What will this program cover?
Why learn about kidney disease?
Knowing how to make informed choices about your health can help you live a longer, healthier life.
Your Treatment, Your Choice is a new program from the National Kidney Foundation (NKF) that will help you understand your kidney health, your treatment choices, and what you can do to live well with kidney disease. Your Treatment, Your Choice was especially designed for Medicare's new kidney disease education benefit.
Six classes provide up-to-date information on chronic kidney disease, how it affects your body, available treatment choices for kidney failure (including transplant, at-home dialysis and in-center dialysis), and what you can do to live well with kidney disease. Classes are informal and run approximately one hour each.
Where are classes offered?Ask your doctor or other health care profes-sional about Your Treatment, Your Choice and the new Medicare benefit for kidney disease education.
What will this service cost?You may be asked to pay 20% of the Medicare-approved amount, and, if so, the Part B deductible applies.

Did you know that you may qualify?You may qualify for a new Medicare benefit! Beginning January 1, 2010, Medicare will pay for education about kidney disease if you have:
• Medicare
• Chronic kidney disease stage 4
• A referral from your doctor
There are 5 stages of kidney disease
• The stage of kidney disease you have is based on your Glomerular Filtration Rate (GFR).
• Your GFR tells you how much kidney function you have.
• People with stage 4 kidney disease have a GFR between 15 and 29.
• If you do not know your stage of kidney disease, ask your doctor or other health care professional.
