In 2013, the Minnesota Legislature authorized the Minnesota Department of Health to designate hospitals in Minnesota as stroke facilities. View Minnesota State Statutes 144.492-494

Goal: every person in Minnesota has access to timely stroke care, regardless of their location in the state
Destination for potential stroke should be the closest “stroke-ready” hospital or emergency center
Vision: every hospital is equipped and prepared to diagnose and treat ischemic stroke
MINNESOTA STROKE SYSTEM

1. An acute stroke team available or on-call 24 hours a days, seven days a week
2. Written stroke protocols , including triage, stabilization of vital functions, initial diagnostic tests, and use of medications
3. Written plan and letter of cooperation with EMS regarding triage and communication that are consistent with regional patient care procedures
4. Emergency department personnel who are trained in diagnosing and treating acute stroke
5. The capacity to complete basic laboratory tests, electrocardiograms, and chest x-rays 24 hours a day, seven days a week
6. The capacity to perform and interpret brain injury imaging studies 24 hours a day, seven days a week
7. Written protocols that detail available emergent therapies and refl ect current treatment guidelines, which include performance measures and are revised at least annually
8. A neurosurgery coverage plan, call schedule, and a triage/transportation plan;
9. Transfer protocols and agreements for stroke patients10. Designated medical director with experience and expertise in
acute stroke care

Applications will be accepted starting April 1, 2014 for an eff ective date of April 1, 2014.

All hospitals to be designated by January 1, 2015

Obtain a Username and Password: The facility administrator for your hospital has an account
and can set up the designation manager with an account. If you don’t know who your facility administrator is, contact

Facility Administrator to login to the Minnesota Stroke Central/ Minnesota Stroke Registry Tool website with their credentials.

Go to Facility in the main menu and select My User Accounts

Click on Add Staff

Type in the new user’s fi rst and last name, phone number, email and User ID. Check Designation Manager . Check a Facility. Click Submit.

New user to access Minnesota Stroke Central and click on Reset/Request Password.

Type in User ID and click on Send Request.

An email will be sent to the user with a temporary password.
Type in the User ID and temporary password.Click on Log In.

Enter and verify a new password for your account.Click on Save New Password.

Go to the Designation tab in the menu bar.

A maximum of 10 attachments per documentation requirement is allowed
A maximum of 2MB per attachment is allowed
NOTES ABOUT THE APPLICATION
ASRH Criterion 1 - 10 can be completed in any order
The application will automatically save when you attach a fi le.
After clicking on Documentation Complete, you can make changes to your application by deleting fi les or attaching new fi les.
