Who Write your author’s last name, followed
by a comma:

Add your author’s first name, and if there is no middle name or initial, finish with a period
If your author has a middle name or middle initial, it goes after the first name

What Write the title, and underline it,
and finish it with a period.

Hanging Indention Instead of indenting the first line as you
would in a paragraph, the first line begins at the margin
The rest of the lines are indented 5 spaces from the margin.
Where (the city) Write the city (but not the state) where
the publisher is located, and insert a colon after the city name.
