Prepare an AHDConsider preparing a living will and/or advance healthcare directive (AHD) or medical power of attorney.

Choose RepresentativesConsider appointing a Healthcare Proxy and/or a Financial Power of Attorney.

A Healthcare Proxy Person you appoint to legally speak on your behalf and make healthcare decisions if you are unable to.

A Financial Power of Attorney Person you appoint to legally act on financial matters if you are unable to.

Prepare a WillIdentify your final wishes for your funeral services and for distributing your financial and digital assets and other possessions after your death.

Planning ahead can bring you peace of mind today. Your End-of-Life plan can be as easy as taking a few first legal steps and completing a few legal forms.

Complete these documents if you want them as part of your End-of-Life plan:

Physicians Order for Life-Sustaining Treatment (POLST) A POLST is a physician-signed order that explains your wishes for End-of-Life care.

Do Not Resuscitate Order This document states your preference not to have cardiopulmonary resuscitation performed to save or prolong your life.

Organ Donor FormThis form provides legal proof that you want to donate your organs or tissue.

Final Steps to take to ensure your legal choice are honored:

Medicaid is a joint Federal and State health insurance program to help people pay for medical costs.

M e d i c a i d S p e n d D o w n I f you have too many assets you can st i l l qual i fy i f you “spend down”

your income unt i l the threshold is reached.

How It’s DoneMedical expenses are subtracted from your income. Once your

income reaches a pre-determined threshold, you qualify for benefits.

Paying off legitimate debts can also reduce your total assets.

A s s e t C a t e g o r i e s Assets are divided into two categories: “exempt” and “non-exempt” assets.

Your state of residence determines the specific criteria for each.
