This form is to be completed and submitted when requesting an Individual Vehicle Approval (IVA) under the Covid - 19 Priority Vehicle Scheme.

This application can only be processed if your normal application for an IVA submitted under the Technical Application System (TAS) has been processed and released for examination.

Please read all guidance notes, complete all questions and ensure that supporting evidence is provided where requested. Failure to comply will result in rejection of this application.

Note – The vehicle(s) listed will be prioritised by the Approval Authority based on the above categories. Those in 1 and 2 will be allocated the highest priority. All category 1 or 2 applications must be supported by vehicle specific evidence from the end user that describes how the vehicle will be used to support the Covid - 19 response. The final category will be allocated by the Approval Authority.

k) From what date will all the vehicles be available for examination?

Note 1 – All vehicles listed above must be capable of being presented at the time of examination in the fully completed condition. Note 2 – There is no additional fee for arranging the priority examination.

I (i) Cars and Light Goods vehicles subject to “Basic” IVA, all Buses and Priority 5 vehicles (as defined at item “h)” requiring Cleansing/Quarantine prior to examination (if your vehicle is not in this category go to item “m”). Conditions that apply at the time of examination:

• Arrive at least 15 minutes prior to appointment time• Only one person to attend with the vehicle• Open all apertures including doors, lockers, glove box, boots, bonnets windows and load

areas together with switching on any forced air ventilation• Inform our Examiner of your presence (observing social distancing rules)• Be prepared to move your vehicle onto pit/hoist if requestedNote - at this point the examiner may check some items i.e. Lights, brake operation etc. with you in the vehicle.

• Ensure our Examiner has contact details if different from completed PT1 form• Presenter to leave the site during some of the examinationNote - you will be informed when to return

• On completion of the examination, the vehicle will be parked in a safe area away from the testlane

• Upon collection, you should consider the benefit of venting the vehicle with all doors andwindows open for 15 minutes before driving away

l (ii) Special conditions prior to examination.

In accordance with government advice, prior to examination and to minimise the risk of COVID - 19 infection you need to confirm that the vehicle(s) listed on this application will comply with the following:

• that the vehicle has been constructed or modified in compliance with all current COVID -19 Government and industry sector guidelines and, either:

• all vehicle interior surfaces have been thoroughly and virally cleaned immediately before theexamination following current COVID - 19 Government and industry sector guidelines, or

• the vehicle has been securely quarantined for at least 72 hours immediately before thescheduled IVA examination event

If your vehicle/s is/are one of those listed in this item you must sign the declaration below confirming that these conditions will be met at this time appointed for the examination.

Note – At the time of examination you will be asked to leave the site while the examination is conducted. The examiner will refuse to test the vehicle if evidence suggests that the above conditions have not been met.

m) Cars and Light Goods Vehicles subject to “Normal” IVA (except Priority 5 vehicles asdefined at item, “h)”, HGVs and Trailers - Special Arrangements.Conditions that apply at the time of examination: Note –This provision does not apply to the vehicles listed at item “l” above.

• When arriving, you and any passengers must remain in the cab throughout the process unlessdirected by the Examiner

• You will be required to communicate with the examiner through the door window• Doors and windows must be kept closed unless directed by the Examiner• You may be requested to communicate with the examiner via your mobile phone• If asked to move from the vehicle you must keep at least 2 metres from the Examiner• If the vehicle does not match the application or is not a “priority vehicle” the vehicle will be

rejected and the test will not be carried out• Additional documentation requested by the examiner must be sent to the examiner by email• No adjustments or defect correction can be made to the vehicle during or after the examination

Potential for the Approval Authority to consider a Remote Assessment – (Vehicles in item “m” only).

Under the Covid - 19 IVA emergency testing arrangements the approval authority may choose to offer an examination following a remote assessment procedure (RAP). i.e. a process where our examiner is office based and connects with your facility remotely through a video link and guides your employee to demonstrate vehicle compliance by enabling the examiner to see all the relevant items.

This process requires the following to be met:

o Suitable connectivity in the inspection area:

o that you test your video/audio equipment prior to the test dateo that your camera/phone has a fully charged battery

o Level area free from obstructionso Vehicle/trailer clearance: Front/rear – 3 metres – Side - 2 metreso Underside inspection facility: Inspection pit, vehicle hoists or ramps:

o Hoist – attached equipment used to raise vehicle i.e. column lifto Ramp - platform that supports vehicle being raised i.e. four post lift

o Adequate lighting for both topside and underside inspectionso Your nominated employee will require an assistanto Equipment required:

Note - The Examiner will suspend/terminate the inspection, if due to technical issues a thorough test cannot be carried out.

You (the presenter) are responsible for ensuring compliance with all Health & Safety requirements during the assessment.
