Welcome to the Practice Assessment Document (PAD)

Student responsibilities

This Practice Assessment Document is designed to support and guide you towards successfully achieving the criteria set out in the Nursing and Midwifery

Council (2010) Standards for pre-registration nursing education.

The PAD makes up a significant part of your overall programme assessment. It will need to be processed through formal University systems. Continuous

assessment is an integral aspect of assessment in practice and you are expected to show evidence of consistent achievement. You should engage positively

in all learning opportunities, take responsibility for your own learning and know how to access support. You will work with and receive written feedback from

a range of staff including Practice Supervisors and Practice Assessors and you are required to reflect on your learning.

You are responsible for raising concerns with a nominated person in the practice setting in a timely manner. You should also alert staff to any reasonable

adjustments that may be required to support your learning.

You should ensure you are familiar with your University assessment and submission processes for this document and contact the academic representative

from your University, or refer to your University’s intranet if you require support or advice on specific University procedures.

The Ongoing Achievement Record (OAR) is a separate document that summarises your achievements in each placement and with the main document provides

a comprehensive record of your professional development and performance in practice.

You are responsible for the safekeeping and maintenance of the PAD. It should be available to your Practice Supervisor, Practice Assessor and Academic

Assessor at all times when you are in placement together with the OAR. Alterations should be made in this document by crossing through with one line, with

a signature and date.

You will have access to confidential information when in practice placements. The PAD should not contain any patient/service user/carer identifiable

information. Contents must not be disclosed to any unauthorised person or removed, photocopied or used outside the placement or University.

People must be offered the opportunity to give and if required withdraw their informed consent to student participation in their care and staff in practice will

provide guidance as required. Before approaching any patient/service user/carer for feedback you must discuss with your Practice Supervisor/Practice

Practice Supervisor responsibilities (Registered nurse/midwife or other registered health/social care professional)

In many practice areas the student will be supported by a number of Practice Supervisors. Some areas may adopt a team based approach due to the nature

of the experience.

As a Practice Supervisor you have an important role in supporting and guiding the student through their learning experience to ensure safe and effective

learning. This includes facilitating learning opportunities including any reasonable adjustments the student may need to get maximum benefit from the

placement. It is your responsibility to contribute to the student’s assessment through the recording of regular feedback on their progress towards, and

achievement of their proficiencies. Specific feedback must be provided to the Practice Assessor on the student’s progress.

Supervision in other placement areas (i.e. those areas where there are no health/social care registrants)

A range of staff can support student learning and have a vital role in student learning and development though may not be contributing formally to assessment

of proficiencies.

However, these staff members are encouraged to support learning and can provide valuable student feedback within the PAD on the Record of

communication/additional feedback pages.

Practice Assessor responsibilities

As a Practice Assessor you have a key role in assessing and confirming the student’s proficiency providing assurance of student achievements and competence.

This includes facilitating learning opportunities including any reasonable adjustments the student may need to get maximum benefit from the placement.

You will observe the student, conduct and record student assessments informed by student reflections, feedback from Practice Supervisors and other relevant

people to confirm achievement. You will liaise with the Academic Assessor scheduling communication at relevant points.

There are numerous elements requiring assessment in practice. One or more Practice Supervisors can contribute to the assessment of some of the

proficiencies in discussion with you, but they must be working in their scope of practice.

When assessing the student, you should take into account sources of evidence that encompass knowledge, skills, attitudes and the views of those receiving

care. Comments should acknowledge those exceptional students who are exceeding expectations for their stage in practice or who have particularly

commendable attitudes, behaviours, knowledge or skills.

If the student is not meeting the required standards this should be highlighted as a development need. If there is a cause for concern or a fitness for practice

issue that requires prompt action, an Action Plan should be instigated to address specific needs or concerns within a specified timeframe. In the event of this,

seek guidance from the Academic Assessor and/or senior practice representative.

Practice assessors cannot, at the same time, act as the practice supervisor or academic assessor for a specific student

Academic Assessor responsibilities

Academic Assessors are Registered Nurses and are nominated for each part of the educational programme. The same Academic Assessor cannot contribute

to the student assessment in consecutive parts. The Academic Assessor will work in partnership with the Practice Assessor to evaluate and recommend the

student for progression for each part of the educational programme.

The student must make contact with the Academic Assessor early in the placement to enable a scheduled communication and collaboration to take place.

All communications/ additional feedback (not already recorded in the scheduled interviews) from the Practice Supervisors, Practice Assessor and Academic

Assessor and other staff members needs to be recorded on the relevant pages in the PAD.

Respecting patient’s consent to treatment and care As a student you have a responsibility to ensure that the principles of informed consent are upheld when you are in practice areas and involved in patient care. You should always introduce yourself to patients as a student nurse/midwife. You must ensure that your involvement in any treatment and care (including observing treatment and care) is with the patient’s informed consent and that you have the patient’s permission to observe or be involved in their care. Patients have the right to refuse to give such consent and may withdraw their consent for you to be involved with their care at any time and this should always be respected. For any patients who are regarded as ‘lacking capacity’ and unable to independently make a decision about their care, you must seek advice from a Registered Health Care professional. You should also ensure that when working with children and young people you understand the laws around capacity, and child and parental consent, including giving and refusing consent for the implementation of any treatment or intervention. Consent is defined as ‘permission for something to happen or agreement to do something’ (Oxford English Dictionary 2018). In relation to health care, it is a general legal and ethical principle that valid Consent must be obtained before commencing an examination, starting treatment, undertaking a physical investigation, or providing care. This principle reflects the rights of a person to determine what happens to their own bodies, what shapes their care, who delivers their care and what support they receive. It is fundamental to good practice. As a student on a programme preparing you for entry to the register with the Nursing and Midwifery Council (NMC), you are expected to abide by The Code (NMC, 2018) which states that “…you must: (4.1) balance the need to act in the best interests of all people at all times with the requirement to respect a person’s right to accept or refuse treatment (4.2) make sure that you get properly informed consent and document it before carrying out any Action’’

Initial meeting of student and practice supervisor / practice assessor

In order to meet the requirements for Progression Point One, all students must be able to demonstrate criteria in relation to; Professional Values, expected

attitudes and behaviours which must be shown towards people, their carers, their families and others, Safety, Safeguarding and Protection of people of all

ages. These will contribute to the achievement of competencies for entry onto the register; these competencies have been grouped under the following

domains;

• Professional Values

• Communication and Interpersonal Skills

• Nursing Practice and Decision Making

• Leadership, Management and Team Work

The criteria for Progression Point One have been mapped against the NMC Domains and the Essential Skills Clusters (2010) have been used to develop

practice outcomes, where appropriate, these will contribute to the achievement of competencies for entry onto the register, the full competencies for

entry to the NMC register can be found in the Practice Assessment Document for Year 3.

The initial meeting between the student and a practice supervisor and/or practice assessor must take place during the first week.

Mid Placement Review

The mid placement review must take place during the middle two weeks of a student’s placement. This allows the practice supervisors, practice assessors

and students the opportunity to review progress and initiate any actions which need to be taken to allow the opportunity for achievement of all NMC

criteria for Progression Point One. Feedback must include discussion against the Assessment Framework and agreed actions to support the achievement

of the NMC criteria

Initial meeting of student and practice supervisor / practice assessor

In order to meet the requirements for Progression Point One, all students must be able to demonstrate criteria in relation to; Professional Values, expected

attitudes and behaviours which must be shown towards people, their carers, their families and others, Safety, Safeguarding and Protection of people of all

ages. These will contribute to the achievement of competencies for entry onto the register; these competencies have been grouped under the following

Assessment of Professional Conduct in Practice

During each practice experience, irrespective of length, an assessment must be made of the students

Professional Conduct, against the Essential Skills Criteria;

1.1 Articulates the underpinning values of The Code: Professional standards of practice and behaviour for nurses, midwives and nursing

associates (NMC 2018).

14.1 Works within the code (NMC 2018) and adheres to the

Guidance on professional conduct for nursing and midwifery students. (NMC 2010)

Two consecutive unsatisfactory assessments or two unsatisfactory assessments within a year will result in discontinuation from the programme. This

assessment has been written in accordance with the NMC Guidance on Professional Conduct for nursing and midwifery students (2010).

Should there be an instance where professional conduct cannot be assessed the circumstances will be reviewed by the NURS 1000 : Professional

Responsibility ( 1 ) module leaders. Two instances within the programme will result in a student concerns form being instigated.

As a student, you have a responsibility to ensure that you act in a way that is expected by the Nursing and Midwifery Council at all times.

Professional Conduct not only refers to the way you conduct yourself but also to the way you develop both in your manner and your knowledge and skills.

Through assessment it is expected that you utilize your set objectives, your own personal objectives and those identified by your practice supervisor and

practice assessor in order to increase your competence to deliver safe and effective care. When addressing Professional Conduct your practice supervisor

/ practice assessor will make a judgment as to whether you have developed to the level required for your stage of training, both in your overall attitude

and your application of theory to your practice delivery.

Your Personal Life Counts Too

It’s important that you’re aware that your behaviour and conduct, both during your programme and in your personal life, may have an impact on your

fitness to practise, your ability to complete your programme and the willingness of your university to sign the declaration of good health and character in

order for you to register with the NMC. Ensure that you familiarize yourself with the NMC Guidance for student nurses and midwives (2010).

Inappropriate use of Social Networking sites will also be considered against Professional Conduct the NMC State;

“Nurses and midwives could be putting their registration at risk if posting inappropriate comments about colleagues or patients or posting any material

that could be considered explicit”

Guidance for Practice Assessors:

It is important that students are supported to understand what is expected of them when developing a professional attitude towards their practice.

Discuss your expectations during the initial interview and allow the student the opportunity to ask any questions in relation to professional practice. The

following guidance should be used to ascertain whether the student has been able to achieve a satisfactory outcome in relation to Professional Conduct.

Whilst it is the practice assessor responsibility to make the assessment of professional conduct, it is important that students behave in the same manner

with all members of the team, including practice supervisors, non-qualified personnel, service users and colleagues. Anyone who raises concerns relating

to conduct should document this in the box below, ensuring they complete the signatory page.

If problems arise during the experience it is important that you discuss these concerns with your student and advise them on what changes need to be

made in order for them to successfully complete this part of the overall assessment, use the box below to document any conversations you have regarding

your students conduct.
