Proposal Preparation/Instructions Clarifications/Questions:

1. The Small Business Participation Plan and Small Business Subcontracting Plans (Exhibits D and

E) require cost data. Should these be moved from Volume 2 to Volume 1 of the Proposal

Submittal.

a. Exhibits D and E, Small Business Participation Plan and Small Business Subcontracting

Plans should be included in Volume 1 of the proposal submittal. DO Not include any

Cost/Price information in Volume2.

2. The SF 1442 in Block 8 indicates to address the offer to a different contact than that provided in

the 00110 proposal Preparation Instructions, paragraph 2.3. Please confirm the address

provided in 2.3 is correct.

a. The address in Block 8 of the SF 1442 is the issued by address. The address and contact

name in Block 2.3 of the Proposal Content section is the correct information for

submittals.

3. We assume that the original bid bond shall be placed in the Price Volume. Please confirm.

a. Yes – please include the Original Bid Bond in Volume 1, Price Proposal.

4. It appears in -0110 Proposal Preparation Instruction a CD is required for the Technical Volume.

However, it is not stated a CD is required for the Price Volume. Please confirm no CDs shall be

provided for the Price Volume.

a. This is correct. A Hard Copy of the Price Volume is required. No CD. Original signature

is required is required on the SF1442.

5. 00110 Proposal Preparation Instructions indicates tab indexing will be used to identify sections

as appropriate. Bidder assumes this is stating that items in the Table of Contents should be

referred to as Tab A, B, C, etc. as appropriate. Please confirm our understanding is correct.

a. Tabs can be in contractors format

6. Please clarify intent of 00110 Technical Capability proposal. We understand that the

government is seeking relevant project descriptions (which may also be found in the

Past/Present Performance information volume) in this section as well as resume to satisfy this

requirement. Please confirm our understanding is correct.

a. See 2.5.2 for clarification.

7. Please confirm the use of tabs and subtabs is acceptable to facilitate organization.

a. Yes, tabs and subtabs in both volumes are acceptable.

8. Section 00110, Proposal Preparation Instructions, paragraph 2.4.4 states all pages of each

proposal shall be appropriately numbered, and identified with the RFP number. Please confirm

this is intended for narrative pages only, and not for forms such as PPQs, CCAS, SF 12442, eSRS

reports, SF30’s, bid forms, and any other similar document that is a government form and not

feasible to provide this information on.

a. This is confirmed.

9. In Section 00120 Subfactor 1, Paragraph 2.2.3.3 states Proposal must demonstrate adequate

quality control processes to give the government confidence of the offeror’s ability to

successfully manage the material submittal process, and ensure subcontractor compliance with

contract documents. Please confirm the intent is for this to be the evaluation criteria for

Subcontractor 3: Quality Control (section 2.2.5).

a. Subfactor 1 and Subfactor 3 address separate requirements/information that will be

evaluated relating to Quality Control. Subfactor 1 will evaluate the quality control

processes and offeror’s ability to manage material submittal process and subcontractor

compliance. Subfactor 3 will be used to evaluate whether the quality control staff and

lines of authority are adequate to meet the contracts requirements.

10. Please confirm Section 00010 – Solicitation Contract Form is the Section that shall be used to

insert pricing to be returned with the proposal.

a. This is correct. Please indicate pricing on the CLIN Lines.

11. Is this project Tax Exempt.

a. Yes this project will be exempt from payment of Sales tax, Local Sales Tax, or

Compensating tax when purchasing tangible property or services. See the attached

Example of the Project Exemption Certificate.

12. It is assumed that legible ink is acceptable to complete the bid form pricing items and the

pricing/percentage items in Exhibit D and Exhibit E. It is common in a hard bid such as this to

enable writing in the numbers in these fields to meet the pricing timelines. Please confirm.

a. Yes. Completion of this information in ink is acceptable.

13. Section 00110 states that the offeror may prepare no more than 5 of the Exhibit A Documents,

but the Exhibit A document states no more than 3 projects may be submitted. Please confirm

which is correct.

a. Section 00110, paragraph 2.5.3.4 change the quantity to 5 for Exhibit A. Also Exhibit A is

revised to reflect a quantity of no more than 5.

14. SF 1442 Block 11 states “the contractor shall begin performance within 60 calendar days and

complete it within 730 calendar days after receiving notice to proceed” however 52.211-10

states “720” calendar days after receiving notice to proceed. Please verify which contract

duration should be utilized for the schedule.

a. The correct number of days is 730 days from the date of Notice to Proceed.

15. Are Exhibits D and E the required formats or can we use our own format/plan as long as it meets

the same requirements?

a. The formats in Exhibits D and E are the required formats.

16. Exhibit E Subcontracting Plan form provides a placeholder for just one option. Please confirm

the intent is to provide a combined total for all the option in the Option category, rather than to

create new tables for each option.

a. Yes- Please submit the proposed Subcontracting plan using the Base and then all the

options combined.

17. Section 00110 Proposal Preparation Instructions, Paragraph 2.5.3.12.1 requires consent forms

for key personnel if they are not presently company employees or are subcontractors.

a. Instead of using Exhibit B or Exhibit C for this purpose please provide a letter of intent if

the intended person is not part of the offeror’s organization at the time the proposal is

submitted. If the key personnel will be a subcontractor then a Exhibit B is required from

the subcontractor company and provide a resume as required by Exhibit A.

20. The 2.5.3.11 Past Performance References Section indicates to send the PPQ directly to the

PPQ for submission? Or is it acceptable to include this previously completed PPQ in our

proposal.

a. The requirement is for the PPQ to be completed by the organization that is providing

the reference and to submit it directly to the Contracting Officer. These are to be

emailed to [email protected] and must arrive no later than the closing date

for the receipt of proposals. Questionnaires received late will not be

reviewed/evaluated. Offerors are not to review the Past Performance Questionnaires

prior to submission to the Contracting Officer.

21. We understand that there are no page limits for the Part 1, Exhibit A, Performance Relevancy

Questionnaire. Please confirm.

SECTION 08 1113 – HOLLOW METAL DOORS AND FRAMES - ATTACHMENT

2.4.1 FEMA 361 STEEL DOORS AND FRAMES FOR SHELTERS

A. Manufacturers: Subject to compliance with requirements, provide products by one of the following: Manufacturers listed below produce either standard or custom hollow metal work unless otherwise indicated. Verify specific capabilities with individual manufacturers. 1. Ceco Door Products; an Assa Abloy Group company. 2. Curries Company; an Assa Abloy Group company. 3. Fleming Door Products Ltd.; an Assa Abloy Group company.

B. FEMA Rated Hollow Metal Doors 1. Shelter entry doors and their frames shall resist the design wind pressures for components

and cladding as described FEMA 361, 2015 edition & ICC 500-2015. Only single opening and paired opening doors, and their frames that can resist calculated design wind pressures and laboratory tested missile impacts are acceptable.

2. All doors shall have sufficient points of connection to their frame to resist design wind pressure and impact loads.

3. A protective missile resistant barrier is permitted to protect the door opening. The door should then be designed to resist wind pressures. The size and number of shelter doors shall be determined in accordance with applicable fire safety and building codes. In the event the community where the shelter is to be located has not adopted current fire safety and building codes, the requirements of the most recent editions of a model fire safety and a building code shall be used

4. Sheets are to be made of commercial quality hot dipped zinc coated steel that complies with ASTM A924 A60.

5. Vertical edges will join the face sheets by a continuous weld extending the full height of the door. Welds are to be ground and filled to make them invisible and provide a smooth flush surface.

6. Hinge reinforcement to be not less than 7 gage (3/16”) plate 1-1/4” X 9” prepared for 4-1/2 x 4-1/2 x .180 hinges.

7. Reinforce tops and bottoms of all doors with an inverted continuous steel channel not less than 16 gage, extending the full width of the door and welded to the face sheet. Doors shall have a steel closure channel welded in place so the web of the channel is flush with the top of the face sheets of the door. Plastic fillers are NOT acceptable.

8. Door systems Shall be tested and must comply with the FEMA 361 Design and Construction Guidance for Community Shelters and have available verifiable third party conformance test results.

C. FEMA Rated Hollow Metal Frames 1. To be 14-gauge hot dipped zinc coated steel that complies with ASTM designations A924

A60. 2. All frames are to be assembled so that the face miter seam is “closed and tight”. Weld the

face seam, grind and dress the weld area smooth. Apply a zinc rich primer over the grinding area, and finish with a matching prime paint.

3. Frame assembly for single doors and paired openings, shall be tested and must comply with the FEMA 361-2015 Design and Construction Guidance for Community Shelters and have available verifiable third party conformance test results.

D. Fabrication

1. Provide steel frames for doors to the size and design as shown on the architectural drawings.

2. All finished work shall be strong and rigid, neat in appearance, square, true and free of defects.

3. Jamb depths, trim, profile and backbends to be as scheduled and shown on approved shop drawings.

5. Hardware reinforcements shall be in accordance with the minimum standard gages as listed in SDI-100.

6. Frames shall be mortised, reinforced, drilled and tapped at the factory for template mortised hardware only, in accordance with approved hardware schedule and template provided by the hardware contractor. Where surface mounted hardware is to be applied, frames shall have reinforcing plates only, all drilling and tapping to be done in the field by others.

7. Hinge reinforcement to be not less than 7 gage (3/16”) plate prepared for 4-1/2 x 4-1/2 x .180 hinges.

E. Anchors

1. Anchors for masonry walls to be of the masonry “T” type, Wire masonry type, or Existing Opening anchors. 5 wall anchors per jamb: 4” max from each end of door opening height and equally spaced. 4 wall anchors per head on pairs, 6” max from each end of door opening & 2 at 6” from each side of head centerline on pairs, and 2 wall anchors per head: 6” max from each end of opening width for singles.

2. Dust boxes/mortar guards to be no less than 26 gage. 3. All frames that are to be welded are to have a steel spreader during shipping and handling.

Spreader bars are for bracing only and are not be used to size the frame opening. 4. Punch the stop for 3 silencers on single door and 2 on double door frames.

3.3 INSTALLATION

C. Hollow Metal Doors: Fit hollow metal doors accurately in frames, within clearances specified below. Shim as necessary.

1. Non-Fire-Rated Standard Steel Doors:

a. Jambs and Head: 1/8 inch (3 mm) plus or minus 1/16 inch (1.6 mm). b. Between Edges of Pairs of Doors: 1/8 inch (3 mm) plus or minus 1/16 inch (1.6

mm). c. Between Bottom of Door and Top of Threshold: Maximum 3/8 inch (9.5 mm). d. Between Bottom of Door and Top of Finish Floor (No Threshold): Maximum 3/4 inch

(19 mm).

2. Fire-Rated Doors: Install doors with clearances according to NFPA 80. 3. Smoke-Control Doors: Install doors according to NFPA 105.

Project Scheduling/Phasing/Staging

A. Phasing: This project will be conducted in a single phase.

North Parking Lot must be complete prior to the start of the building construction

Other construction projects will be occurring at the same time.

B. Staging: anticipated staging requirements, access, restrictions, owner use areas. See

C. Site Access & Work Restrictions:

1. Access to the project sites are through controlled security gates. Vetting

forms for completion by the contractor for all employees entering sites are

required. Refer to specification section 011000 Summary, article 1.5.

2. Access to north parking lot site is limited to after the morning rush hour

ends at 8:30 AM to 3:00 PM when evening rush hour begins. Vetting form is

required for site access. See attached Vetting Form.

6. Contractor’s personnel must where badges, obtained daily from check-in desk

at Tice Hall for work on site.

7. Access for cars and light trucks can pass north through the Sherman Ave. gate to

gain temporary access to the north parking lot site from Sherman Ave. See

enclosed diagram.

8. Heavy equipment must access the north parking lot site via the Grant Ave. Post

Gate. A pass is required and is obtained from the Visitor Center, located across

the street from Tice Hall on Sherman Ave. See map with route attached to this

document.

9. Material delivery trucks and flatbeds that can make the turn are allowed in

through the Sherman Ave. National Guard Gate to the south Readiness Center

and NE Remote Vehicle Maintenance sites.

Enter through the Fort Leavenworth entrance along Grant Avenue. Access willbe allowed after rush hour that ends approximately 8:30 AM to 3:00 PM. Allpersons entering the gate shall provide documentation and identification at thebeginning of the project to allow for the visitor entrance process. A Temporary30-day Pass will be issued by Fort Leavenworth personnel. Contractor shallcontact the Architect for vetting paperwork for the process. Once inside thegate, the Contractor shall turn right on Bullard Avenue, turn immediately righton Dickman Avenue, follow the roadway that changes names to WarehouseRoad which connects into Sherman Avenue. At Sherman Avenue, take a rightand proceed to the contractor’s temporary access entrance for the northparking lot. The contractor shall secure the construction site with perimeterfencing and lockable access gate where construction vehicles enter and exitthe site from Sherman Avenue. The Contractor shall lock the gate at the end ofevery day. The cost for providing, maintenance and removal of the temporaryperimeter fencing and entrance gate shall be the responsibility of theContractor. The Contractor shall provide signage stating the gate is closed toall except for construction traffic to the North Parking Lot site.
