4 Ps of Hourly Rounding

Pain: Does that resident complain of or appear to be in pain? If yes, inform the nurse.

Personal needs: Does the resident need to use the toilet? Check incontinent brief. Ensure resident is clean and dry.

Position: Ensure resident is repositioned and placed safely in bed/chair. Is the bed in the lowest position? Are fall out mats in place?Possessions: Ensure that call bell, TV remote, walker/wheelchair are all within reach. Check that the environment is clean and no fall hazards are present.
