Person Centered Planning

1. Focus Person and Circle of Support develop a plan to achieve outcomes

2. The plan identifies who can help a little or who can help a lot

3. A budget is developed to pay for supports or adaptations

4. Develop a job description and evaluation tool for paid supporters
