2017Annual Report
Featured Grantees of 2016-17In FY 2017, Health Trust grants sustained and strengthened 15 local organizations. These groups provide nutrition education to youth and adults, build skills (such as healthy home cooking) that prevent chronic disease, and offer structure and social support to help individuals reach health goals. Grantee organizations also assist individuals to bridge gaps in access to mental and oral healthcare, where needs are great. We highlight a few grantee-partners’ efforts below. Learn more at www.pvhealthtrust.org/grantmaking.
Hospice of Santa Cruz County ($5,756) — For a child, a loved one’s death can be devastating and have lifelong consequences. Funding supports Hospice’s innovative Children’s Grief Support program, which provides young people ages 6-17 with individual bilingual counseling and grief education, group support, and a special camp where youth learn healthy strategies to cope with loss.
Salud Para La Gente ($11,000) — Funding supports Salud to provide subsidized dental care to uninsured local residents.
Dientes Community Dental Care ($12,000) — Funds support the costs of an Oral Health Educator who informs primary healthcare providers, pediatricians and other health professionals about recent research and clinical practices linking oral healthcare and improved health outcomes.

The PV Health Trust is a 501(c)3 nonprofit organization.
This past year brought long-awaited rains to the Pajaro Valley. As our fertile region enjoyed a restoration of moisture, a parallel of process renewal took place at the Health Trust. Within our walls, we invested in the King Community Room, a safe and welcoming space where individuals can gather and social connectedness can flourish. Programmatically, we invested more than $1.1 million to deliver direct health education and services; award grants that promote access to better health through education, exercise, diet, social support and other lifestyle changes; and confer scholarships to local youth choosing careers in health professions, helping mitigate the looming regional shortage of skilled, culturally responsive healthcare providers.
Against the backdrop of change, our commitment to preventing and mitigating the challenges posed by diabetes remains unwavering. UCLA Center for Health Policy Research recently reported that 55% of California adults have pre-diabetes or diabetes. Locally, approximately 40% of those seen at our Diabetes Health Center in FY 2017 were under age 20. These realities compel us to develop and implement new solutions to prevent the onset of diabetes and create an environment conducive to good health.
We know the solutions surround us – in the playing fields and classrooms, homes and workplaces, farms and ranches across the Valley. In response to opportunities and community members’ needs and hopes, we will continue to unite our assets, aspirations and expertise with organizations and people who share our vision of a healthy community and further the Health Trust’s mission: to improve health and quality of life for all residents of the Pajaro Valley.

Dear Friends, Please Join Us!The conditions in which we live,
work and play influence whether we experience chronic disease, violence or injury – even the occasional flu or cold. Our environment and our community matter. Community Health is at the heart of our name. This reflects our
commitment to provide nutrition and lifestyle counseling to those with chronic
health concerns. Community Health drives us to strengthen nonprofits that
teach young people healthy eating habits and safe, fun ways to stay active. Community Health also motivates us to ensure access to healthcare for all those
facing obstacles to care.
We invite you to participate in fostering individual and community health. Every
dollar connects people to health-improving resources. Join us!
“The Diabetes Health Center team has given me the tools and encouragement to live a healthy and long life along with my family, friends, husband and son. It has been a life changing resource for me, and I am thankful. …”
— Yuliana Mendoza-Flores, Health Trust board member
