Heart Failure
• LVD only
• Currently treated with ACEi or ARB

CKD
• Annual protein: creatinine ratio
Contraception
• CON 1 & 2 removed
• Register of women prescribed contraceptives
• % on oral or patch - given advice re Long acting reversible methods
• % who have takenemergency contraception - given advice re Long acting reversible methods
Anxiety & Depression
• Re-use of PHQ9 at 5 –12 weeks
CVD – primary pevention
• New diagnosis of hypertension alone – Cardiovascular risk assessment within 3/12
• Lifestyle advice given
Diabetes
Thresholds change for HbA1c
7 or less (40-50%) - 42%
8 or less (40-70%) - 69%
9 or less (40-90%) - 80%
COPD
• Inhaler technique drops out
• MRC dyspnoea score instead
Palliative Care
• Now irrespective of age
Meetings restructured recently
Stroke/TIA
• Now irrespective of age
