TMHC offers free and confidential medical exams to team members for personal injury and illness. Due to IRS guidelines, team members covered under the HSA medical plan are not eligible for this service. • Click on NMMC & Clinics Benefits/Payroll Questions

• Complete the Human Resources Request form. – To add an attachment, click the paper clip icon on top right of the request form.

• Click Submit: You will receive a confirmation email verifying that your request has been received.

Both plan options are offered through the PreferredHealth Network which includes providers from North Memorial, Fairview and HealthEast. 2017 WELLNESS PROGRAM CHANGESWellness Dollars

• To be eligible for the wellness dollars, you and your enrolled family members will be required to receive a preventive exam at a North Memorial owned clinic.

• You will receive your wellness reimbursement on your paycheck once we are notified by PreferredOne that the preventive exam has been completed.

• For the 2017 plan year wellness dollars will now be paid as “cash” (subject to applicable payroll taxes).

• Each enrolled member on your health plan will be eligible for an annual $400 cash reimbursement (maximum of $2,000 paid per family).

• You may use your wellness reimbursement dollars for any type of purchase. No receipts or forms required.

You are only eligible for one preventive exam per year through your insurance. If you see a provider outside of a North Memorial owned clinic for your preventive exam, you will not be eligible for the wellness dollars.

Team Member Health Center no longer administers the wellness program. For questions submit a Human Resources request form through Service Now.

2017 OPEN ENROLLMENTEVERYONE must enroll in benefits online. Even if you aren’t changing your coverage, you must re-enroll to continue your benefits. If you do not enroll in benefits for 2017, you will not have coverage.

GET STARTEDIt is important to log in to MyHr before Open Enrollment begins to make sure you know your password and to confirm your address, email, phone number and contact/dependent information is accurate.

To access MyHRFrom a home computer (https://myhrnorthmemorial.ultipro.com) or from a North Memorial Health Care computer:

• Log in to the North Memorial Health Care portal > click on the MyHR folder > click on the MyHR icon• Enter your user name (e + team member number, for example: E01234)• Enter your network password (password used to log on to network computers)If you forgot your password or don’t have one, or have trouble accessing the MyHR, call IT at (763) 581-2580.

MyHr requires Internet Explorer version 11.0 or Firefox version 44 or Chrome version 48. Other browsers may work, but some features may not be available.

To review and update your contact information• In the upper left corner of the screen, hover your mouse over Myself and click on Personal > Name,

Address, and Telephone• To make changes, click Edit, make your changes, and click Save when you are finished• Hover your mouse over Myself and click on Personal > Contacts• To make changes, click Edit to make changes or Add to add a new contact or dependent.
