Training Quick Steps Using the Patient Portal The Patient Portal A unique URL is assigned to the Patient Portal to ensure its secure relationship with your clinic’s primary EMR database. This URL will be provided to your web designers to enable your patients to easily access it directly from your practice’s web site. Typically your web site will include a link or a button clearly identified as Patient Portal. Clinic Web Site The overall appearance of your web site and the location of the link to the Patient Portal will vary. The link may show as a call-out frame (as seen in this example) where the patient directly enters his/her User ID and Password or it may simply be a button or a hyperlink. See your web designer more specific instructions as applicable.

Logging into the Patient Portal Patients will login to the Patient Portal from the clinic’s/physician’s web site. Once the patient has logged in, there will be a drop-down selection box which includes the clinic’s Patient Forms. Once the patient completes this data (e.g.: Current Medications, Past Medical History, etc.), the data will flow directly into PrognoCIS for the physician’s review and approval.

Note: Logins are maintained directly within PrognoCIS (see instructions and rules below).

1. Enter the Old Password (the original one assigned by you on the Patient Register). 2. Enter the New Password (of your choice – see rules above). 3. Re-enter Password (i.e.: enter the new password a second time). 4. (Optional), enter a Secret Question in case you forget your password. 5. (Optional), enter the Answer to your secret question in case you forget your password. 6. Click the Save button

The Patient Portal Home Page The contents of the Patient Portal Home Page are similar to the Patient Encounter screen within the EMR application. The applicable Properties can be customized to control the data that is available to the patient. Typically, Patient Forms, demographics, and insurance areas will be available. In addition, the clinic also define specific areas of the patient’s chart (e.g.: Labs, Prescriptions, etc.).

Patient Summary By default, the upper right corner of the home page displays a Patient Summary. This includes the name of the patient who is logged in. Similar to the Patient Band that displays across the Patient Encounter screen in the EMR, you can define specific tags to display in this area (e.g.: Encounter Date, Primary Physician, etc.). This data is pulled directly from the Patient Register within PrognoCIS and is not editable by the patient. Note: All settings are defined through Settings Configuration Properties.

Summary The Summary is similar to the Face Sheet and includes hyperlinks to various portions of the patient’s chart within PrognoCIS. The contents of these links scroll directly underneath the section heading without changing screens.

Note: The clinic may choose to display the Summary (which is the default) or another option such as Past Visits.

Table of Contents The Table of Contents includes links to specific aspects of the patient’s chart within PrognoCIS (e.g.: visit history, test results, intake forms, etc.). This enables the patient to view specific clinical data, request prescription refills, schedule an appointment, complete registration forms, to update demographic and financial information, and to otherwise communicate with the clinic or physician via the Message Center.

Note: All settings are defined through Settings Configuration Properties.

Health Record This section provides access to the patient’s previous Encounter history, visible test results, and comparison matrices of Vitals and Laboratory results. Clicking the desired link will display the applicable data in the right panel of the screen. If there is no data, the section heading will display, but the screen will be blank.

Summary – returns the user to the top of the Patient Portal.

Past Visits – displays a table of all previous encounters within PrognoCIS for the patient.

Test Results – displays all applicable Laboratory and/or Radiology results the doctor has defined as visible.

Vitals Matrix – displays a grid of the patient’s vitals as compiled through the previous encounter history.

Lab Matrix – displays a grid of that patient’s laboratory tests/results through the previous encounter history.

Clinic Interaction This section allows the patient to request a prescription refill, scheduled an appointment, or complete registration/intake forms.

Renew Prescription – displays the RENEW panel, which lists all of the patient’s current/active prescriptions and allows the patient to select which one(s) needs to be refilled.

After selecting the check box ( ) beside the desired medication and clicking the Request button, the system will prompt with a confirmation. A message will be sent to the appropriate person within the clinic to request the refill from the pharmacy.

Appointment – displays the SCHEDULES panel, which enables the patient to request an appointment for a specific provider within the clinic.

To select the desired appointment:

1. Click the next to the Doctor: pick list and select the desired provider.

2. Click the next to the Location: pick list and select the desired clinic location. 3. Enter the Reason: for the appointment.

4. Enter the On/After: date desired for the appointment, or click the to select it.

5. Click the next to the Enc Type: pick list and select the type of appointment desired.

6. Select the appropriate check boxes ( ) within the Search for these days frame.

7. Click the next to the From: and To: pick lists and select the desired Search for Time Range. 8. Click the Find button, which will populate all available appointments within the specified criteria. 9. Highlight the desired time slot for the desired provider and click the Take Appointment button. 10. (Optional) If the desired time slot is not displayed, click the Find Next button and repeat step 9. 11. The selected appointment will display within the SCHEDULES pane.

Patient Form – displays the Patient Intake Form window, which allows the patient to complete the applicable registration and medical history information.

To select and complete the desired form:

1. Click the next to the Forms: pick list in the upper left of the screen. 2. Select the desired form.

3. Messaging This section enables the patient to correspond with the clinic via email within the PrognoCIS system. He or she may receive messages from the clinic or send messages to the clinic.

Creating a Patient’s User Login

1. From the Patient Encounter, select Patient Register.

2. Click the login info… button.

3. Assign the UserId. 4. (Optional) Enter the patient’s Email Id. 5. (Optional) Select the AutoGenerate check box for the system to generate a password

automatically. 6. Enter a valid New Password. 7. For validation, ReEnter New Password. 8. Click the ok button.

Notes:

The Userid initially defaults to the patient’s first name; however, you may change it.

If an email is specified for the patient, a confirmation email will be sent from within PrognoCIS informing the patient of his/her login ID and password.

The initial password is only good the very first time it is used. The system will prompt you to change it.

Passwords must be between 8-10 digits in length and are alphanumeric. It must contain at least one number and one letter with any other combination of digits. Once you change it to a unique password, it will not expire; however, you will always have the option to change it at will if you so desire.

When choosing to Auto Generate the password, a valid Email Id must be specified in order for the patient to be notified of the password.

Patient Portal Properties You can determine what displays on the Patient Portal Table Of Contents by defining certain properties within PrognoCIS itself.

1. Select Settings Configuration. 2. Select Admin Properties. 3. Search for Patient Portal.

Note: This feature requires proper security access and may need to be defined by Bizmatics Customer Support or Implementation personnel.
