BPD incidence in infants born at gestational ages 22
through 28 weeks from 1991 to 2011 at the NICHD
Neonatal Research Network Centers.
This twenty-year epoch represents a critical
period in our journey as a subspecialty, one
marked by widespread adoption of antenatal
steroids, broader application of surfactant,
new modes of ventilation, and novel therapies
such as inhaled nitric oxide. These advances
have led to gains, albeit modest for some
gestations, in the overall survival as well as
survival without morbidity. Yet, one morbidity
stands out as a recalcitrant rebel, seemingly
oblivious of the singular attention it has
received from the entire subspecialty:
bronchopulmonary dysplasia (BPD)!

Interpretation The application of surfactant via a thin catheter to spontaneously breathing preterm infants receiving continuous positive airway
pressure reduces the need for mechanical ventilation.
Studiepopulatie : 26 – 27 – 28 weken
• intubated and mechanically ventilated +/- surfactant, if
– severe respiratory distress syndrome or asphyxia
– high FiO2 (30 to 60%)
– low pH (7,15 to 7,20), or high pCO2 (60 to 70 mm Hg)
• spontaneously breathing infants on nasal CPAP and FiO2 > 30%
– a thin catheter (diameter 2・5–5 french) placed in the trachea with use of Magill forceps and a laryngoscope.
– laryngoscope removed surfactant (100 mg/kg bodyweight) instilled intratracheally for 1–3 min
– after instillation, catheter immediately removed

Theoretische voordelen
van de LISA procedure
Minimum een paar mechanische ventilaties zijn gebruikelijk bij surfactant
toediening via intubatie en kunnen reeds longbeschadiging veroorzaken
zoals aangetoond bij dierexperimentele proeven
De LISA techniek onderbreekt CPAP niet en verhindert verlies van PEEP
met potentieel longbeschadiging door atelectase
De spontane ademhaling tijdens de LISA procedure geeft een betere
distributie van het exogene surfactant over beide longvelden

Nadelen LISA procedure
Discomfort :
onaangenaam voor het kind zonder sedatie
geen efficïente back up ventilatie bij gebruik van sedatie
Bradycardie tijdens procedure
Desaturatie tijdens procedure
Reflueren van surfactant, vrije luchtweg
• Surfactanttherapie is een bewezen hoeksteen voor behandeling van
hyaliene membraanziekte
• Ventilator geïnduceerde longbeschadiging
– is een te vermijden factor in het ontstaan van chronisch longlijden van de prematuur
– door mechanische ventilatie tot een minimum te beperken of niet te beademen
• LISA is een attractieve vorm van surfactant therapie waarbij mechanische
ventilatie wordt vermeden met potentieel vermindering van pulmonale
morbiditeit

Besluit
• LISA is het minst succesvol bij de patienten die er het meest baat bij
kunnen hebben (< 28 weken)
• de LISA techniek is een grote uitdaging zowel op medisch als op
verpleegkundig vlak
• of het potentiële voordeel van de LISA techniek de nadelen
rechtvaardigen in vergelijking met de INSURE techniek, is nog niet
onomstotelijk bewezen
