2.0: STATEMENT OF INTENT 2.1 Purpose:

This agreement represents the collaboration between all Member Groups listed in Section 1.0 to improve collaboration, coordination, and the capacity of the system to better meet the needs of youth who experience psychosis and their families. We will formalize our NorthBEAT Collaborative using collaborative partnership best practices, and co-design resources that will make it easier for youth with psychosis to navigate the healthcare system and receive timely and appropriate intervention.

2.2 Objectives:

• To improve pathways and connections between services and sectors at key points of access. • To improve the quality of community/collaborative care and pathways to care. • To address critical system gaps for Northwestern Ontario's vulnerable populations. • To adapt existing educational resources to provide useful and relevant materials for a range of

target audiences, including but not limited to: youth, families, vulnerable groups, and "gatekeepers".

2.3 Timeline:

The Collaborative Agreement will begin May 2017, and will continue through March 2021, the duration of the Ontario Trillium Foundation funding period, and will be ongoing to ensure the continued success of the program.

3.0: STRUCTURES AND PROCEDURES 3.1 Coordination and Administration:

St. Joseph's Care Group is the lead applicant and has entered into a contractual relationship with Ontario Trillium Foundation. As lead applicant, SJCG and the NorthBEAT Coordinating Team (led by Dr. Chiachen Cheng) will assume all administrative responsibilities and activities related to project coordination, including: sole spending authority, personnel and volunteer recruitment/hiring, data collection and management, marketing and dissemination, newsletter development, booking and coordinating meetings, media and public relations.

3.2 Roles and Responsibilities:

Members will be asked to participate as members of one or more Groups that best fit their organization's role/context.

• Resource Co-design: co-design and adapt existing educational resource for use by mental health care providers. Contribute feedback, organizational context, relevance and impact of these resources. Use the resources, and disseminate.

• Increase Capacity: educate collaborative members about the service context at CAPS, and as children's mental health lead for the region.

• Policy Updates: shift the system towards a more integrated model of care by championing the educational resources, updates to internal policies/procedures, and mandatory training to include early psychosis intervention.

care for youth with psychosis by mapping the processes and pathways to mental health care, and identify the community resources available to youth and their families.

• Resource Co-design: co-design and adapt existing educational resource for use by mental health care providers. Contribute feedback, organizational context, relevance and impact of these resources. Use the resources, and disseminate.

• Increase Capacity: bring to collaborative key youth leaders through CCTB, educate collaborative members about the service context at CCTB, and as children's mental health lead for the region.

• Policy Updates: shift the system towards a more integrated model of care by championing the educational resources, updates to internal policies/procedures, and mandatory training to include early psychosis intervention.

Member 1.06 Dilico Anishinabek Family Care

• Group(s): Healthcare Providers' Advisory Group • Service Mapping: assist project coordinating team to enhance system of mental

health care for youth with psychosis by mapping the processes and pathways to mental health care, and identify the community resources available to Indigenous youth and their families.

• Resource Co-design: co-design and adapt existing educational resource for use by mental health care providers. Contribute feedback, First Nations cultural context, geographic context (e.g. remote, fly-in), relevance and impact of these resources. Use the resources, and disseminate.

• Increase Capacity: bring to collaborative key youth leaders through Dilico, educate collaborative members about the service context at Dilico, and unique situation of Indigenous children’s mental health provider.

• Policy Updates: shift the system towards a more integrated model of care by championing the educational resources, updates to internal policies/procedures, and mandatory training to include early psychosis intervention.

3.3 Decision-Making:

Decisions will be made by consensus and will involve representatives from all Members listed in Section 1.0. A minimum of 50% +1 members are required for quorum.

3.4 Core Partners’ Group: Any conflict not resolved by the roles and responsibilities described above will be addressed by the Core Partners’ Group. This Group would be chaired by the NorthBEAT Program Coordinator, and would consist Project Coordination Team (including C. Cheng, project staff), with the core members of this collaborative, including at least 2 youth and family members with lived experience.

3.5 Youth Advisory Group:

Chaired by Program Coordination Team (SJCG). Membership will include six youth recruited via Member Groups. Advisory members will co-design and adapt existing educational resource for youth use/public education. Advisors will also be asked to contribute feedback to the Collaborative on a regular basis via meetings/teleconferences for the duration of the project.

3.5 Family Advisory Group:

Chaired by Program Coordination Team (SJCG). Membership will include six family members recruited via Member Groups. Advisory members will co-design and adapt existing educational resource for families' use/public education. Advisors will also be asked to contribute feedback to the Collaborative on a regular basis via meetings/teleconferences for the duration of the project.

3.6 Communication amongst members:

Open and transparent communication will take place between the Project Coordination Team (SJCG) and all Member Groups at all times, including minutes will be taken at meetings and shared with all parties. The Collaborative will meet regularly via conference call. The Coordinating team will send out regular email correspondence, including but not limited to: newsletters, meeting reminders, task follow up, etc

3.7 Management of Funds:

St. Joseph's Care Group is the lead applicant and has entered into a contractual relationship with Ontario Trillium Foundation. Representatives from SJCG will sign the Letter of Agreement, accept the funds, and assume responsibility for fiscal accountability and all required reporting.

3.8 Reporting: St. Joseph's Care Group will be responsible for the reporting requirements of the Ontario Trillium Foundation funding agreement.

4.0: RESOURCES 4.1 We will provide the following resources to the Project:

a) The Collaborative will co-develop EPI e-learning modules for use as a training resource for all Member Groups listed in Section 1.0, and will remain available after the cessation of the Collaborative. The intention is that the Member Groups will implement this training into their mandatory staff training and orientation procedures.

b) Provide teleconferencing and videoconferencing lines, and other supplies for meetings c) Travel stipends for in-person meetings/workshops.

4.2 Invizzen Knowledge Group will provide the following resources to the Project:

a) Expertise in: Partnership Brokering, Best practice implementation in community settings, Change management, Strategic communications and marketing; Facilitation and consensus building; Adult education and eLearning best practices; Interactive on-line resources; Agile project management; Developmental evaluation and process improvement

b) Experience in: collaborating with First Nations and Metis elders and communities in northern Ontario; creative ways to engage youth with psychosis and their system of care in knowledge to action projects; developing interactive eLearning programs for physicians, health care providers, service providers, patients, families and youth based on research evidence, best practice guidelines and lived experience.

c) Facilitation toolkit: Partnership brokering and evaluation frameworks; Facilitation, change management, process improvement and system change models and techniques.

d) eLearning toolkit: Software to edit videos and audio, create interactive eLearning (Articulate), a basket of tools to develop creative on-line tools; instructional design, technical IT and graphic design expertise.

4.3 Member Groups will be asked to provide the following resources to the Project:

a) In-Kind staff time for attending teleconferences/meetings (maximum one hour per month) and in-person meetings/workshops (up to two days per year) for the duration of the project.

5.0 LIMITATIONS 5.1 This agreement does not permit the use of copyright materials (including logos) or the dissemination

of confidential information to any third party without the written permission of the Member(s) concerned.

5.2 This agreement does not bind Member Groups or their staff / officers to any financial or other liability

without further formal documentation.

5.3 The resources developed as part of this initiative will remain the intellectual property of NorthBEAT and St. Joseph's Care Group. Member Groups will receive secondary authorship on all resources as well as full acknowledgement wherever possible. Materials produced or distributed as part of this Collaborative are not to be reproduced or further adapted without explicit written consent by NorthBEAT or one of its official representatives.
