MUNICIPALITY COMPLAINT PORTAL
A single point solution to register and track your complaint.
Customer registration page: First time users have to first sign up with the Municipality using the
Customer registration page where the Customer is asked to fill in his details. A unique Consumer id will
be generated once he has registered successfully. Customer will have to login using this consumer id
and his registered password to register his complaint with the Muncipality.
Login Page: To register a complaint a registered customer has to give in the20 digit Consumer id and his
password.
Complain Registration page: Once successfully logged in the Customer will be taken to the Complaint
Registration page where the customer will fill in the required details of his complaint. Once the details
have been successfully entered into the system an unique Complaint registration ID again of 20 digits
will be created and set to the Customers registered mail address.
Complaint Status page: Here the Customer can check the description and status of the complaint.

Inputs:
INPUT FIELDS MANDATORY TYPE COMMENTS
UserName Yes Email id Customer has to use his registered email id as user name in this page
Password Yes String Customer has to enter his registered password to log in
Direction:
For already registered User :Customer has to Enter both the UserName, Password and then press the
SUBMIT button. On successful submission of the username and password the user will
be redirected to the next form that of Complaint registration.
For new user: New user have to first register from the link given below the login window. Please refer
to the link under “New Users Click here to Register”.

Complaint Category
Yes Dropdown Customer has to choose the type of complaint namely they are:
Water Supply
Road Repair
Garbage Cleaing
Complaint Description
Yes String Customer has to type in his complaint in words
Direction: Choose the complaint category from the drop down and type in the complaint description.
The complaint Id will be auto generated and will be displayed on the screen. Also a mail will
be set to your mail having the complaint description along with the unique 20 digit Complaint Id
generated. The user will be redirected to the login page and his session will end.

Complaint Id Number ( 20 digit) A 20 digit unique Complaint id is generated. The complaint id sent to the customer via mail.

Name Yes String Customer has to enter his name in this field
Email Address Yes Email Id Customer has to enter a valid email id here. This email id will be later used as the user name when logging in into this portal. All alerts will be sent to this email id when the customer registers his complaint.
Password Yes String The Customer has to enter the password.
Gender Yes Radio Button The Customer enters his or her gender
Contact Number
Yes Number. (10 digit)
The Customer enters the contact number.
Address Yes String The field has the address of the customer
City Yes String The City of the customer is provided in the field.
Zip Code Yes Number (6 digit)
The zip code is entered in this field.

Verification code
Yes String( 5 letter)
Customer has to enter the 5 letter code displayed beside the field.
Direction: Enter all the above fields according to the description of the fields and then the Submit
button to register the customer. On successful register the customer will be redirected to the login page
where he can register with the email id and password he entered in the registration forward.

After we enter the complaint id and click outside the form. Please note we should not submit the form
at this point. To view details of the complaint we have to just enter tab or click outside the form.

Complaint Id Yes Number (20 digit)
The complaint id for which the Customer wants to check the status.
Direction: The customer enters the complaint id that he has received in the email while registering the
complaint and then clicks outside the form. On clicking outside the form , information of the respective
complaint displayed in the form. Also the information that the complaint has been closed or open is also
displayed. On submitting the form the customer will be redirected to the login page and his session will
end.
