Campbell Clinic Physical Forms

The Physical forms needed for Campbell Clinic are now available on our

website (http://www.sbaeagles.org/formspolicies). All pages must be

filled out and signed prior to arriving at the clinic on Saturday, May 14th,

at 7:45am. In order for Campbell Clinic to perform the physical, a $20

payment is required at the time of service (Please make checks payable

to SBA). This is a great opportunity for any student interested in playing

a sport next year. All athletes must have a physical on file dated after May

1st of this year.

If you prefer to go to your family physician for a physical, those forms

will be available online beginning on Monday, May 16th.

Mother-Daughter Tea News Thanks again to everyone that helped with the planning, preparing,

volunteering and for all of you that blessed us with your generous

contributions for the 2011 Mother Daughter Tea. Despite the Tornado

evacuation, it was a great success and one event that will always be

remembered!

For those of you that took pictures, you will be able to go online to view

and order your prints at: www.carriefolksstrang.com

Click on the online ordering tab and then the SBA Mother Daughter

Tea thumbnail. You must allow pop-ups.

Good luck Seniors!
