National Services - 2017 Service and Funding Review Process for 2018/19 Background In 2010 a programme of National Services was endorsed by the Minister of Health, where it was identified that a national, coordinated, approach was required. A number of services were established and implemented as national services in 2012. In addition, there are a small number of services that are centrally funded, that pre-dated the National Services work programme. Scope and Purpose This document provides a framework for the National General Managers Planning & Funding Network to review funding in Services being managed under the National Services programme, including those funding through Top Slice, where there may be an impact on clinical performance or quality. Key elements of the framework are: that there is a known timetable for consideration of funding requests, where these can be considered and prioritised in association with other local priorities there is a transparent and clear expectation of the information required to support a review of funding. Within scope for this Funding Review Process are Services that are endorsed National Services, or services that are funded through Top Slice, outside of normal Population Based Funding (as identified in Appendix One). National Service Improvement programmes, and other DHB funded services, including those being supported to develop pathways or guidelines, are out of scope. Service and Funding Review Process for 2018/19

BackgroundIn 2010 a programme of National Services was endorsed by the Minister of Health, where it was identified that a national, coordinated, approach was required. A number of services were established and implemented as national services in 2012. In addition, there are a small number of services that are centrally funded, that pre-dated the National Services work programme. Appendix One identifies:

Decision makingNational Service funding decisions will be made by the National General Managers Planning and Funding Network.

Decisions on whether a Funding proposal will progress to the Network will be made by consensus (or majority) of the National Services Governance Group. The Governance Group is comprised of Ministry of Health, host DHB and funding DHB representation.

The Group’s purpose is to provide cross sector advice, oversight and leadership for National Services to ensure they are supported to deliver patient focused, high quality and accessible services that are clinically and financially sustainable.

Process and Key Datestimelines 1. A Funding Review may be initiated either by a Host DHB, or DHBs referring patients to the

service, particularly where there are changes in technology or model of care that impact on how and where services are provided.

[2.] The National Services Governance Group endorsement: all Funding Review Initiation templates will be reviewed by the Governance Group to determine whether there is sufficient merit for the request to progress to development of a business case. The Governance Group will also review business cases, prior to submission to the National General Managers Planning & Funding Network for consideration. The Governance Group recommendations will be included with the business case submission. The 2017 meeting dates are in Table 1. Meeting papers are provided 7 days prior to the each quarterly meeting, these are generally held in March, June, September and December of every year.

[3.] The National General Managers Planning & Funding Network approval: The Network will consider a business case, along with the Governance Group recommendation at their November next quarterly meeting. In some cases, with Network approval, requests may also be tabled in December 2017. Funding changes from November or December 2017 may be conditionally approved pending confirmation in February. The 2017 meeting dates are in Table 1. Meeting papers are due 10 days prior to the meeting.

The Ministry of Health and the National Services Governance Group will make every effort to provide timely feedback to the initiating DHB on their request. Indicative feedback timeframes are:

within two weeks of receipt of the draft business case the initiating DHB will be advised of any additional information required

within one week of any Governance Group review the initiating DHB will be advised of the outcome of that review

where a review is initiated by a referring DHB, the host DHB will be advised on the pending review

within two weeks of a National General Managers Planning & Funding Network review the host DHB will be advised of either the decision or the next steps.

Format of Funding proposalsProposals will be submitted on the approved Funding Review Initiation template and/or Business Case template (Appendix Three).

Endorsing funding updatesFollowing approval by National General Managers Planning & Funding Network, any change that involves a recommendation to change the Top Slice will need to be approved by the Minister of Health, as part of the Funding Envelope.

Changes endorsed at the November meeting may be implemented as either an IDF Programme change, or a Top Slice change.

Changes endorsed after the November meeting may only be implemented as an IDF Programme change for the next financial year, but may be considered as a change to the Top Slice within the following year’s Funding Envelope.
