MTL Requisition Requirements
• Custom printed for your clinic• Clinic information• Custom panels • Based on “Physician
Custom Profile” form• MUST be filled out
completely and accurately to eliminate delays

• Sample must reach minimum fill line to ensure testing
• POC cups must rest at least 1 minutes in order to accurately register results
• Lid must be SECURELY FASTENED. Failure to secure lid may cause leakage and prevent testing
• Write Patient name and DOB on cup
• Place collection cup in front pouch of specimen bag with absorbent material
• Place requisition, face sheet and copy of insurance card in rear pouch of specimen bag and securely fasten bag closed
• Place secured specimen bags in provided FedEx Clinical Pak
• Affix FedEx shipping label to Clinical Pak

• 12-Panel Point of Care (POC) Rapid Screen cups1 will be provided to clinic upon request. Provider may bill for screening
• Standard Collection cup2 with temperature strip may also be selected
1 – POC cups are billed at $5.00 per kit
2 – Standard cups are offered at no charge
