Direct Aid Program
Program Overview• Types of Grants
– Food – Publix and Winn Dixie - Max $100 in 3 months
– Gas – Mobile - - Max $100 in 3 months– Transportation Grants
• Checks made payable to Transportation Providers
– Emergency Grants for Utilities or Rent - Max $250 1 time a calendar year
Application process – Online only
– Application through Clinic Social Worker– Must have signed authorization from
patient attesting to information faxed. – Must provide financial information for all
family members– Must provide information on how any
outstanding balance will be paid
Approval Process
• Application is reviewed by a Social Worker– Miami, Tampa, Jacksonville, Orlando
• Applications are checked to make sure that information is complete and makes sense according to the applicants circumstances.
• Social Workers make suggestions on other resources available.
• NKF Staff makes sure that amount of grants do not exceed monthly budget.
If Approved
• Gift cards for Gas or Groceries and checks for transportation services made to the Transportation provider are sent to the applying Social Worker for distribution.
• $250 Emergency Grants are sent to the Utility provider or landlord.
• Correspondence may be included advising other resources available.
• Checks are never made payable to the patient.

Example
• Pt is 71 year old incenter Dialysis patient who has recently been diagnosed with an intestinal infection. She continues to lose weight and her albumin is steadily declining. Pt receives only $16 in food stamps monthly and is unable to buy the types of high protein foods that she needs on her $1000 per month fixed income. She is on the waiting list for Meals on Wheels, but list is long. She is a widow and her only daughter died of Leukemia. Grandchildren help when they can. She is requesting a $100 Publix card.
