MyBenefits Online Benefits Enrollment following a Qualifying Event This guide will help you navigate MyBenefits (mybenefits.nd.edu), the University’s online benefits enrollment and change application. MyBenefits Online Benefits Enrollment following a Qualifying Event

This guide will help you navigate MyBenefits (mybenefits.nd.edu), the University’s online benefits enrollment and change application. Benefit-eligible faculty and staff are required to submit an online benefits change request to the Office of Human Resources within 31 days of experiencing a qualifying event.

What You Will Need

• Your active University NetID and password. Click here to activate - https://accounts.nd.edu/activation • Biographical information for eligible dependents (Ex. Social Security Number, Date of Birth) • Dependent verification documentation (if adding dependents) • Other required documentation specific to certain qualifying event (divorce, dependent gain/loss of coverage)

Enrollment Change Requests are a Two-Step Process

Step 1. Log in to the secure site to process your benefits change request. Access MyBenefits via the quick app link in https://inside.nd.edu or visit https://mybenefits.nd.edu and login with your University NetID and password.

Step 2. If enrolling dependents such as a spouse or child, you must present documentation to the Office of Human Resources to verify each dependent’s relationship within 31 days of experiencing a qualifying event. This step should be completed following successful completion of your online benefits change request. Please review the University’s dependent verification document at https://hr.nd.edu/benefits/group-insurance/dependent- verification-for-university-benefits for a list of acceptable documents.

Step-by-Step Online Instructions

1. MyBenefits Employee Dashboard

The employee dashboard provides instant access to your current benefit elections, dependent child(ren) and spousal information, enrollment summary, and confirmation statement. To view this information, select the corresponding widget.

2. Initiating a Qualifying Event – Select Make Changes to begin the process.

Select the appropriate qualifying event and enter the Event Date.

Select Save to proceed to the next step in the change process.

3. Landing Page – Review the welcome message, then select Continue.

4. Dependent Verification Process – Review the dependent verification requirements, check the confirmation statement, and then select Save & Continue to proceed to the next step.

5. Dependent Child(ren) and/or Spousal Information

If you will be adding a dependent spouse and/or child(ren), click Add on the appropriate page to enter their biographical information.

Complete the highlighted fields. If entering a spouse or single child, select Save & Continue to proceed to the next step in the enrollment process.

To enter additional children, select Add Another Child. Repeat this process until all children have been added, then select Save & Continue to proceed to the next step in the enrollment process.

6. Enroll Now – Medical Insurance

Step 1. Select the check box next to each dependent you wish to cover on your Medical Plan within the “Members Covered” section. If a dependent is not listed, select Back to add this dependent on Page 7.

Step 2. To elect your Medical Plan, select Enroll within the plan window you are selecting. Your current medical plan election will be displayed in the upper left corner, marked with a green “enrolled” banner.

To view additional information for each plan, including a benefits overview and links to search a list of in-network providers, select Benefit Description.

To waive Medical Plan coverage, select Enroll within the Medical Waiver window.

7. Enroll Now – Dental Insurance

Step 1. Select the check box next to each dependent you wish to cover on your Dental Plan within the “Members Covered” section. If a dependent is not listed, use the left hand navigation to select Page 7. Add your dependent, then proceed to Page 9 using the left hand navigation.

Step 2. To elect your Dental Plan, select Enroll within the plan window you are selecting.

To view additional information for each plan, including a benefits overview and links to search a list of in-network providers, select Benefit Description.

To waive Dental Plan coverage, select Waive this benefit in the upper right corner of the page.

8. Enroll Now - Vision Insurance

Step 1. Select the check box next to each dependent you wish to cover on your Vision Plan within the “Members Covered” section. If a dependent is not listed, use the left hand navigation to select Page 7. Add your dependent, then proceed to Page 10 using the left hand navigation.

Step 2. To elect your Vision Plan, select Enroll within the plan window you are selecting.

To view additional information about the vision plan, including a benefits overview and links to search a list of in- network providers, select Benefit Description.

To waive Vision Plan coverage, select Waive this benefit in the upper right corner of the page.

9. Enroll Now - Base Life Insurance

The University provides a $25,000 base group term life insurance policy through Minnesota Life to eligible faculty and staff. You will be automatically enrolled in this plan.

Select Continue to proceed to the next step in the enrollment process. To view additional information about the base life policy, select Benefit Description.

10. Enroll Now – Supplemental Life Insurance

Supplemental life insurance is purchased in multiples of your annual salary, from 1 to 10 times up to a maximum of $1.5 million. If you are currently enrolled, you may elect a one salary level increase up to $100,000 without completing an Evidence of Insurability (EOI) form unless you were previously declined coverage by Minnesota Life. You are also required to complete an EOI form if you did not elect life insurance when first eligible, or if you elect supplemental life insurance coverage of more than one salary level increase or the increase exceeds $100,000.

Select your coverage level from the drop-down menu, then select Enroll to proceed to the next step in the enrollment process. To view additional information about the supplemental life policy, select Benefit Description.

To waive Supplemental Life Insurance coverage, select Waive this benefit in the upper right corner of the page.

11. Enroll Now – Spousal Life Insurance

You may elect either $12,500 or $25,000 in life insurance coverage for your eligible spouse.

Note: If both you and your spouse are employed by the University, you may not be double-covered. That is, if you are covered under the University’s base life insurance plan, you may not also be covered as a spouse.

Select your coverage level from the drop-down menu, then select Enroll to proceed to the next step in the enrollment process. To view additional information about the spousal life policy, select Benefit Description.

To waive Spousal Life Insurance coverage, select Waive this benefit in the upper right corner of the page.

12. Enroll Now – Child Life Insurance

You may elect either $5,000 or $10,000 in life insurance coverage for your eligible child(ren). If you elect child life and have more than one child, each child must be enrolled at the same coverage level.

Note: If both you and your spouse are employed by the University, one (not both) may insure your eligible child(ren).

Select your coverage level from the drop-down menu, then select Enroll to proceed to the next step in the enrollment process. To view additional information about the child life policy, select Benefit Description.

To waive Child Life Insurance coverage, select Waive this benefit in the upper right corner of the page.

13. Enroll Now – Health Care Flexible Spending Account

The health care flexible spending account allows you to set aside pre-tax dollars (up to $2,550.00 annually) to pay for eligible health care expenses.

Enter the annual coverage amount, then select Enroll to proceed to the next step in the enrollment process. To view additional information about health care flexible spending accounts, select Benefit Description.

Note: the amount shown under “Employee Cost” is prorated based on the number of months left in the current calendar year. This will be the amount that is deducted from your pay check each month.

To waive health care flexible spending, select Waive this benefit in the upper right corner of the page.

14. Enroll Now – Dependent Care Flexible Spending Account

The dependent care flexible spending account allows you to set aside pre-tax dollars (up to $5,000.00 annually) to pay for eligible dependent care expenses so that you or your spouse can work.

Enter the annual coverage amount, then select Enroll to proceed to the next step in the enrollment process. To view additional information about dependent care flexible spending accounts, select Benefit Description.

15. Upload Documents

If enrolling dependents such as a spouse or child, you must present documentation to the Office of Human Resources to verify each dependent’s relationship within 31 days of experiencing a qualifying event. This step should be completed following successful completion of your online benefits change request. Traditionally these documents were to be provided to the Office of Human Resources in person, but now they can also be submitted through MyBenefits. Please review the University’s dependent verification document at https://hr.nd.edu/benefits/group-insurance/dependent-verification-for-university-benefits for a list of acceptable documents. To upload a document, click on the Browse button to locate the scanned document on your local computer. Select the document, then click OK.

Once the document appears in the white box in the middle of the page, click Upload to upload it to the system. You can view the documents that were successfully submitted by clicking the View Uploaded Document(s) button at the bottom of the page. Once complete with your documents, select Save & Continue. Note: Documents will still be accepted in person at the Office of Human Resources.

16. Employee Beneficiary Designation

You are required to designate beneficiaries for your life insurance policies next. If you entered your dependents’ information earlier, they will automatically be provided as options for designation within the drop-down menu.

If you do not have a spouse or child(ren), or wish to name someone else as a beneficiary, you can select the relationship from the available options and provide their contact information.

You have the ability to designate multiple primary beneficiaries. Click on the “+” icon to add another primary beneficiary. Percentages must equal 100% to continue.

You may also designate multiple secondary, or contingent beneficiaries. The process for adding these is the same as adding primary beneficiaries. To begin, select Add Secondary Beneficiary.

If you wish to apply the same beneficiaries from your base life plan to your supplemental life plan, you can select Assign same beneficiaries to all plans. All beneficiary designations will be replicated for you automatically.

When beneficiary designations are complete, select Save and Continue to proceed to the next step in the enrollment process.

17. Confirmation Statement

This page allows you to review both your current benefit elections, and your future pending changes prior to submitting the request to the Office of Human Resources for approval and processing. Please review your elections to ensure accuracy of completion.

Once you have confirmed your elections are correct, select Finish in the bottom left hand side of the page.

Congratulations, your online benefits change request is complete!
