the Travelers Risk Control Unit. This unit creates, updates, manages and provides onsite training for Travelers Construction clients.

ROBERT SIMMONS, MS The Builders’ Association/AGC Robert started with The Builders’ Association in 2007. Prior to joining the safety and health services team, he was Assistant Director of the Division of Labor Standards and Program Manager for the State of Missouri’s Onsite Safety and Health Consultation Service. He has over 17 years’ experience in the safety, health, and environmental field and is responsible for serving the association’s members in Columbia, Jefferson City, and Springfield. Robert holds a Bachelor and a Master of Science in Industrial Hygiene from University of Central Missouri.
